North East London ICB wishes to procure a Dementia Navigation, Information and Support Service for City and Hackney. Dementia is one of the main causes of disability in later life. Characterised by progressive memory loss, behavioural and personality changes, impaired reasoning and ability to care for oneself, it progresses over time, and in the later stages of the disease people become increasingly frail, may have difficulty eating and swallowing, experience incontinence and lose communication skills such as speech and become increasingly dependent on others. The Dementia Service is a joint collaboration between the successful bidder and the lead NHS Mental Health Trust organisation. The service being tendered is for the Dementia Navigation Service, which is part of the overall Dementia Service and comprises of Dementia Navigators supported by a Team of professionals from the NHS lead Organization which includes Consultants (Psychiatrist and Psycho-Geriatrician), Nurses, Psychologists, Occupational Therapists, and Administrators. The Dementia Navigation, information and Support Service will work as an integral part of the Dementia Service (DS) to support City and Hackney residents aged 18 years and over with dementia, their carers, families and friends. The purpose of the dementia navigation, information and support service is to enable people diagnosed with dementia and their carers to come to terms with their diagnosis, reduce social isolation and live well with dementia.

The NHS commissioning authorities in London will re-procure the existing pan-London NHS CHC in nursing homes AQP framework. The current framework is due to expire on 31 March 2023. Applications are open from Monday 17 October 2022 to Friday 2 December 2022 through the CarePulse eProcurement system (https://london-nhaqp-2023.carepulse.co.uk/). Successful Providers will be added to the Pan-London Nursing Homes AQP list. This AQP list is used by the commissioning authorities as the primary route of commissioning NHS Continuing Healthcare placements for adults. The services will be delivered in CQC registered Nursing Homes. The Contract does not guarantee any volume of activity or payment to the Provider. There is no minimum number of packages that the Provider can expect to receive through the contract. Expressions of Interest (EOI) for the provision of Hospice Palliative and end of life care for Children and Young People. NHS North East London are seeking to commission a community-based Hospice and palliative care service for Children and Young People. Timely, high quality palliative and end of life care to be provided to eligible children, young people and their families. Key services required include: -hospice based day and overnight respite; step down care; symptom management; end of life care; hospice at home to include respite and end of life care; post death care and bereavement support. Personalised care planning and review, integrated working with statutory and community partners, development of transition support, and a focus on equity of access are key features of the service. Please refer to the draft service specification document for further information on the service. NHS North East London is considering options for making future arrangements for these services beyond March 2023 and at the present time is looking to directly award contracts to hospice providers depending on the outcome of the NHS North East London's review and any organisation coming forward in response to this notice. The following requirements should be considered by providers: •The provider must be community based within North East London •Offer appropriate locations to provide the full range of commissioned services across North East London from 1st April 2023. •Demonstrate the management and clinical experience to provide all the elements of the service from 1st April 2023 •Have a track record of integrating pathways and services with local acute and community providers in North East London. •Quality - the provider will be expected to provide care in accordance with national clinical guidelines (NICE etc) and achieve improved outcomes for children and families The current combined annual caseload across the two hospices of children and young people receiving care (not including post bereavement support to families) is approximately 450. NHS North East London currently have the following contracts in place, which are partially funded by the service providers to provide the above services: There are two lots: • LOT A - Inner North East London • LOT B - Outer North East London The contract is going to be for 5 years with an option to extend for an additional 2 years. The total contract value if extended is £7,280,000. (£1,040,000. per annum). Contract start date is 1st April 2023. - LOT A - Inner NEL = £460,000 per Annum - LOT B - Outer NEL = £580,000 per Annum Additional information: NHS uplifts will be applied year on year dependent on NHS allocations and guidance. The ICB may, from time to time, be in receipt of Non Recurrent monies and providers would be expected to work with us to deploy funding, and deliver activities alongside the core contract. The ICB currently has NHS matched funding in addition to the core contract values; there may be a recurrent element from 2024 onwards but this is to be confirmed. Providers are expected to be CQC registered and be a member of Hospice UK. The emotional support service provides trauma informed support to children, young people and appropriate family members living in North East London who disclose historic/ non acute child sexual abuse. The service will work closely with the wider Child Sexual Abuse (CSA) service team to ensure a joined up an integrated approach to addressing the needs of children and young people. The service is currently peripatetic and provided across NEL with young people being seen at the location of their choice, although ESS practitioners may also see young people in the clinic alongside the paediatrician and other clinic staff. Where appropriate and agreed with the young people support could be provide virtually or via a 'blended' offer.

NHS NEL ICB is looking to procure Raising awareness of Lung cancer symptoms in City and Hackney Services Cancer is a leading cause of mortality and morbidity in Hackney and the City, much of which is preventable. Around a third of all deaths in Hackney and the City are attributed to cancer, with breast, prostate, lung and colorectal cancers in particular contributing substantially to this burden. It is well established that early recognition of cancer symptoms, and therefore earlier help seeking, diagnosis and treatment, can lead to improved survival and patient experience. Raising awareness of cancer symptoms is therefore a priority for action locally The activity covered by this specification will focus on increasing awareness Lung cancer symptoms amongst individuals in Hackney and the City and subsequently earlier help seeking and diagnosis. The overall aim of the service is to improve earlier diagnosis of Lung cancer in Hackney and the City through raising awareness of Lung cancer symptoms and encouraging earlier engagement with healthcare services. The project will also aim to collaborate with a range of community groups, health and social services (including GP practices and Local Authority provision) to enhance the public's knowledge and understanding of lung cancer symptoms. The main aim of the Take Home and Settle Service is to safely support patients’ discharge from hospital. Short term support is provided in the person’s home or other interim accommodation in order to support independence and prevent readmission to hospital. The service will enable individuals to regain confidence, motivation and skills to self-manage, and encourages them to engage with the local community to improve socialisation and wellbeing. The service will provide 7-day support, working closely with health and social care to deliver integrated and co-ordinated care to adults. Service recipients will be registered with a C&H or Tower Hamlets GP and/or residing in these Boroughs who have been admitted to or are at risk of admission to the following hospitals: • Homerton Healthcare NHS Foundation Trust • Royal London Hospital (Bart's Health NHS Trust) Patients at other hospitals out of borough but who live within Hackney or Tower Hamlets may also be referred to the service through our local discharge/transfer of care hubs. These will be a small number of the overall eligible residents. The key aims of this service will be to: • Prevent unnecessary admissions through A&E • Proactively engage with Ward and Discharge Teams to facilitate early, safe and sustainable discharge reducing length of stay and delayed transfers of care from the hospitals listed above • Reduce avoidable re-admissions over the short period following discharge period • Work closely with health and social care services to improve patient experience • Signpost and refer to community services and support with the aim of enabling patients to remain independent at home wherever possible.

Improving health and care outcomes for the homeless population is a key priority of the ICB, the BHR places and Local Authorities. All are committed to addressing inequalities experienced by homeless people and transforming the way in which homeless health and care is delivered locally. This service specification details the ICB's commissioning intention for a single, integrated, proactive (including outreach), accessible model of care for homeless people, aligned with the strategies of the ICB and the London Boroughs of Barking & Dagenham, Havering and Redbridge. The intention is to commission a multidisciplinary model supporting integrated care. A concept central to effective care because many homeless people present with multiple health and care needs. Evidence shows using peer advocacy and a partnership approach to addressing health and care needs of homeless people improves the effectiveness of care delivery.
