South East London Integrated Care Board (NHS South East London ICB) (hereafter referred to as "The Authority") invites Expressions of Interest (EoIs) and completion of the ITT from all suitable providers, who are capable of delivering the Community Anticoagulation Service. NHS South East London ICB is seeking to procure a comprehensive community anticoagulation service providing initiation of treatment and monitoring of warfarin patients in the community. This will serve patients aged 18 and over and registered with a Bromley GP. Referrals come from primary and secondary care. This contract will be activity led with different appointment rates depending on the types of appointment and numbers of referrals, the indicative value range for the total contract value is between £750,000 and £960,000. During the contract NHS South East London ICB is keen to explore the feasibility of managing switches of treatment from Warfarin to a DOAC where this would be clinically beneficial for the patient. This is currently out of the scope of the current service and therefore out of the scope of this tender process but an area for potential service development. Further details of the Requirements are set out in the Service Specification. The Procurement process will be facilitated entirely online via The Authority's e-procurement portal (ProContract). This service covers South East London and is a dedicated primary care service for older adult residents living in residential and nursing care homes in Southwark. The service will operate under an APMS primary care contract and provide core General Medical Services together with an enhanced service specification for medical support for nursing and residential home residents in an agreed list of Care Homes in Southwark. The service will provide GP services to a cohort of 368 Older People residing in an agreed list Nursing and Care Homes. The indicative contract value is approximately £400,000 per annum. The contract will have an initial term of 5 years (60 months), followed by an optional extension of 5 years (60 months) plus 5 years (60 months) at the discretion of the commissioner. If all available extension is taken up in full the contract will have a maximum duration of 15 years (180 months). SEL ICB wish to request market feedback from potentially interested providers to inform the upcoming procurement process. Outline The service will operate under an APMS or PMS contract and provide core General Medical Services with an emphasis on Neighbourhood Integration and development to improve outcomes for patients registered with the practice and with the wider Primary Care Network. Aims and objectives The aims of the service are to: • Provide a Modern General Practice Model of Care • Deliver all NHS GMS Primary Care Services to registered patients at Queens Road • Work in an integrated way with other primary care services, community services and all acute care services working within Southwark • Collaborate and engage with third sector organisations and other professional groups supporting residents, in order to increase uptake of health interventions to improve health outcomes for registered patients. We would like to invite people to come to an informal event during the morning of February 22nd to work with us to think through what is needed to provide a sustainable MSK Community service for Greenwich which meets the diverse needs of our population. We know that there are some aspects of the current offer that don't work as well as they need to. We now have the opportunity to co-design and commission a new model for the provision of MSK Community services for Greenwich that can address this. We want to shape our thinking in collaboration with a diverse mix of people who will bring different perspectives, ideas and experience, so we are inviting interest from a wide-range of organisations. We must ensure integration with all those providing other services that link to MSK Community services in Greenwich, for example, secondary care Orthopaedic or Rheumatology services, so that the service model is robust and people experience a seamless service when moving between different parts of the system. The Authority (NHS SEL Integrated Care Board) hereby gives notice of its intention to award a contract in accordance with the Provider Selection Regime (PSR) using the Most Suitable Provider (MSP) award route. Under the MSP route, the Authority is not required to run a competitive procurement.
