Description

In conjunction with Suffolk County Council the ICB are tendering for a Suffolk Dementia Support Service The overall aim of the Suffolk Dementia Support Service is to empower people concerned with their memory, diagnosed with mild cognitive impairment or dementia across the county of Suffolk (West and Ipswich and East but excluding Waveney), their carers, and wider family, to live as independently, safely, and well, for as long as possible within their communities. The service will be visible and community facing with a proactive focus on promoting early identification, providing a personalised response, and signposting people to appropriate support within their local neighbourhoods. The submission deadline is 12 noon on Friday 23rd August 2024.
