N&W ICB's overall Population Health Management strategy requires analysis at various levels and enable access to and use of PHM information at all levels, across NHS and wider partners within the Integrated Care System (ICS) to support a strategically aligned approach to Population Health.
The PHM digital solution being sought must be secure, complying with all Information Governance requirements.

This is a notice to inform the market the procurement has been abandoned.<br/><br/>NHS Norfolk & Waveney ICB (NWICB) invited bids for the Provision of a Learning Disability Respite Service for People aged 18+ with a learning disability, who are under the duty of care of NHS Continuing Healthcare (CHC) and are registered with a GP within Norfolk & Waveney.<br/><br/>The successful Provider will deliver a five-bed residential respite service with nursing care for adults with learning disabilities and complex health needs.<br/><br/>The service will be provided 24 hours per day, seven days a week including days, evenings and weekends, and will be provided over a full 52 weeks per year, as defined and detailed in the Essential Standards set by the Care Quality Commission (CQC). Except for closure for staff training (4 days per year) and Christmas closure arrangements to be agreed with the nominated liaison team manager who oversees referrals to the service.

This notice is an intention to award notice under the Provider Selection Regime competitive process.<br/><br/>NHS Norfolk and Waveney Integrated Care Board NHS Norfolk and Waveney Integrated Care Board invited suitably qualified and experienced providers to submit competitive bids for the provision of Therapeutic support for survivors of sexual abuse or violence.<br/><br/>The aim of the service is to provide tailored and evidence-based specialist therapeutic support and treatment to survivors of sexual abuse and violence in Norfolk and Waveney.<br/><br/>This procurement is subject to the following lots<br/><br/>Lot 1 - Therapeutic support for survivors of sexual abuse or violence - Aims to provide tailored and evidence-based specialist therapeutic support and treatment to survivors of sexual abuse and violence in Norfolk and Waveney.<br/><br/>This contract will be procured on the basis of an initial 3-year contract period, with an option to extend for a further 2-years (5-years in total) subject to Commissioner discretion and requirements.<br/><br/>Lot 1 has an initial annual contract value of £225,000, £675,000 over the three year contract term and £1,125,000 over the maximum 5 year contract term.<br/><br/>An additional £150,000.00 per year may be available at the discretion of the ICB depending on demand and financial availability – this is not a guaranteed available sum.<br/><br/>Lot 2: £100,000 over the full 4 year contract period. 6 Provision of CHC Fast Track Services for Nursing Home Beds
This notice is an intention to award notice under the Provider Selection Regime competitive process.<br/><br/>The purpose of this Service is to deliver care within a Residential Care Home with Nursing (referred to from this point as a Nursing Home) for those patients who have been assessed as eligible for NHS Fast Track funded care, have expressed a preference for care in a Nursing Home and whose needs can be safely and appropriately managed in a Nursing Home. The Service is commissioned to provide dignity and assurance to people and their families when they are entering their final weeks of life and is expected to be safe, responsive and efficient. This includes enabling patients to achieve the best possible quality of life whatever their disease or disability, gender, ethnic background, or sexual orientation.<br/><br/>This will be achieved through the application of a holistic person-centred approach to care which is integrated seamlessly with all other health services, especially GP primary medical services and specialist palliative care services.<br/><br/>The aims of this Service are:<br/>• To deliver a responsive, caring and person-centred service for patients and their loved ones in the last weeks of life.<br/>• To facilitate prompt commencement of care in a Nursing Home, for patients identified as in the end stages of life under the NHS Continuing Healthcare Fast Track pathway.<br/>• To commission end of life care which is delivered by staff with enhanced levels of training and competencies in relation to end of life care.<br/>• To prevent inappropriate or avoidable admissions to hospital for end-of-life care.<br/><br/>Please note that all Lots are restricted to a maximum of 2 beds per nursing home, except for Lot 2 (which is 3 beds). You must be able to deliver the maximum number of beds stated for the Lot you are bidding for. Bids received with less than the maximum or more than the maximum stated Beds will be excluded. Provision of Hypertension and Lipids Optimisation Project
This is a notice to inform the market the procurement has been abandoned.<br/><br/>NHS Norfolk and Waveney Integrated Care Board sought to procure a Hypertension and Lipids Optimisation Project. The successful provider staff was expected to work with the ICB Cardiovascular disease team, the N&W Protect NoW team, and General Practice to improve clinical effectiveness through case finding and management of patients.<br/><br/>NHS Norfolk and Waveney Integrated Care Board sought to launch a large-scale CVD Prevention project using a Population Health Management approach to case finding and intervention. The project will focus on hypertension and lipid management, with patients being identified and subsequently risk-stratified using the ICB software.<br/><br/>The ABCDE program aims to improve the care of patients at risk of Cardiovascular events through case finding and improved management of high-risk individuals. This project is considered a system priority to see our population lead healthier, longer lives, whilst simultaneously reducing pressures on the health system through early detection and management.<br/><br/>The successful provider is was expected to supply and manage Clinical Prescribers who will liaise with the ICB Protect NoW team, General Practice, and patients. A personalised care approach will be used to support pharmacological and social prescribing for our patient cohort.<br/>Prescribers must be able to initiate relevant medicines.<br/>Prescribers must initiate medicines in line with N&W criteria.<br/><br/>Patients will be identified using the ICB’s population health management software (Eclipse). The ICB Protect NoW Team will contact patients via digital (and print, if necessary) mediums to inform them of their relative risks and changes they can make in their lifestyles and diets and to encourage medication adherence where necessary.<br/><br/>Patients identified as eligible for medication will be consented by Protect NoW and subsequently made available to the provider, and the prescribers are to systematically contact this patient cohort and seek to initiate and / or optimise treatment in line with ICB and national guidance.<br/><br/>The provider will be expected to liaise with the relevant Practice where required on necessary ongoing management and monitoring.
