The aim of the service is to deliver local consultant-Led Community Dermatology & Basal Cell Carcinoma (BCC) services for the diagnosis, management and treatment of dermatology problems for adults and children of all ages which complements and works in partnership with existing and future primary and secondary care dermatology services; and in a manner which ensures that patients receive access to appropriate diagnosis treatment and management in the most effective and efficient ways possible. This includes diagnosis and treatment for low risk BCC. The service will be led by a consultant dermatologist who holds a substantive appointment within an NHS trust, and is listed on the dermatology specialist register at the General Medical Council (GMC). The provider will deliver a comprehensive consultant led community-based dermatology service; able to provide a full local pathway for Leeds patients, of all age groups, who require: - Advice & guidance - Routine assessment & diagnosis - Routine treatment - Follow up and management of suitable long-term conditions - Diagnosis and excision of Low Risk Basal Cell Carcinoma (BCC) The commissioned service will operate in accordance with the agreed citywide dermatology pathways published on Leeds Health Pathways.

To provide a community vasectomy service for patients registered with a GP Practice. The service will deliver an innovative, high quality, efficient service closer to patients' home. The service will: -Offer patients a choice of locations and appointment times, ensuring equitable timely and appropriate access; -Ensure that patients are seen and treated in a safe environment, most appropriate to their care needs; -Provide best value for money, delivering effective and sustainable use of resources. The service will provide on-demand video based clinical consultations which will be available 24/7 The TMS Includes: - Initial clinical assessment by an appropriately trained registered practitioner - Escalation to other specialist clinical resources (e.g., an appropriately qualified consultant, medical registrar, or specialist nurse) where required - Coordination with local clinical resources (district nurses, community teams, ambulance service and the resident's GP) where required - 24/7 clinical assessment and support for all residents who fall to reduce attendance at ED and acute admission - Provision of virtual meeting room to facilitate community service providers and other relevant partners to conduct their MDT meetings - Consultant Pharmacist support on prescribing, deprescribing, mentoring/training, medicines safety and research. Input to developing services responsive to local needs and available as an expert resource. - Clinical support for safer implementation of Medicines Optimisation - Support for the DES Requirements of the Enhanced Health in Care Homes Initiative The principal mechanism for consultations will be video but voice calls and other forms of communication will be used where necessary. All consultations will be recorded via SystmOne into the patient record and a copy of this will be supplied to the GP. Each clinical consultation will entail: - Initial assessment of the person's condition by a registered practitioner - Escalation to a suitably qualified consultant, medical registrar or specialist nurse if required - Administration of a prescription if required - Referral to and/or coordination with the persons GP, district or community nursing team, ambulance service to include any phone calls or video calls required - Provision of advice and guidance to the persons local carers - Clinical supervision of any procedures or other activities undertaken by the local carers at the request of the clinical assessors - Follow-up calls by the TMS team to check on the person's progress - Creation and updating of the electronic patient record to include the TMS clinical consultation and the supply of a copy of that record to the GP. If required CH staff will be offered clinical supervision by the TMS to perform basic procedures.

The Personalised Care Programme are benchmarking our previous and ongoing workforce personalised care training which aims to upskill and support the wider personalised care workforce, including volunteers, throughout 2022/23. Following a successful training programme delivered in 2021/22, which trained over 1000 people throughout the region, this training offer will aim to upskill voluntary sector staff, volunteers, health and social care professionals, care coordinators, health coaches and social prescribing link workers across West Yorkshire in delivering personalised care approached to address inequalities, meet the needs of our most vulnerable communities, connect people to what matters to them and change the relationship between people and professionals. The aim of this benchmarking project is to present an opportunity to invest and upskill our workforce to deliver standardised personalised care approaches to reduce variation in the delivery of care with a focus on people living with long term conditions including mental health. The outcome of this project will provide us a final position, allowing us to demonstrate the impact of embedding personalised care approaches has on employee health and wellbeing by producing robust evaluation mechanisms and will provide opportunities to identify where we can maximise impact through the sharing of good practice. We are inviting bids from academic institutions but are open to considering other organisations with experience of research, to deliver an evaluation, benchmarking our current position, completing data analysis, engaging with our workforce, communications to support the project, and developing a robust evaluation mechanism to understand the impact that personalised care training and development is having on our workforce and communities. This service is to be delivered under an NHS Standard Contract. The contract will have a term of 5 months from the 3rd January 2023. The completion date will be the 31st May 2023. Reducing Health Inequalities around Physical Health Checks for People with Severe Mental Illness programme. The aim of this service is to support a reduction in health inequalities for people with SMI by increasing access to PHC and improving the quality of this experience. This one year, non-recurrent funding from NHS England will increase staffing capacity for targeted work across Calderdale by developing a leadership and training champion role that can consistently increase best practice across the PCN's. This provision will work alongside existing staff, with a particular focus on Primary Care staff. The aim is to support and encourage practices to increase both access to and the quality of the physical health checks to reduce health inequalities for people with SMI. This increased support will encourage practices to use the available health information and co-designed resources, completion of Health Action Plans and to increase tests and interventions that reduce the risk of preventable health conditions. High level objectives of the service are: - To provide leadership and educational approaches to staff delivering the health checks and other practice staff, to increase the uptake and quality of SMI Health checks and Health Action Plans. - To provide training for staff to support sustainable change around best practice in a consistent manner across the multidisciplinary teams in PCN's. - To support consistent use of commissioned resources such as the co-designed practice resource pack, patient invitation letters and films to support best practice across Calderdale. - To support the 21 GP practices in Calderdale and their patients registered with an SMI, to promote a personalised care approach including positive lifestyle changes. - To support work with a range of multi-agency services such as VCSE and Mental health practitioners to promote integrated working to reduce health inequalities. The WY ICB has awarded contracts to Any Qualified Providers (AQP) accredited to provide the consultant-led community ENT/AHL services. The principle aim of the service is to provide a local consultant-led community ENT services for the diagnosis, management, and treatment of ENT problems, which complement and work in partnership with existing primary, community, and secondary care ENT. In addition, by providing a fully integrated Adult Hearing Loss service, the providers will support adults experiencing difficulties with their hearing and communication who might benefit from hearing assessment and care, including the option of trying hearing aids to reduce these difficulties. Details of the requirements are stated in the service specification and can be found as an attachment in the Qualification Envelope in the procurement portal.
