This Care Model sets out how care home provision in Bath & North East Somerset will be commissioned, delivered and integrated under the recommissioned Care Home Framework. It responds to increasing demand, rising complexity of need, particularly dementia and frailty and growing financial and workforce pressures, while aligning with statutory duties, strategic priorities and system-wide transformation.
