Project Background The council commissions through contracts the provision of community- based carers breaks services. The Services will support carers caring for an adult aged 18 and over who are ordinarily resident and living in Hertfordshire. This will include older adults (65 plus), adults with learning disabilities, physical disabilities, or mental health conditions. This includes the provision of a preventative service to those who have not had an assessment of need and an ongoing breaks service to those who have been assessed to meet the Council's eligibility criteria for unmet needs. The provision of both services is considered to contain elements of regulated activity therefore it is a requirement that all providers are registered with the Care Quality Commission (C Q C). The provision of personal care is not the primary aim of the service however where incidental care is required during the provision of a break it is expected that the providers are able to support the person with this. Current Position and Scope Currently these two services (preventative and ongoing) are commissioned separately with the preventative service being commissioned as a block service and the ongoing service being commissioned on a call-off basis. The aim of the preventative service is to provide carers with up to 18 hours of support over a 6-week period, whereas ongoing breaks can be commissioned to meet a longer-term need for a breaks service following a carers assessment. The services include a range of provision to enable the carer to take a break from their caring role. This will include but is not limited to: Supporting the person they care for in their own home, supporting the person they care for to access community-based services, supporting the carer and the person they care for to access the wider community. Hertfordshire County Council ('the Council') wishes to invite interested Providers to return the PPME Appendix A- Supplier Questionnaire by 12.00 (noon) Thursday 7th December 2023. The Council will consider the input and responses received as part of its PPME to help inform the Council's options and further decision making regarding the below intended project and to help inform its specification. Project Background Victims who are homeless as a result of domestic abuse have priority need for homeless assistance and suitable accommodation. Local authorities have a duty to secure accommodation for victims of domestic abuse if they are homeless. This includes ensuring there is support specifically for victims with unique and/or complex needs, such as mental health advice and support, drug and alcohol advice and support, as well as signposting accordingly. The Whole Housing Approach (WHA) aims to reduce the number of people who are made homeless as a result of domestic abuse and increase tenancy sustainment options for victims and survivors. A key element of the WHA model is co-located advocacy. Co-located advocacy focuses on victim and survivors' self- identified needs, rather than needs defined by the service. The proposed service would be based on the Domestic Abuse Housing Alliance (DAHA).Co-located advocacy improves access to specialist domestic abuse support for victims and survivors who approach the local authority for housing. It plays a key role in ensuring their safety and meeting individual housing needs. Current Position and Scope The Council currently commissions SADA (Survivors Against Domestic Abuse) to deliver the housing navigator pilot. The Housing Navigator Pilot is Hertfordshire's localised version of housing advocacy. Housing navigators work closely with the ten districts and boroughs in Hertfordshire's housing teams and domestic abuse providers to offer housing-related support and advice to people experiencing domestic abuse. This can include advice on housing options, safety planning and risk management. The Housing Navigator Pilot will end on the 30th June 2024. It is anticipated that the Co-located Housing Advocacy service will commence on the 01 July 2024. The Council will act as the lead commissioner for a range of funding partners, which will include Hertfordshire's ten District Councils. Scope going forward The Council wants to commission a countywide service to support victims and survivors of domestic abuse, to access safe and sustainable housing through housing advocacy. Hertfordshire County Council (the 'Council') acting as lead commissioner on behalf of District funding partners wishes to procure a countywide housing advocacy service to support victims and survivors of domestic abuse who have a housing need, to access safe and suitable accommodation. The service will also provide domestic abuse training to housing teams. The service will be based on the Domestic Abuse Housing Alliance (D A H A) housing advocacy model and forms part of a Whole Housing Approach. The aim of the service will be to increase housing stability and safety for all victims and survivors of domestic abuse. Housing advocates will be co-located with district and borough housing teams and will be that conduit between victims/survivors who have a housing need and the districts, through providing housing advocacy. The advocates will also be embedded in the local multi-disciplinary teams supporting homeless adults experiencing multiple disadvantages. The current service arrangements, which is running as a pilot, expires on 30 June 2024 The anticipated total annual joint expenditure for this Service is approximately. GBP 200,000.00. Services support those at the early stages of progressive memory loss, after diagnosis, those with advancing dementia, those preparing to move into residential care and people still living in the community at end of life. Hertfordshire County Council (the 'Council') acting as lead commissioner on behalf of District funding partners wishes to procure a countywide Framework Agreement for a carer breaks service to support and provide respite for carers in Hertfordshire. Service Providers will be required to be registered with the Quality Care Commission (C Q C) and to maintain that registration throughout the duration of the Framework. The Service Provider must be able to demonstrate a commitment to maintaining and delivering high quality services for adults with a variety of needs and/or conditions and provide services where all aspects of a person's care are met. The Service Provider must provide services that meet the needs of the individual and are provided by competent staff in a way that supports the safety and security of the person supported. The Services will be responsive, reliable and maintain a person's dignity and respect at all times- be accessible and delivered with understanding and without discrimination. The Services will support Carers caring for an adult aged 18 and over who are ordinarily resident and living in Hertfordshire. -The Breaks Services- shall be open to anyone meeting these criteria and who has been assessed to meet the Council's eligibility criteria for unmet needs. This will include older adults (65 plus), adults with learning disabilities, physical disabilities, autism or people who have other Neurodiversity, or mental health conditions (including people who experience suicidal ideation). The services will include a range of provision to enable the carer to take a break from their caring role. This will include but is not limited to supporting the person they care for in their own home, supporting the person they care for to access community-based services, supporting the Carer and the person they care for to access the wider community. The Service aim is to allow people to live as independently as possible in their own home and communities. The Services will be pivotal in preventing people from going into bed-based services such as residential homes, hospital, or other formal care settings and to support Carers to continue in their caring role for as long as they wish to do so. The Service Provider will ensure that their delivery of Carers support services is part of a network of services, which is underpinned by partnership working with a wide range of agencies. N H S Talking Therapies counselling services provide N I C E recommended psychological interventions for adults and older adults with anxiety disorders and / or depression. The review of data received from N H S Talking Therapies counselling services indicates that a significant number of people who choose N H S Talking Therapies report feeling more able to cope and an improved health and wellbeing at the end of their intervention. Currently approximately one in two people who have a course of treatment in N H S Talking Therapies recover and two out of three people show worthwhile improvements in their mental health. Project Background The nationally led evidence based N H S Talking Therapies programme has been offered in Hertfordshire since 2008, previously called Improving Access to Psychological Therapies and known as I A P T. In 2023 the I A P T programme was renamed N H S Talking Therapies for anxiety and depression to facilitate public recognition and clarify the clinical conditions that are treated in the services. The Council currently commissions on behalf of the Herts and West Essex I C B for the delivery of N H S Talking Therapies counselling services with six (6) providers working across Hertfordshire. The Council is looking to commence a procurement as the current arrangements for N H S Talking Therapies Counselling Services delivered under a Hertfordshire County Council framework expire in January 2025. This framework contributes to the I C B's N H S Talking Therapies programme.
