To be effective, a clinical expert needs to have a detailed understanding of the health issues affecting people with learning disabilities; as well as of the psychological, psychiatric and forensic models of care and treatment relevant to people who present a range of behaviours of concern that place them at risk of exclusion from ordinary community living. They also need to be well versed in national policy and the complex legal system that either facilitates or blocks progress. This underpinning knowledge is vital to ensure that experts can effectively challenge clinicians involved in a person's care and treatment and provide clear evidence-based rationales for recommendations regarding future treatment. Please note that the service is to be delivered to Leeds patients only and not across West Yorkshire. The aim of the service is to deliver local community hearing loss services across Leeds to include diagnosis and management of hearing loss and supply of hearing aids. This service complements and works in partnership with existing and future primary and secondary care hearing loss services; and in a manner which ensures that patients receive access to appropriate diagnosis, treatment and management in the most effective and efficient ways possible. This should include micro-suction, tinnitus clinics or other specialist clinics, domiciliary visits for bed bound and end of life patients, care homes. We would also expect audiologists to work in support of Dementia pathways and working in collaboration with Learning Disabilities team to ensure equity of access for the bi-annual health check of these patients which will include earlier diagnosis of hearing loss. The service will be led by an experienced Head of Audiology/Healthcare Scientist who holds a substantive appointment within a current accredited service and have had appropriate qualifications for at least 5 years. The contract we intend to offer to the successful bidder at the end of this procurement will be for a duration of 5 years, which is intended to run from 1 April 2023 until 31 March 2028. There would be a possibility of up to 2 extensions of a further 1 year each, subject to any restrictions placed nationally by NHS England or the Department of Health. Current evidence suggests that community services such as these will increase patient choice, improve local access and increase capacity within Health Communities. Vasectomy is a surgical technique employed in order to provide (usually) permanent contraception in males. This notice is to inform the market there is a contract opportunity to provide complex homecare to those eligible for continuing healthcare in the city of Leeds, West Yorkshire. The purpose of the Complex Homecare service is to enable CHC eligible service users to remain in their own home, living as safely and independently as possible. For the purpose of this specification, complex homecare will involve the provision of some or all of the following: - Up to 24 hour care in service users own home - (see section 5.1 for further detail) - Personal Care - Health Care - Maintenance of independent living skills - Domestic tasks including shopping, laundry and meal preparation - Carer support, including night sitting and respite care Complex Homecare Service Users require up to 24 hour care in their own home. The care can normally be provided by Health Care Assistants who develop specific skills to meet individual needs, are working to care plans developed by the Provider's qualified registered nurse and are closely supervised by a trained nurse. Very exceptionally a trained nurse may be required to deliver some of the care required. Examples of complex clinical needs include, but are not limited to: - Mechanical ventilation, suction or use of cough assist - 24 hour invasive and non-invasive ventilation - Multiple disabilities - Tetraplegia and complex spinal injuries - Final stages of some degenerative conditions - Service Users requiring advanced competencies and/or case management by a qualified nurse - Other intensive needs which may require continuous 24 hour care These are in addition to the standard continuing healthcare domiciliary care service. The Personalised Care Programme are benchmarking our previous and ongoing workforce personalised care training which aims to upskill and support the wider personalised care workforce, including volunteers, throughout 2022/23. Following a successful training programme delivered in 2021/22, which trained over 1000 people throughout the region, this training offer will aim to upskill voluntary sector staff, volunteers, health and social care professionals, care coordinators, health coaches and social prescribing link workers across West Yorkshire in delivering personalised care approached to address inequalities, meet the needs of our most vulnerable communities, connect people to what matters to them and change the relationship between people and professionals. The aim of this benchmarking project is to present an opportunity to invest and upskill our workforce to deliver standardised personalised care approaches to reduce variation in the delivery of care with a focus on people living with long term conditions including mental health. The outcome of this project will provide us a final position, allowing us to demonstrate the impact of embedding personalised care approaches has on employee health and wellbeing by producing robust evaluation mechanisms and will provide opportunities to identify where we can maximise impact through the sharing of good practice. We are inviting bids from academic institutions but are open to considering other organisations with experience of research, to deliver an evaluation, benchmarking our current position, completing data analysis, engaging with our workforce, communications to support the project, and developing a robust evaluation mechanism to understand the impact that personalised care training and development is having on our workforce and communities. The service supports commissioners wishing to evaluate a service through either carrying out an evaluation of the service themselves or finding a partner to evaluate some, or all of the service for us. In addition the framework will be open for use by other partners in the West Yorkshire Health and Care Partnership. The domiciliary care service is to enable eligible service users to remain in their own home, living as safely and independently as possible and will involve the provision of some or all of the following: - Personal Care - Health Care (where the Provider has been approved to offer health care level services) - Rehabilitation/Maintenance of independent living skills - Domestic tasks including shopping, laundry and meal preparation - Carer support, including night sitting and respite care - Support for service users at the end of life (these are usually identified as Fast Track funded) **AS THIS IS A DYNAMIC PURCHASING SYSTEM (DPS) THE OPPORTUNITY TO APPLY WILL REMAIN OPEN FOR THE DURATION OF THE DPS. As part of the NHSE inequalities strategy funding "Core 20 plus 5" NHS England » Core20PLUS5 - An approach to reducing health inequalities for children and young people Bradford District and Craven Health and Care Partnership are looking for a provider of a resilience support model in Primary schools. Working as part of the districts "Act as One" approach to children and young people the service will be integrated in to the wider children and young peoples mental health pathways and support the Bradford District Adversity Trauma and Resilience strategy Who we are - Bradford ATR (bradfordatrpartnership.co.uk) This service will support a whole school (Staff, pupils, and their families) approach to preventative models of wellbeing. We will require a provider to offer: - To provide an evidence based preventative resilience program in Primary schools situated in the 20% most deprived communities in the country amounting to approximately 50% of Bradford schools over a 3-year period - To provide full communication with eligible Primary schools ensuring the schools understand the offer of the program and appreciate the demands on the school staff, pupils, and parents/carers - To provide engaging and evidence-based resources for each year group from Reception to Year 6 - To provide initial, continual, and appropriate support and guidance for school staff and leadership in delivering materials to promote resilience to maximise the impact of the program - To provide resources and support for parents/carer and significant others to support embedding the methods in family homes using a range of means to narrow inequalities in access - Provide assured monitoring of the use of approach and materials and regular reporting to the Bradford District and Craven Health and Care partnership and NHSE - Contribute to the wider Adversity, Trauma and Resilience strategy in Bradford district ensuring maximum impact of the program and adopting a service improvement methodology where required - Evaluation of the model to describe and demonstrate the impact of the program on prevention and ultimately decreased demand on specialist children and young people mental health services - Work closely with the leadership of the Living Well schools program to embed the program locally using expertise and guidance of the wider partners Living Well Schools | Make the Healthy Choice | Bradford District (mylivingwell.co.uk) Additional information: If you would like to submit your interest for this service, please submit your response by registering on https://nhswyicb.ukp.app.jaggaer.com/ and complete the requirements included in ITT_64.
