As identified in the NHS Long Term Plan, NHS 2023/24 Priorities, Operational Planning Guidance and the Delivery Plan for Recovering Urgent and Emergency Care Services there is a national expectation for all Integrated Care System’s to have an Urgent Community Response service. Kirklees Urgent Community Response is comprised of four contracts, one with each of an alliance of providers and delivers urgent community-based services to the population of Kirklees.
Aim of the Service
• To provide Primary schools in Bradford and Craven with resources, training and support to offer a whole school approach to equipping children with the skills required to build wellbeing and resilience and prevent long term harm.
• To provide an evidence based preventative resilience program in Primary schools situated in the 20% most deprived communities in the country amounting to approximately 50% of Bradford schools
The contract for Kirklees Community Services (previously known as Care Close to Home) covers a wide range of community-based services delivered to the population of Kirklees. The contract is based on a lead provider model with the contract holder being responsible to either provide the services directly or in collaboration with other providers of services.
