NHS South West London is seeking to appoint suitable providers to a Provision of Patient Choice Fertility Services Any Qualified Provider (AQP) agreement. The aim of the procurement is to is to provide a range of appropriate assisted conception treatment services for patients and their partners if they have one (following fertility assessment) who are registered with a GP practice in South West London and who meet the eligibility criteria set out with SWL ICB Evidence Based Interventions (EBI) policy 2023. The intention is to set up an AQP agreement with providers to offer patient choice to these services. The Contract to be offered to the Preferred Applicants at the end of this Procurement will be for a duration of 3 years, which is intended to run from approximately 1st April 2024 (contract start date to be agreed) until approximately 31st March 2027 with the possibility of contract extensions of up to a total of 24 months, subject to any restrictions placed nationally by NHS England or the Department of Health and Social Care. There are two key elements in this ophthalmology pathway for which SWL ICB is looking to commission an appropriate provider(s); Lot 1 - a community eyecare service to manage and deliver minor eyecare services in community and primary care optometry, and Lot 2 - a secondary care single point of referral management system.

NHS South West London ICB is seeking to procure five recovery/crisis cafes located in Sutton, Croydon, Wandsworth, Merton, Kingston & Richmond. The Recovery /Crisis Cafés provide a safe space, non-clinical, and welcoming spaces for individuals experiencing mental health distress to receive immediate support and de-escalation, particularly during evenings and weekends when other services may be unavailable. These services offer a vital alternative to emergency departments for adults (18+) in mental health crisis across Southwest London. SWL ICB's crisis café model aims to support early intervention, reduce ED presentations and improve access to out-of-hours support.

NHS South West London Integrated Care Board (ICB) (Croydon Place) is seeking to commission Long-Term Conditions (LTC) Community Outreach, Expert Patient Programme (EPP), LTC Self-Management and Pentathlon Healthy Lifestyle Programme Services. Since 2020, the LTC Community Outreach and Expert Patients Programme has been focussing on prevention, early identification and improved management of LTCs in Croydon with the aim to raise awareness with people who do not regularly access GPs or local NHS services. With the population in Croydon with two or more long term conditions at 23% and set to increase, a collaborative approach across all long-term conditions is key to manage, support and prevent increases in the number of Long-Term Conditions and the costs associated with that to health and the individual. The aims of the LTC community outreach and EPP programme are to deliver cost-effective, culturally competent, and preventative community health interventions that address the health inequalities in Black, Asian and Minority Ethnic (BAME) communities which constitute circa 50% of SWL ICB's core20 communities, particularly in deprived and underserved wards such as Thornton Heath and New Addington. In particular the LTC community out-reach health and wellbeing checks, Pentathlon healthy lifestyle courses and Expert Patient Programme (EPP) LTC self-management course are anticipated to: • Support raising grass roots awareness of LTC risk and risk factors and support people to adopt healthier lifestyles to reduce the risks of LTC, support early identification and diagnosis and self-management in key Croydon communities. • Empower patients with LTCs to feel more confident in managing their own condition and keep them active and engaged in their lives. • Tackle health inequalities and increase uptake from hard-to-reach groups. NHS SWL ICB is seeking to commission Healthy Communities Service ("the Services"). The objectives of this procurement are to: • Put LCPs at the centre of developing local Community Delivery Plans that address each localities priorities in relation to health inequalities and our residents' health and wellbeing and • Help to make the LCPs representative of Croydon - from the larger charities to BME organisations, grassroots groups, faith communities and local residents; as well as health representatives from local neighbourhood health general practice services; • Nurture information sharing, collaborative working and a strengths-based approach in the localities; • Partnership development with all stakeholders, including primary care services, care homes. Three events are held annually in each of the 6 LCPs, which are one of the main interfaces between the VCFS and the statutory sector. These events bring active communities into the heart of Croydon's Locality Operating Model, which are designed to empower local people by connecting them with their neighbours and community-based services, thereby increasing their independence and reducing their reliance on statutory services. The Provider will work collaboratively with the geographically relevant PCN or general practice (see Appendix 1) neighbourhood health leads to ensure their attendance at the relevant LCP quarterly review meetings. Where commissioners require locality-based insight or feedback on a specific topic, policy area or programme of work, this will be communicated to the Provider and relevant Local Community Partnerships. Subject to reasonable notice and agenda planning, the subsequent LCP meeting agenda will seek to include the topic for discussion to enable community-led insight, feedback and recommendations to be gathered. Feedback captured through LCP discussions will be documented and shared with commissioners to inform commissioning decisions, service design and system priorities. The Provider will also work with the geographically relevant PCN or general practice neighbourhood health leads to ensure that the development and implementation of the LCP Community Plans are informed by general practice or PCN lead feedback, supporting alignment with the evolving neighbourhood health model of care. Locality Operating Model: Figure 1: Croydon's Locality Operating Model Through regular LCP events, population health data analytics, local intelligence and community insight are reviewed to identify locality-specific needs. In Quarter 1 of 2026/27, each Local Community Partnership (LCP) will propose three priority areas for action, focused on improving the health and wellbeing outcomes of their locality and reducing health inequalities informed by ICB provided locality specific population health data. The ICB will provide locality specific population health data by February 2026 which will be used by LCPs to interrogate data and underpin the evidence base upon which each of three priorities 2026/27 Local Community Delivery Plan will be based on. For each locality, these three priorities will be set out within a Local Community Delivery Plan, which will demonstrate clearly how the agreed priorities will be addressed through targeted activity and partnership working. There will be one Delivery Plan for each of Croydon's six localities, and these plans will be used to monitor progress. The Community Delivery Plans are live documents and will be reviewed and updated at each LCP meeting to reflect emerging data, insights and changing local needs. LCPs will be expected to maintain a strong understanding of health inequalities within their locality and to support the wider system in designing and delivering initiatives that improve residents' health and wellbeing. Over the coming year, LCP Co-Chairs will continue to expand engagement and participation, with the aim of strengthening LCPs as a key voice and delivery mechanism for residents and communities. Ongoing support and development for Co-Chairs will be required to enable them to fulfil this role effectively.
