South-East London Integrated Care Board (SEL ICB) wish to commission the following • an Urgent Treatment Service (UTC) that includes an out-of-hours service • a GP Out of Hours Home Visiting Service The intention is to commission the two services in separate lots. Greenwich UTC – Lot 1 The borough of Greenwich has one UTC which is co-located within the Emergency Department (ED) at the Queen Elizabeth Hospital (QEH Woolwich). It will be open 24 hours a day seven days a week. Out-of-Hours Home Visiting – Lot 2 The service will be expected to treat and care for people with an urgent care need in their own home, thereby reducing the number of ambulance call outs and reducing the need for hospital admission. The provider will be expected to work closely with the NHS 111-CAS provider, who will be the source of referrals into the service. This service will provide urgent home visits when GP surgeries are typically closed, from 6.30pm to 8am weekdays, all day at weekends and all day on Bank Holidays. NHS South East London ICB (SEL ICB) is seeking to commission the new All Age Continuing Care (AACC) Partnership Delivery Service (PDS) for Bromley CHC. The service will work alongside the ICB in-house AACC team to deliver the totality of requirements for the service. The PDS will be a clinically-led service that will deliver critical CHC nurse-led assessments and reviews, in support of the completion of Multi-Disciplinary Team (MDT) Decision Support Tools (DSTs). NHS South East London ICB is seeking expression of interest (EOIs) from capable, qualified and interested providers to provide an all-age wheelchair service which includes local clinics, wheelchair clinical assessment, clinical review, equipment supply, delivery & collection; personal wheelchair budget options; repair and maintenance of NHS wheelchairs; emergency/urgent response. The service provides a stock of Mobility Equipment (and manages the ongoing maintenance of equipment on that List) to support residents in achieving the best possible independence in their day-to-day living. The stock of Mobility Equipment includes postural seating, seating cushions and further flexibility in the client’s preferences for their wheelchair. This is referred to as Personal Wheelchair Budgets-PWBs- or (interchangeably), Personal Health Budgets-PHBs. The total activity, which includes clinical assessment and review, provision of equipment, spare parts, collection and scrapping of equipment, is universally referred to as the “Wheelchair Service”. The contract will be offered for a period of three years 13 months, with an option to extend for an additional 24 months (3 years and 1 month in total) at the Authority’s sole discretion. South East London Integrated Care Board serves an area that comprises of six local authorities and a number of NHS Trusts. At present, the neurodevelopmental assessment pathways vary in each borough. All boroughs experience challenges and concerns about lengthy and increasing wait times for autism assessments. Furthermore, localities experience a range of associated challenges in relation to issues such as pre- and post-diagnostic support and children & young people with additional mental health needs. Many of these issues combine and coalesce for a growing, specific cohort of young people: those young people who do not access full time education. <br/><br/>Missing school has a significant, negative impact on children’s academic skills; well-being; mental health and can lead to adverse life outcomes. School attendance provides a safeguard for many children. Persistent school absence removes this safeguard. In addition, the challenges to the system outlined previously means that this cohort of children & young people can remain unsupported for many years. <br/><br/>In all 6 boroughs covered by the ICB, there are growing numbers of children who are unable to attend school due to social emotional and mental health reasons. Social communication difficulties that are typical in presentations such as such as Autism Spectrum Condition (ASC) are frequently associated with negative school experiences and linked to higher rates of school non-attendance. A study conducted by Totsika et.al, found that 43% of autistic children experienced persistent absenteeism during the study period. <br/><br/>This project aims to support children & young people who are identified as emotionally based school avoiders by offering an autism assessment and, most importantly, developing a need led personalised plan to support reintegration of the child or young person into school. <br/><br/>This project aims to screen children who are persistently absent from school for Autistic Spectrum Conditions. The SEL LDA commissioning team will work with the school inclusion teams in all 6 boroughs of SEL, identify 90 children and young people (CYP) who have low school attendance due to social or emotional distress and undertake a NICE approved assessment for autism. Each C/YP will then be provided with a a written, personalised support plan that the provider will deliver via support workers working directly with the C/YP, their family, their school and community services.

South East London Integrated Care Board (SEL ICB) and Southwark Council are jointly recommissioning the Southwark Wellbeing Hub - a core part of the borough's mental health infrastructure. This recommissioning marks a strategic transformation of the Hub, evolving it from a primarily signposting service into a person-centred, community-led model that embeds culturally competent provision, expands one-to-one and group support, and integrates with emerging Integrated Neighbourhood Teams (INTs) and SLAM's Community Care Development programme. The NHS Long Term Plan (LTP, 2019) emphasises the ability of NHS Talking Therapies services to provide effective support for people with depression and anxiety disorders that can be managed effectively in a uni-professional context. This service is for adults with multiple long term conditions, over the age of 18, who either live in the London Borough of Bexley or are registered with a Bexley GP practice. Following the publication of the NHS 10 Year Plan, Bexley Place is driving forward with establishing a Neighbourhood Health Service in each of its three neighbourhoods. A fundamental component of a neighbourhood health service is to support residents with the wider determinants of health which have an impact on how they are able to manage their long term conditions. The population in Bexley is diverse, both culturally and economically. The borough is split into 3 Neighbourhoods, these are: • Frognal (Sidcup & Bexley) • Clocktower (Bexleyheath & Welling) • North Bexley (Erith, Thamesmead & Abbey Wood) There are approximately 26,000 people living in Bexley who have 3 or more long term conditions. North Bexley is the most culturally and economically diverse part of the borough, with 15,995 residents living in the lowest 20% deprivation decile - this is 15% of the North Bexley population. In the other parts of the borough, the number of residents living in the lowest 20% deprivation decile is 2,774, which is 2% of that population. People living in the most deprived 20% of Bexley have a shorter life expectancy than those living in the least deprived 20%. For females, the gap is 2.2 years, and for males the gap is 5.2 years. Bexley Place seeks an established provider of voluntary services based in the London Borough of Bexley, with a proven track record in currently delivering social prescribing and peer support services. The provider must be an existing key partner of the integrated neighbourhood teams' development in the London Borough of Bexley. The provider must be able to mobilise the Health & Wellbeing Coaching with Social Prescribing Service within 3 months of the contract award. NHS South East London (SEL) Integrated Care Board (ICB) (hereafter referred to as the "The Relevant Authority"), invites Providers to submit Expressions of Interest (EOI) for the delivery of the Community Anticoagulation Service (Warfarin only) in the Royal London Borough of Greenwich.<br/><br/>The service will be closer to patients' homes, using less invasive point-of-care testing. This allows a one-stop service including initiation, monitoring and optimisation of warfarin treatment for patients who are registered with a Greenwich GP Practice. The service must be delivered at a site within the Royal London Borough of Greenwich, or at the patient's home (if they are housebound). This community service aims to release the pressure of Queen Elizabeth Hospital anticoagulation clinic so that the hospital service is reserved for managing complex patients who require the multidisciplinary discussion between different specialists. The point-of-care test (using a finger pricking device) will reduce the burden on pathology laboratory for processing venous sample and improve patient experience with less venepuncture.<br/><br/>Warfarin has a narrow therapeutic window, and is affected by dietary intake of food and alcohol, as well as other medication. It is considered as a high risk medication. Patients who are on warfarin require regular blood test monitoring to ensure the international normalised ratio (INR) is within therapeutic range. Out of range INR leads to complications such as bleeding or blood clots. Providers involved in delivery of the service require additional training and have their competency signed off to meet the requirement of their role. Providers must be adequately trained to provide effective education and support to patients and carers. Providers must also ensure that there is a mechanism for ongoing continued professional development for all professionals involved in an anticoagulation service. <br/><br/>In order to provide a high quality and safe care to patients who are on warfarin, Providers will need to have robust governance and quality mechanisms put in place, including subscription to computer clinical decision support software (CDSS), access to a point-of-care testing device such as Coagucheck® and register with an external quality assurance scheme such as the UK National External Quality Assessment Service (NEQAS), therefore Providers are expected to receive adequate tariff to reimburse for their service. <br/><br/>This is a cost and volume contract, with no minimum activity guarantee.<br/><br/>Tariffs:<br/>£60 for a new patient warfarin initiation appointment<br/>£30 per INR monitoring appointment<br/>£15 supplement for home visits

The event will be held at the London Borough of Bexley (LBB) Civic offices Bexleyheath DA6 7AT This will be an informal event to share the vision of Bexley's future UTC in the context of the wider urgent care services offer and receive feedback from potential providers regarding the draft specification and other aspects of the planned procurement. NHS South East London Integrated Care Board (NHS SEL ICB, the Authority) has stated its intention to commission a model of Urgent Care service for Bexley residents. Procurement and Contracting Hub hosted by NHS North East London Integrated Care Board is managing this market engagement exercise on behalf of the Authority. There are currently 2 standalone Urgent Treatment Centres (UTC) across the borough • One standalone unit in North Bexley at Erith Hospital • One standalone unit at Queen Mary's Hospital Sidcup There are currently 2 local primary care access hubs however this may change with the introduction of the Primary care Network Contract Enhanced Services (DES). Within Bexley the vision is to create an urgent and emergency care system (one system multiple facilities) that can deliver equitable access to the right care first time for local patients through a networked model with services provided along robust pathways 24/7. This is an opportunity for a genuine integrated Urgent and Emergency (UEC) Care service, aligning NHS 111, London Ambulance Service (LAS), the Intermediate Care Services, Primary care, GP out-of- hours, GP extended hours hubs, and Emergency Departments. The new urgent treatment centre service will be delivered from the existing 2 standalone sites. Across the borough these are situated in North Bexley at Erith Hospital and at Queen Mary's Hospital Sidcup, alongside and in conjunction with local primary care access hubs. Additional information: The expectation of the 2 standalone UTC's in Bexley is that one will be open 24hrs a day/ 7 days a week (Queen Mary's Hospital) and the other will be open 13hrs a day/7 days a week (Erith Hospital). The UTC will include open access patients and will include scheduled or appointed patients (e.g. via NHS 111) with urgent need. It is expected that the urgent treatment centre model will be fully implemented in Bexley by April 2023. Bexley's Local Care Partnership (LCP) involves the NHS, local authority, service providers, charities and voluntary sector coming together to work in partnership, to design health and care services around the concept of 'place'. The LCP is continuing to develop and test its draft model of care requirements and invites potentially interested providers of urgent care services to help test aspects of the new service requirements and potential commercial options. Commissioners are seeking feedback from interested organisations in the form of completed Market Engagement Questionnaires (MEQ). One in four people in the UK will suffer a mental health problem in the course of a year. Mental illness covers a wide range of conditions such as depression, anxiety disorders and obsessive-compulsive disorders, through to more severe conditions like schizophrenia. Common mental disorders (CMD) include conditions such as depression and anxiety. Results from the 2014 Adult Psychiatric Morbidity Survey (APMS) show that 1 in 6 adults had a common mental disorder (CMD) in the week prior to the survey, rising to almost in 1 in 5 adults in London. Mental ill-health represents a significant burden on our local population and the health and care system. It is estimated that almost 47,600 adults in Southwark are currently experiencing a CMD, this is expected to rise to approximately 52,000 individuals over the next decade Although less disabling than major psychiatric disorders, the higher prevalence of CMD mean that their cumulative cost to society is greater. All types of CMD are more prevalent in women than among men, and they are also more likely to experience more severe symptoms. Young women were identified as a particularly high-risk group with an estimated 5,600 cases in the borough. Employment and socio-economic status were identified as a substantial risk factor. In Southwark, approximately half of the claims for employment and support allowance (ESA) are related to mental health - with 6,000 cases in February 2016. The aims of this long-term supported living service are to enable service users with enduring mental health needs to live the community and function at an optimal level of independence, with appropriate levels of support. The Service will therefore play an essential role in meeting the accommodation and support needs of people who may have not lived independently or succeeded in living independently previously. In particular, the Service will meet the needs of individuals who are leaving hospital or residential care services. A key aim of this service is to support residents with complex needs to live as independently as possible, developing the skills necessary to remain living in the community.

NHS South East London Integrated Care Board (SEL ICB) is seeking to commission a Neighbourhood Homeless Health Hub Service for the London Borough of Bromley. This service builds on the successful three-year Homeless Health Project partnership between Bromley GP Alliance (BGPA) and Bromley Homeless Charity. It aims to provide trauma-informed, integrated, and accessible healthcare to people experiencing homelessness (PEH), addressing significant health inequalities and barriers to care. The service will operate through a hub-and-spoke model, offering drop-in clinics, longer appointments, and outreach services. It will support urgent and routine care, chronic disease management, mental health support, safeguarding, and referrals to wider health and social care services. Key objectives include: • Delivering trauma-informed care in safe, non-judgemental environments. • Reducing A&E attendances and hospital readmissions. • Improving access to primary care and long-term condition management. • Supporting strategic integration with local authority, mental health, and acute services. • Providing a replicable model for addressing health inequalities in PEH. The term of the contract will be for 3 years. The value of the contract excluding VAT is estimated to be £162,000 for the first year, increasing annually to £176,000 for year 3.
