Description of service
The main aim of the Care at Home service is to provide proactive and personalised care within the individual's home and community. This means that the service supplier should work in a person-centred way and respond to a person's changing needs to preventing crisis situations from occurring. This will address the person's social-care related quality of life as well as their wider wellbeing in line with their assessed needs and individual outcomes.
The key principles behind this service will be reducing, preventing, or delaying the need for further care and support, promoting the statutory principle of individual wellbeing and introducing positive behavioural change to encourage independence where possible.

The service will
• prevent youth homelessness by providing safe, stable, high-quality accommodation across the borough.
• deliver personalised, outcome-focused, co-produced support plans that build resilience, independent living skills and progression towards education and employment.
• provide a nurturing, trauma-informed environment that promotes emotional wellbeing, engagement and trust, with early identification and referral for specialist support (including CAMHS) where required.
• support safe mediation with young people and families to restore relationships and enable a safe return home where appropriate.
• promote healthy lifestyles and relationships, including access to information, safe spaces for discussion and routes into support for mental/emotional health, sexual health, substance misuse and exploitation.

People in receipt of supported living services will receive strengths-based support which focuses on their abilities and aspirations.
Contracts will encourage innovation and co-production of services between providers and people in receipt of support
People in Bury will have support which delivers outcomes and promotes independence with consideration of least restrictive approaches and utilisation of assistive technology.

Delivery of Gas Compliance, Repairs, Smoke Detector Testing and Capital Boiler works within Housing Service. There is also a call off option within the Contract being utilised by Facilities Management for the delivery of services in Corporate Buildings.
The contract delivers Landlord Statutory Compliance Services on behalf of the Council.
