Hartlepool Borough Council wishes to invite registered domiciliary care providers to tender for two contracts for the provision of domiciliary care services in Hartlepool. \r

The primary objective of the service is to enable people living in the Hartlepool community to remain living at home for as long as possible and maintain a good quality of life which meets their identified outcomes. People who use the service should be able to achieve and maintain their maximum levels of independence and self-care allowing them to live more independently with dignity. \r

The providers will be expected to show a commitment to partnership working, quality assurance and continuous improvement. Performance will be monitored through self-assessment and the departments monitoring process for domiciliary care providers. \r

The two contracts relate to geographical areas within Hartlepool, with one designated service provider in each area having specific contractual responsibilities. \r
\r
Utilisation within the areas has remained fairly static with average hours delivered in each area in the region in the last 12 months of around 1900 - 2100 per week. The number of users within each geographical area range between 400 - 540 over ther last year.\r

The Council will allocate the geographical area to the successful providers.\r

The successful providers will be required to accept all referrals from the Council for any service user within their contracted geographical area unless following assessment they are unable to meet the persons needs. The service must be provided within 48 (forty eight) hours of receipt of the referral. There may be occasion where referrals are made to providers outside of their geographical area. Acceptance of these referrals will be at the providers discretion. \r

In line with the Public Health England and Brook guidance 'C-Card condom distribution schemes why, what and how' and NICE guideline NG68 April 2017, the Provider will deliver, co-ordinate and manage a safe, easily accessible and affordable condom distribution scheme, to meet the needs of resident young people aged 13-24 years in Hartlepool. This will include building knowledge and challenging social norms among young people about carrying and using condoms.
The Provider is required to be the lead agency coordinating the delivery of the C-Card service offer, advertising and promoting the scheme; and ensuring young people in Hartlepool recognise the C-Card brand. The scheme must target high-risk groups including those engaging or likely to engage in risky behaviours.
Additionally, the Provider is required to develop and deliver training to a variety of health professionals and non-health professionals to be C-Card assessors and distributors. The Provider should train staff on how to use the system effectively to provide condoms, condom demonstration, information on sexual health and signposting to other services relating to the C-card scheme.
