This advert describes the requirements Bridgend County Borough Council (the Council) has for an Older Person’s housing related support service. The contract will consist of the following two elements of service delivery to deliver one seamless service:
Older Person’s Floating Support in the community
&
Support to Extra Care Schemes
According to the most recent Census (2021) the population of Wales is estimated to be 3,107,500. When reviewed most recently in mid-2024, this had increased to 3,187,000. This is mainly as a result of positive net international migration, according to Welsh Government research. People aged 65 years or older were estimated to account for just over a fifth of the total population in Wales in mid-2024 (21.7%, or 693,000 people). The number has increased by 13.2% in the last 10 years, since mid-2014. The number of people aged 75 years or older has increased by just under a quarter during the same period (up 23.7%), from around 275,000 people in mid-2014 to around 340,000 people in mid-2024.
There have also been some stark societal shifts over recent years. For example, the estimated number of people living alone and in need of support has been increasing steadily over the last decade. Today, people aged 65 and over make up 47% of single person households and may need additional support with housing related matters.
This tender exercise seeks to appoint a support provider who will deliver a service offering housing related support to the growing population of vulnerable older people aged 55+. The service provider will be flexible in accepting referrals for those 50+ should demand for the service arise, and capacity allow. The service will develop innovative responses to meet the increased demand and to support people in a different way. The service will help Service Users maintain their independence in the community, including in the county borough’s Sheltered Accommodation schemes as well as in the three Extra Care schemes.
Housing related support enables vulnerable people to sustain their tenancy or home and reduces the need for more intensive and costly support at a later stage. It aims to help people maintain their independence and encourages people to settle back into the community. Support will be solution focussed, aimed at resolving issues and preventing crisis.
This contract period will be for 3 years, commencing on 1st January 2027 with the option to extend for up to 24 months.
The service will assist with the vision of the Housing Support Grant that promotes “A Wales where nobody is homeless and everyone has a safe home where they can flourish and live a fulfilled, active and independent life”.
Furthermore, it will support the Council in fulfilling its duties under the Housing (Wales) Act 2014, Social Services and Wellbeing (Wales) Act 2014 and Well-being of Future Generations (Wales) Act 2015.
The estimated value of this contract will be around £411,000 per annum. However, the estimated value is subject to market feedback and review, so this could change at the point of the formal Invitation to Tender.
The contract is for an accommodation based support service providing high-quality supported housing with 24/7 on-site staffing for men aged 18 years and over with complex, dual diagnosed mental health needs who are homeless or at risk of homelessness, based in the borough of Ealing.
Preliminary Market Engagement for Housing Related Accommodation Based Support for Homeless Households

Leicester City Council (‘the Council”) is preparing to establish a Contract for Families and Singles Temporary Accommodation Services.
This Procurement will be split into 2 Lots:
Lot 1 – Families support package - The Council has had capital funding to purchase 2-3 bed family homes to be used as temporary accommodation for families who are homeless. This requirement is for a suitable Supplier to provide accommodation-based housing support services from the Council-owned building.
They must enter into property agreement with the Council, including payment of market rent and to fully furnish and decorate and maintain the properties for suitable use as temporary accommodation.
Lot 2 – Singles support package - The Council has statutory duties to provide temporary accommodation for eligible households. The Council requires a range of support options for singles/childless couples who have complex needs who it can be hard to place in appropriate accommodation and requires an experienced provider who can successfully deliver this service.

Generic Homeless Housing Support at Emergency Accommodation
The provision of a Generic Homeless Housing Support at Emergency Accommodation. This service will work with individuals residing in the Local Authority's emergency accommodation and support them to have the capability, independence, skills, and confidence to access and maintain a secure and suitable home.
Medium to High Support Supported Housing Services for Young People with Complex Needs
The Royal Borough of Kensington and Chelsea (RBKC) is undertaking preliminary consultation in relation to the delivery of supported accommodation service for homeless young people (aged 16-24) with complex needs for 61 units over 7 sites

This initiative aims to provide a safe, supportive, and nurturing environment for some of the most vulnerable members of our community. The supported accommodation service will be designed to meet the needs of homeless young people (aged 16-24) with a complexity of needs. We are seeking expressions of interest and input from experienced supported housing providers who can deliver a trauma-informed and psychologically informed service.
It is proposed service delivery will be based on a 'Supportive Relational Model’ based on a psychosocial system which supports clients in developing their psychological and social resources to cope with problems they face. This principle has also fostered strengths-based, psychologically informed environment, trauma-informed and co-produced approach, which will deliver the aspirations of its clients, enabling them to achieve their potential for independent living through a time of continued change - which will bring with it challenges and opportunities. The support hours are allocated to clients flexibly depending upon their support needs.

Ealing Council: In house supported living provision – flow and future investment
PCH are seeking a contractor to work with Ealing Council to improve flow through the present in house supported living provision and make the case for future investment.
This would involve:
• Identifying opportunities to improve flow, particularly for people receiving short term support for mental health needs.
• Review the use and effectiveness of unplanned respite care
• Developing a business case for investment in infrastructure and housing stock, including options for updating or redeveloping existing sites.

Home At Last Team (HALT) – Housing Support Service
The primary objective of this commission is to improve long-term recovery outcomes for people in drug treatment by integrating targeted housing support into the wider recovery framework. This initiative stems from the 10-year Drug Strategy, which allocated £53m to address the significant housing instabilities often cited by individuals entering dependency services. By focusing on the delivery of stable housing, this project seeks to remove a primary barrier to successful treatment completion. Following the success of the HALT project, which commenced in 2021, this specification invites providers to bid to provide a Housing Support Service as part of the Home At Last Team (HALT) to support those with substance use issues into structured treatment and long term sustainable accommodation, which fits into our strategic objective of ‘Increasing Treatment Capacity.’ It will seek to reduce the number of rough sleepers with substance use issues who are not currently engaged in structured treatment, to prevent those within treatment who are at risk of homelessness due to their substance use from becoming homeless, and to offer support (professional and peer) to individuals who are within this cohort.

Housing, Health & Wellbeing Hospital Discharge Support Service
The service is to prevent individuals aged 18 and over from being discharged from hospital into street homelessness, and to support safe, timely, and sustainable discharges through coordinated in-hospital intervention and time-limited post-discharge support.
The service will provide proactive, responsive, and coordinated support through:
•Early identification of individuals who are homeless or at risk of homelessness;
•In-hospital assessment, advocacy, and discharge coordination; &
•Time-limited post-discharge support to sustain accommodation and reduce the risk of readmission.
The service will specifically support individuals who:
•Are homeless or at risk of homelessness;
•Have support needs relating to drug and alcohol use; and
•Are known to Adult Social Care services, including those with assessed care and support needs, those open to Adult Social Care teams, and those requiring coordinated discharge planning.
Durham County Council (the Council) are seeking submissions from suitably experienced and skilled organisations to provide the specified services for the Housing, Health & Wellbeing Discharge Support Service.
The service is commissioned in recognition of the significant risks associated with hospital discharge into homelessness, and the adverse impact this has on individual health and wellbeing, as well as system pressures across health, housing, and social care. The Council is committed to ensuring that no individual aged 18 and over is discharged from hospital into street homelessness, and that all discharges are safe, timely, and supported by appropriate accommodation and care pathways.
The service will operate within the framework of statutory duties under the Care Act 2014, including the duty to promote individual wellbeing, prevent, reduce, and delay the need for care and support, and ensure the integration of services where this contributes to improved outcomes. The service will also support the Council in meeting its responsibilities under the Homelessness Reduction Act 2017, including duties to prevent and relieve homelessness and to work collaboratively with partner agencies through the Duty to Refer process.
The service will deliver an integrated model of support spanning in-hospital intervention and time-limited post-discharge assistance, ensuring continuity of care for individuals with complex and multiple needs. It will form a key part of a wider system approach to discharge planning, working in close partnership with Adult Social Care, housing services, health providers, and other relevant stakeholders to facilitate coordinated and person-centred outcomes.
A core feature of this integrated approach is the requirement for the provider to work proactively and collaboratively with Transfer of Care Hubs, clinical teams, and wider hospital-based staff to identify individuals who are homeless or at risk of homelessness at the earliest possible stage of admission. The service will accept and respond to referrals from Transfer of Care Hubs, hospital wards, Emergency Departments, and clinical teams, ensuring timely engagement and intervention to support discharge planning processes.
A requirement of the contract is that staff work flexibly and may need to align as part of a hospital discharge pathway resulting in the need to be co-located from time to time within other relevant commissioned services and embrace opportunities for alignment with the National Health Service Neighbourhood Hubs Programme and integrated working with health, housing and adult social care partners.

Housing Support linked to Short Term Accommodation and Emergency Temporary Accommodation for Lot 3 Eden
4.2. Providers must be Ofsted registered, with Ofsted registered services to host the service within.
4.3. The Authority is seeking a Service Provider with proven track record of delivering supported short term accommodation services and emergency temporary accommodation services for young people.
4.4. Service Providers will:
• Work with young people aged 16 and up to their 18th birthday, or up to age 24 if care experienced, who are homeless or at risk of homelessness
• Work with young people who have complex support needs including mild learning disabilities, high functioning autism; and complex trauma.
• Work with statutory services to provide support services to young people who are homeless or at risk of homelessness;
• Work with young people who have been involved in the criminal justice system, including rehabilitation of offenders;
• Implement person centred approaches to service delivery to enable young people to identify and achieve their goals;
• Deliver support to obtain and sustain suitable independent accommodation;
• Operate in innovative and flexible ways to improve efficiency and the quality and effectiveness of services.
4.5. The Service Provider shall be responsible for maintaining accurate, up-to-date records of all service occupancy, move-in and move-on activity. This shall include, but is not limited to:
• The total number of units available and occupied at any given time;
• Details of young people entering and leaving the service, including dates of admission and departure;
• Reasons for move-on or service exit, including whether move-on was planned, unplanned, or due to enforcement action; and
• Destination of young person upon move-on, where known.

Provision of Housing & Homelessness: Accommodation, Support, and Prevention
Somerset Council is developing future options for its housing and homelessness provision. The aim is to establish a more flexible, person‑centred, and solutions‑focused offer that can better meet the diverse and changing needs of residents. This includes strengthening our homelessness prevention approach and expanding opportunities for social prescribing into appropriate accommodation and support pathways.
As part of this work, the Council intends to establish a Countywide Framework that brings together previously fragmented services into a more coherent, consistent, and coordinated system. This new model will support improved accessibility, clearer pathways, and more equitable provision across Somerset.
We want to move toward a model that enables tailored interventions, promotes independence, and provides responsive support at the right time. Through this market engagement activity, the Council is seeking insight and feedback from providers to help shape a more effective and collaborative system.
This engagement also forms part of the wider re‑commissioning programme, where joint ownership, partnership working, and co‑production with providers and stakeholders - including Somerset Council, commissioned service providers, housing partners & referral agencies, Somerset NHS Foundation Trust, Public Health, VCFSE partners, and people with lived experience - will be key elements of the future design (this list is not exhaustive).
It is anticipated that any new arrangements would commence on 1st April 2028
