NHS South East London ICB is seeking to procure a comprehensive community anticoagulation service providing initiation of treatment and monitoring of warfarin patients in the community. This will serve patients aged 18 and over and registered with a Bromley GP. Referrals come from primary and secondary care. This contract will be activity led with different appointment rates depending on the types of appointment and numbers of referrals, the indicative value range for the total contract value is between £750,000 and £960,000. During the contract NHS South East London ICB is keen to explore the feasibility of managing switches of treatment from Warfarin to a DOAC where this would be clinically beneficial for the patient. This is currently out of the scope of the current service and therefore out of the scope of this tender process but an area for potential service development. The aim of the programme continues to be reducing the reliance on inpatient beds, by effectively preventing admissions and building community capacity to support discharge from hospital. How we ensure this aim is achieved is therefore important and the SEL LDA User and Parent Forum is a key partner in shaping and co-producing requirements. By 'inpatient units' we mean specialist hospital or other unit used by people with learning disability, mental health hospital for adults or children, specialist ward for people with autism and assessment and treatment units. The LDA Engagement and Events project will support the overall SEL LDA programme objectives of improving the care and lives of people with learning disability and/or autism. NHS South East London ICB is seeking to commission Intermediate Care Beds Services. The service will provide a 14 Intermediate Care Beds to support a responsive and deliverable bed based rehabilitation service, within the London Borough of Lewisham, 24 hours a day, 365 days a year. The main service outcome will be to deliver a 14 bed based Intermediate Care rehabilitation and reablement service. The service is for adults aged 18 years and upwards. It will have capacity to support at least 4 double handed patients at any one time though the provider will be expected to give consideration to supporting a higher number of people with initial double handed support needs at point of hospital discharge. There will also need to be at least 2 beds designed and allocated to meet the needs of Bariatric patients. Patients will predominantly be referred as part of the hospital discharge pathway. However, there will also be a ‘step up’ pathway to support avoidable hospital admissions where bed based rehabilitation would deliver the same or better results. The step up referral might be appropriate for the following types of patients: • Those that have coped well at home in the past but are beginning to demonstrate a marked reduction in their level of functioning. Referrals will most likely be made by a healthcare professional and screened by the Urgent Care Response Team. • Patients identified within the Emergency Department (ED) and Urgent Care Centre (UCC) who are deemed suitable with appropriate Rehabilitation Goals. In these circumstances, a written discharge passport will be provided through the Trusted Assessor at the ED or UCC.

The main aim of the Child Sexual Abuse Emotional Wellbeing Support Services is to provide trauma-informed and child centred emotional support and advocacy to CYP victims of CSA, improving wellbeing, self-efficacy and reducing traumatisation. The early emotional support service will provide trauma-informed, child centred support to CYP victims of CSA including advocacy, case management, and symptom management. The number of sessions offered will be based on need and could be up to 15 sessions. This will be offered to CYP and their families across the 6 boroughs of SEL. The early emotional support service will work with the paediatric teams in each of the 6 boroughs as one team, providing a holistic health review focused on the needs of the child/young person and their family. Please note, not all boroughs in SEL have local CSA paediatric teams, and therefore the early emotional support provider will work with borough level teams and the CYP havens as needed.
