Calderdale Cares Partnership - Community Dermatology Service

Description

Calderdale Cares Partnership is looking to commission a Community Dermatology Service. The Community Dermatology Service for Calderdale which will operate within the context of dermatology service provision across the West Yorkshire Health and Care Partnership. The Calderdale Community Dermatology Service will provide clinical triage assessment and treatment for Calderdale patients, according to need and patient choice. This would provide advice, guidance, diagnosis, care and treatment in the community, closer to home, rather than primary care/GPs referring directly into an acute setting. It will operate through a fully integrated, accessible model of care for people, whilst ensuring the specialist elements of the service are delivered within a high quality and cost-effective framework.
