The aim of this service is to provide the clinical infrastructure to support the safe, timely and appropriate dispensing of antiviral medication for the treatment of Covid-19 for non-hospitalised patients across Suffolk and North East Essex. The service is designed to ensure choice and prompt access to these medicines in the community setting for eligible patients when they are required at all times whilst the pharmacies are open. Each provider will stock a minimum of THREE packs of each antiviral medication (or higher based on local demand) and will make a commitment to ensure eligible patients have prompt access to these medicines to facilitate treatment within 5 days of the onset of symptoms. Patients will receive a full clinical review by a SNEE Covid Management Delivery Unit (CMDU) prescriber and if eligible for treatment, an EPS prescription will be sent to the commissioned community pharmacy, in line with the consent and choice of the patient.
Pharmacists will contact the patient as soon as possible upon receipt of the prescription to confirm consent and clinical suitability for treatment. This discussion is recorded on an electronic platform confirming all checks have been performed to confirm appropriateness of treatment. As appropriate, the pharmacy will make arrangement with the patient/carer for the medication to be collected or delivered.
The service provides an initial health assessment and subsequent access to mainstream services for marginalised and vulnerable adults such as Migrants, Refugees and Asylum Seekers; homeless people, Gypsies and Travellers, Sex Workers, and Ex-offenders/Offenders. To deliver a culturally aware, culturally sensitive service and culturally competent service that demonstrates appropriate behaviours, attitudes, policies, processes and structures which enable effective working across marginalised and vulnerable communities. Sharing knowledge and expertise regarding cultural competence across the system.
To support and assist marginalised and vulnerable adults to overcome barriers in accessing mainstream services including Integrated Neighbourhood Areas (e.g. Connect), providing advocacy, client support, staff training and partnership working to remove barriers which reduce access to primary and secondary services. Provide initial urgent care and support as required. To work collaboratively with GP Practices and support new registrations from the highlighted groups and ensure people with complex needs are supported flexibly depending on need. Ensure liaison and working with partner agencies to provide an integrated approach to the health and social care needs of MVA and avoid service duplication, including substance misuse services.
The service shall be required to be flexible and take into account the changing evidence base in order to maintain best practice and provide high service. The service shall continuously measure improved outcomes for service users, and, through memorandums of understanding with other providers across the system, be able to clearly describe where service users have been on their pathway.
The green social prescribing non-medical intervention support for people living with or facing health inequalities or complex needs is an integral component of the Community Based model across IES aiming to reduce health inequalities. It focuses on prevention and enables our communities to support and promote health and wellbeing for specific Core20Plus5 identified priorities - hypertension and chronic respiratory diseases. This is a two year programme, joining up a range of community, primary care, secondary care and VCFSE prevention programmes into a Community Based Approach focusing on improving diagnosis rates of hypertension to reduce cardiac events, raising awareness of mitigations for modifiable behaviours and risk factors, offering a range of non-medical interventions based on referral model from primary care, secondary care, INTs and wider Community Champions and anchor institutions.

The programme provides community-based support, personalised to the individual, using a "what matters to me" approach to create a personalised care and support plan with the person to help improve health and wellbeing by addressing wider determinants of health.

The IPS service is for adults with mental health needs and adults who have vocational needs or aspirations to gain employment. The target group for these services are for adults with mental health needs, those who are receiving support through secondary mental health services or have a diagnosed Serious Mental Health Illness. Suffolk IPS team delivered by Essex Partnership Foundation University Trust (EPUT) but embedded within Norfolk and Suffolk NHS Foundation Trust (NSFT) clinical treatment teams.

This service will be for north east Essex based GP practices, who face a range of significant challenges in the day to day delivery of primary medical care. This service will reactively support invited practices, via quality improvement methodologies, to build their capability and resilience. Support is also available to practices wishing to proactively achieve quality improvement. There will be a focus on collaborative working with other local support providers, to achieve a co-ordinated approach that optimises resources.

The IUC contract in Suffolk and North East Essex is provided by Practice Plus Group (PPG). The contract expires on 31st October 2025, having been extended by two years. No further options to extend the existing contract are available.
The IUC contract includes our 111 service, Clinical Advice Service (CAS) and Out of Hours (OoH). All elements of the contract are delivered directly by PPG with the exception of the OoH service in East and West Suffolk, which is provided via a sub-contracting arrangement with the Suffolk GP Federation (SGPF).

Intention is to commission a specialised bereavement counselling service that meets the needs of women or the birthing person and their partners, who are experiencing mild to moderate mental health distress because of pregnancy-related bereavement or baby loss. Access to the service will be provided via professional referral and self-referral. The service is accessible following a stillbirth, second trimester miscarriage, terminations for medical reasons, recurrent miscarriages, neonatal loss, birth trauma, pregnancy after loss, and early miscarriage (by professional referral only). A fixed number of counselling sessions will be provided either virtually or face-to-face based on parent choice.
