The anticipated value of the Service is a maximum of £343,952.00 per annum and the contract duration will be 3 years from 28th September 2022, plus an optional (option of the Commissioner) extension period of up to a further 24 months. Key Aims and Objectives of the Service The High Intensity User Service (HIUS) is based on the 'Blackpool model' developed by Blackpool CCG, as published in the Commissioning for Value Casebook, February 2015. The model offers a robust way of reducing HIU activity to EDs, 999, NHS 111, Primary Care services as well as unscheduled hospital admissions. The HIU service liberates front line resources to focus on those who need it most and helps reduce costs. The objective of the service is to work closely with identified frequent users of emergency urgent departments, with an aim to reduce unheralded attendances at ED and associated non-elective (NEL) admissions, whilst improving the service user's quality of life and outcomes. This will be delivered by: • Identifying patients who frequently attend ED and associated non-elective (NEL) admissions • Identifying patients who are evidentially on a trajectory to becoming a high intensity user of emergency services • Proactively connecting and engaging with the patient • Supporting the patient to manage their needs using a personalised approach. Sussex NHS Commissioners are seeking bids from suitably qualified providers for the provision of a place-based Medicines Optimisation in Care Homes (MOCH) service as part of the Enhanced Health in Care Homes Programme (EHCH). The three places in Sussex are Brighton & Hove, West Sussex and East Sussex. Contract start date = 01 February 2023 Contract end date = 31 January 2026 (plus Commissioner option for 2 years extension to 31 January 2028) This contract is divided into lots: Yes. The service will ensure • A MOCH pharmacist and pharmacy technician will be available to care homes and the wider EHCH Multi-Disciplinary Team for support and advice Monday-Friday (excluding bank holidays) during business hours. • Rational and safe use of medicines, this can be achieved through but is not limited to Structured Medication Reviews (SMRs). • Appropriate medicines optimisation and medicines use to support the reduction of workload of General Practitioners (GPs) and other MDT members. • That residents and/or their families/representatives or carers are given the opportunity to be fully involved in decisions about their medicines • Lead on the implementation of safe medicines optimisation systems in care homes that also support waste reduction initiatives and medicines inefficiency. • Support for residents and care home staff as part of the transfer of care process including timely identification of residents requiring medicines optimisation support when moving between health and social care providers and sectors. In addition, MOCH teams will be expected to provide strategic leadership to their MDT, place and wider EHCH Programme Board including: • Advising on the processes for safe transfer of care around medicines use when residents are moving between sectors. • Supporting care homes to identify and improve their medicines optimisation processes, systems, and training needs for care home staff. • Supporting pharmacy workforce development - including the provision of Pharmacist and Pharmacy Technician trainee rotations for the specialism. • In collaboration with system partners lead innovation for the development and implementation of effective MOCH initiatives across Sussex. Additional information: The successful Bidder will offer a MOCH service to the residents of care homes that are within the boundaries of the Brighton & Hove, West Sussex or East Sussex places. NHS Sussex ICB intend to award a contract to Arch CIC following engagement with providers. The current contract with Arch CIC is due to expire on 31st March 2023. This justification relating to the direct award of this 84-month contract will cover security and stability of provision for a service in a market with limited provider interest. The current contract was awarded by NHS Brighton and Hove CCG (now NHS Sussex ICB) following the publication of a Prior Information Notice (PIN) as a Call for Competition on 023488-2021 (Ref number). This interim arrangement was put in place to enable service continuity whilst the CCG took time to build a better understanding of the potential market interest in providing the service in the longer term. Subsequent to the publication of this PIN an RFI was issued to seek interest, with the incumbent being the only provider to respond. In order for Sussex ICB to deliver against this agenda, the ICB intends to direct award (effectively extending the existing contract) to Arch CIC. The rationale and evidence to support this decision is contained in the section below. The extension will be for a period of 84 months from 1st April 2023. The direct award will satisfy secure high quality, safe, effective, and efficient provision of a difficult to procure service with limited market interest. The stability of the service via a longer-term award will enable greater engagement from the provider in partnership discussions for the strategic development of the service and interactions with other related services.
