The NHS SEL ICB is seeking expressions of interest for the delivery of peer-led structured education courses for people living with more than one long term condition(s) mental health courses and courses for carers in the London Borough of Southwark. . The Service will be for people registered with a General Practitioner or resident within Southwark. Services will be provided in accessible locations across the borough to ensure equitable access for all Southwark residents. The service will also be available via an online platform. This will be a hybrid model of face to face and online model. Personalised care is one of the five key priorities within the NHS Long Term Plan, which states that 'people will get more control over their own health and more personalised care when they need it.' Supported self-management and peer-led structured education is part of the NHS Long Term Plan's commitment to make personalised care the norm. The SEL Integrated Care System (ICS) Development Plan (last revised in March 2022) sets out the next chapter in the development of an 'integrated care system' for South East London. The ICS is a partnership and has identified the six system-wide priorities for improving care: • Preventing ill-health and supporting wellbeing; • Compassionate, whole person care, delivered in community wherever possible; • Rapid access to high quality specialist services when people need them; • Joined up care across health and other public services; • Addressing health inequalities; • Building resilient communities. Self-management support / peer -led structured education is a key element in the first and second priority and enables the last three priorities. Self-management is considered to be effective in improving health outcomes and is supported by clinical studies. Supporting and educating people to look after themselves improves their motivation, the extent to which they eat well and exercise, management of their symptoms, and clinical outcomes; and is evidenced to reduce their use of NHS resources. Following the programme, participants will have a better understanding of how they can support themselves as active self-managers. The contract will be offered for a period of two and a half years (2 ½ years). The contract length will be a maximum of 2 ½ years. South-East London Integrated Care Board (SEL ICB) wish to commission the following • an Urgent Treatment Service (UTC) that includes an out-of-hours service • a GP Out of Hours Home Visiting Service The intention is to commission the two services in separate lots. The tenders may be submitted for one or for both services. Greenwich UTC – Lot 1 The borough of Greenwich has one UTC which is co-located within the Emergency Department (ED) at the Queen Elizabeth Hospital (QEH Woolwich). It will be open 24 hours a day seven days a week. Out-of-Hours Home Visiting – Lot 2 The service will be expected to treat and care for people with an urgent care need in their own home, thereby reducing the number of ambulance call outs and reducing the need for hospital admission. The service provides education and training to general practice on how to identify and appropriately refer potential victims of domestic abuse, following the recognised evidence-based IRIS model. The service also receives referrals categorised as low risk and provides support to victims of domestic abuse. Mental illness covers a wide range of conditions such as depression, anxiety disorders and obsessive-compulsive disorders, through to more severe conditions like schizophrenia. Common mental disorders (CMD) include conditions such as depression and anxiety. Results from the 2014 Adult Psychiatric Morbidity Survey (APMS) show that 1 in 6 adults had a common mental disorder (CMD) in the week prior to the survey, rising to almost in 1 in 5 adults in London. Mental ill-health represents a significant burden on our local population and the health and care system. It is estimated that almost 47,600 adults in Southwark are currently experiencing a CMD, this is expected to rise to approximately 52,000 individuals over the next decade Although less disabling than major psychiatric disorders, the higher prevalence of CMD mean that their cumulative cost to society is greater. All types of CMD are more prevalent in women than among men, and they are also more likely to experience more severe symptoms. Young women were identified as a particularly high-risk group with an estimated 5,600 cases in the borough. Employment and socio-economic status were identified as a substantial risk factor. In Southwark, approximately half of the claims for employment and support allowance (ESA) are related to mental health - with 6,000 cases in February 2016. The aims of this long-term supported living service are to enable service users with enduring mental health needs to live the community and function at an optimal level of independence, with appropriate levels of support. The Service will therefore play an essential role in meeting the accommodation and support needs of people who may have not lived independently or succeeded in living independently previously. In particular, the Service will meet the needs of individuals who are leaving hospital or residential care services. A key aim of this service is to support residents with complex needs to live as independently as possible, developing the skills necessary to remain living in the community.

For those individuals who are assessed as eligible for AACC, the NHS will fund services to meet their assessed needs. The service provides a stock of Mobility Equipment (and manages the ongoing maintenance of equipment on that List) to support residents in achieving the best possible independence in their day-to-day living. The stock of Mobility Equipment includes postural seating, seating cushions and further flexibility in the client’s preferences for their wheelchair. This is referred to as Personal Wheelchair Budgets-PWBs- or (interchangeably), Personal Health Budgets-PHBs. The total activity, which includes clinical assessment and review, provision of equipment, spare parts, collection and scrapping of equipment, is universally referred to as the “Wheelchair Service”. South East London Integrated Care Board serves an area that comprises of six local authorities and a number of NHS Trusts. At present, the neurodevelopmental assessment pathways vary in each borough. All boroughs experience challenges and concerns about lengthy and increasing wait times for autism assessments. Furthermore, localities experience a range of associated challenges in relation to issues such as pre- and post-diagnostic support and children & young people with additional mental health needs. Many of these issues combine and coalesce for a growing, specific cohort of young people: those young people who do not access full time education. <br/><br/>Missing school has a significant, negative impact on children’s academic skills; well-being; mental health and can lead to adverse life outcomes. School attendance provides a safeguard for many children. Persistent school absence removes this safeguard. In addition, the challenges to the system outlined previously means that this cohort of children & young people can remain unsupported for many years. <br/><br/>In all 6 boroughs covered by the ICB, there are growing numbers of children who are unable to attend school due to social emotional and mental health reasons. Social communication difficulties that are typical in presentations such as such as Autism Spectrum Condition (ASC) are frequently associated with negative school experiences and linked to higher rates of school non-attendance. A study conducted by Totsika et.al, found that 43% of autistic children experienced persistent absenteeism during the study period. <br/><br/>This project aims to support children & young people who are identified as emotionally based school avoiders by offering an autism assessment and, most importantly, developing a need led personalised plan to support reintegration of the child or young person into school. <br/><br/>This project aims to screen children who are persistently absent from school for Autistic Spectrum Conditions. The SEL LDA commissioning team will work with the school inclusion teams in all 6 boroughs of SEL, identify 90 children and young people (CYP) who have low school attendance due to social or emotional distress and undertake a NICE approved assessment for autism. Each C/YP will then be provided with a a written, personalised support plan that the provider will deliver via support workers working directly with the C/YP, their family, their school and community services.
