CONFIRMATION OF YOUR EXPRESSION OF INTEREST IS THEREFORE IMPORTANT. • Any responses and information that are shared through this pre-market engagement exercise may be used by the ICB to inform commissioning and potential procurement strategies. SERVICE SUMMARY NHS South West London Integrated Care Board (SWL ICB) is undertaking pre-market engagement to inform the potential commissioning of a targeted prevention and empowerment service for Central Croydon. The programme is designed to address key health inequalities identified through locality population health analysis and will focus on interventions that improve access to healthcare and address wider determinants of health. The proposed service consists of three targeted prevention programmes aligned to the differing population needs of Central East and Central West Croydon: • Central East Croydon - Digital Inclusion and NHS App Uptake Programme Supporting residents aged 40 years and over who experience digital barriers to accessing health services and who are not currently registered with the NHS App. The programme will provide targeted outreach and personalised support to enable residents to access NHS digital services, improve self-management of long-term conditions and strengthen access to healthcare. • Central West Croydon - Welfare, Financial and Housing Advice Programme Providing structured casework support for adults living in IMD 1-3 areas who experience financial or housing insecurity and whose wellbeing is significantly affected by these pressures. The programme will prioritise individuals with moderate-to-high anxiety and either a long-term condition or high primary care utilisation, supporting residents to resolve financial stressors, improve housing stability and strengthen their capacity to manage their health. • Central West Croydon - Childhood Immunisation Equity Programme Delivering culturally competent outreach and navigation support for families from Black and minority ethnic communities with children aged 0-5 who are not up to date with MMR vaccination. The programme will work with communities and GP practices to improve uptake of routine childhood immunisations and reduce inequities in protection from vaccine-preventable disease. Together these interventions aim to improve population health outcomes by strengthening access to NHS services, addressing wider determinants of health, and reducing inequalities in preventative healthcare uptake. The service will operate through targeted outreach, structured behavioural and casework interventions, and coordinated referral pathways with primary care, voluntary sector organisations and local authority services to ensure residents receive timely and appropriate support. The total indicative financial envelope for this service is £209,063 per annum. Further detail regarding the proposed cohorts, delivery model and outcomes framework is set out in the Draft Service Specification provided in Appendix A. SERVICE SUMMARY NHS South West London Integrated Care Board (SWL ICB) is undertaking pre-market engagement to inform the potential commissioning of a NHS High-Risk Communities Targeted Lifestyle Behaviour Service for North Croydon. The proposed service will deliver targeted, evidence-based interventions focused on weight management, smoking cessation and alcohol abstinence for residents at highest risk of preventable long-term conditions and premature mortality. The service will focus specifically on high-risk populations living in North Croydon. The intention is to complement the existing universal prevention offer commissioned by Croydon Council by providing a targeted NHS-funded programme aimed at populations with the greatest unmet need and potential for measurable health impact. The service will deliver structured behavioural interventions, targeted outreach, and coordinated referral pathways with primary care, voluntary sector partners, and existing public health services to ensure residents can access the most appropriate support and that duplication of services is avoided. The programme will contribute to improving cardiometabolic health outcomes, reducing smoking prevalence and alcohol-related harm, and addressing health inequalities in line with NHS Long Term Plan prevention priorities and the Core20PLUS5 approach. The total indicative financial envelope for this service is £184,539 per annum. Further detail regarding the proposed population cohorts, service scope, delivery expectations, and outcome measures is provided in the Draft Service Specification attached as Appendix A. System Alignment and Strategic Context Croydon operates within a collaborative health and care system, with both NHS and Local Authority partners contributing to the prevention and health improvement agenda. The London Borough of Croydon Public Health team is progressing plans to procure a borough-wide, universal Adult Healthy Lifestyle Service. This service is intended to provide broad access to weight management, smoking cessation and alcohol reduction for Croydon adult residents. In parallel, the NHS-commissioned High-Risk Communities Targeted Lifestyle Behaviour Service for North Croydon is being designed as a targeted intervention, focused on residents at higher risk of poor health outcomes, in line with Core20PLUS5 principles and local population health intelligence. These two offers are intended to be complementary and mutually reinforcing, rather than duplicative. The expectation is that providers will: • Understand the distinction between universal and targeted provision • Actively collaborate with Public Health-commissioned services • Establish clear referral, signposting, and escalation pathways between services • Contribute to a coherent, system-wide prevention offer that maximises reach, engagement, and outcomes Providers should therefore consider how their delivery model will align with, and add value to, the wider Croydon prevention landscape, including working in partnership to ensure residents receive the most appropriate level of support based on need. RESPONSE INSTRUCTIONS Organisations should ensure that they have read the supporting information provided prior to responding, noting that the information and any intentions shared may be subject to change (in both form and content) should the ICB decide to procure a new service. CONFIRMATION OF YOUR EXPRESSION OF INTEREST IS THEREFORE IMPORTANT. • Any responses and information that are shared through this pre-market engagement exercise may be used by the ICB to inform commissioning and potential procurement strategies. SERVICE SUMMARY NHS South West London Integrated Care Board (SWL ICB) is undertaking pre-market engagement to inform the potential commissioning of a South Croydon Targeted Weight Management and Wellbeing Behaviours Service. The proposed service comprises three locality-focused prevention programmes designed to address the differing population health needs within South Croydon: • Targeted Weight Management Prevention & Empowerment (South East Croydon) - supporting adults at elevated cardiometabolic risk to achieve sustainable behaviour change and clinically meaningful weight loss. • Carer Strain Support Prevention & Empowerment (South West Croydon) - providing targeted support to unpaid carers experiencing high levels of caring strain. • Loneliness & Social Connection Prevention & Empowerment (South West Croydon) - supporting older residents experiencing significant loneliness or social isolation. The service design reflects differing population health drivers across South Croydon localities. In South East Croydon, higher levels of adult obesity and physical inactivity create a need for targeted weight management support. In South West Croydon, the older population profile and prevalence of long-term conditions create increased demand for carer support and interventions that reduce social isolation. The service will focus on defined high-risk cohorts, including residents living in areas of socioeconomic deprivation, individuals from ethnic groups with elevated cardiometabolic risk at lower BMI thresholds, people living with severe mental illness, unpaid carers experiencing high caring strain, and older residents experiencing loneliness. The programmes will deliver structured behavioural interventions (typically 8-12 weeks), targeted outreach, and coordinated referral pathways with primary care, voluntary sector partners and existing public health services. The service is intended to complement rather than duplicate the Croydon Council commissioned Adult Healthy Behaviours Service, which provides a universal lifestyle offer across the borough. The service will contribute to reducing preventable morbidity and supporting residents to maintain independence by improving cardiometabolic health, reducing carer strain and addressing loneliness. Providers will be expected to demonstrate measurable outcomes using validated indicators such as clinically significant weight loss, reductions in carer strain and improvements in loneliness and mental wellbeing. The total indicative financial envelope for this service is £169,430 per annum covering delivery of all three programme components. Further detail regarding the proposed cohorts, service scope, delivery expectations and outcome measures is provided in the Draft Service Specification attached as Appendix A.

NHS South West London Integrated Care Board (SWL ICB) is undertaking pre-market engagement to inform the potential commissioning of a NHS High-Risk Communities Targeted Lifestyle Behaviour Service for North Croydon. The proposed service will deliver targeted, evidence-based interventions focused on weight management, smoking cessation and alcohol abstinence for residents at highest risk of preventable long-term conditions and premature mortality. The service will focus specifically on high-risk populations living in North Croydon, including individuals living in areas of socioeconomic deprivation, people from ethnic groups with elevated cardiometabolic risk at lower BMI thresholds, and individuals with severe mental illness or existing cardiovascular risk factors.

NHS South West London Integrated Care Board (SWL ICB) is undertaking pre-market engagement to inform the potential commissioning of a South Croydon Targeted Weight Management and Wellbeing Behaviours Service.
