Medway Council is seeking to appoint a suitably experienced Care Quality Commission (CQC) registered residential care Provider to provide an assessment bed service from a care home located within Medway's geographical boundary. \r The service will care for working age adults and older people, with or without a dementia.\r The service will provide eight assessment beds, ideally situated on a separate unit/wing within a residential care. Individuals placed into the service will be medically stable but require further support and assessment to determine long term care needs, or short-term support to return to independence. \r The aims of the service are:\r \r • To deliver residential care and support to adults in Medway demonstrating evidence of cognitive impairment linked to the unfamiliarity of their hospital admission or acute episode. It is envisaged the assessment beds will be primarily used for older people with complex needs and or behaviours that challenge.\r \r • To reduce a person's length of stay in hospital and prevent deconditioning by facilitating a safe, planned discharge from hospital into an appropriate care setting.\r \r • To care for individuals in a non-clinical environment for up to six weeks. Allowing people to settle and return to their optimal baseline of health prior to Adult Social Care, or other health professionals' undertaking long term care need assessments.\r \r • To improve the individual's physical and mental health and wellbeing, to a point where they can be safely discharged home, with or without a package of care, or into a long-term care placement. \r \r • To help adults in Medway to live independently for as long as possible.\r \r Procurement documents are available from the Kent Business Portal.\r \r \r \r \r \r \r \r \r \r
