Post health check GP appointments To develop/use an escalation criteria to identify patients who would benefit from GP intervention following an NHS health check. Where the criteria is met, patients will be offered a GP appointment/s where the following will be undertaken: • Initiation of treatment • Stabilisation of treatment • Health promotion 3. Access to complimentary services To work with other services to ensure that patients have the opportunity to access complimentary services where appropriate. This contract is to carry out a pilot and will not extend beyond 6 months duration. Once concluded, an evaluation of the pilot will be used to inform future service design and delivery. The pilot and evaluation needs to be completed within the 2025/2026 financial year. This is a new contract to carry out a pilot of a new model to inform future service design and delivery.
