In Ipswich and east Suffolk locally, we have a 2 hour Urgent Community Response (UCR) team known as REACT - Reactive Emergency Assessment Community Team. REACT is a multi-agency team (health, social care and VCS) to reduce avoidable Emergency Department (ED) attendances /admissions by providing short term support for adults (18 years +) in crisis situations. Urgent assessment can be arranged within 2 hours and REACT can access an Interface Geriatrician via the Frailty Assessment Base (FAB) and step up to community beds as needed. It provides a short-term primary, secondary and community care approach for patients with nursing needs, to be treated in their own home and to prevent an acute admission. REACT comprises nurses, occupational therapists, physiotherapists, health care assistants, generic workers, British Red Cross and Suffolk Family Carers support workers.
As part of the Urgent Community Response (UCR), this model aims to enhance the patient experience within the Reactive Emergency Assessment Community Team (REACT) by introducing social prescribing to the multi-disciplinary teams across the 3 hubs in Ipswich, Stowmarket and Woodbridge.
The model will provide additional support and capacity that offers more holistic management of a person's crisis episode and less focus on medical management, leading to a more sustainable and proactive care model.

The Contracting Authority does not intend to hold itself bound by any of the regulations, save those applicable to Schedule 3 Services.

The short breaks offer for children & young people with a diagnosis of a learning disabilities and/or who are autistic with coexisting significant mental health needs, funded by the SNEE ICB, is designed to add additionality to existing short breaks or to provide bespoke short breaks that provide the additional support for the child or young person's mental health needs that will ensure they remain safe and well in their community. The service specification outlines a variety of ways in which Providers could support this service, including but not limited to:
-The delivery of bespoke packages that deliver short breaks,
-Additional intermittent support to ensure that children and young people can continue to access short breaks when their mental health needs increase
-An assessment short break.
-Additional support to allow children and/or young people to access a short break without having to rely on their family to support their mental health needs.
The Suffolk and North East Essex Integrated Care Board would welcome applications from any Provider who could deliver traditional and / or innovative short breaks which would improve the experience of children, young people and their parent, carers and families.
The Contract will be for a term of 36 months (3 years), with the possibility of extending the term for a period of 24 additional months (2 years) beyond the initial contracted duration by agreement between the Authority and the Applicant. The aggregate contract value is £880,000.00. across the potential 5 years, but this is the total potential spend on all short break packages, which will be allocated on an individual placement basis.
