NHS Somerset are seeking expressions of interest from organisations to provide an asynchronous and synchronous IT solution for the provision of Advice and Guidance between practitioners and clinicians for use by all within their respective Integrated Care System. A robust and effective Advice and Guidance digital platform solution is central to the vision outlined in the NHS Long Term Plan of 'one click away' specialist advice and guidance for primary care. The Commissioner requires the provision to support senior clinician advice and guidance for acute care pre-admission/pre referral communications between Primary care (including community and paramedic staff) and Secondary care for use across both Urgent and Planned Care pathways, and to enable uploading and sending documents and images from mobile devices/laptops to support patient outcome and ease of access to the health care providers. The contract will be let for an initial period of 2 years, with optional extension of up to a further 2 years, to a maximum total length of 4 years. The value for the initial 1st year term is £252,000 and the maximum whole life (2 year) value is £504,000. All values provided are inclusive of any non-recoverable VAT. Three partners of the Somerset Integrated Care System, Somerset Integrated Care Board, Somerset NHS Foundation Trust, and Yeovil District Hospital NHS Foundation Trust are seeking the provision of Non-Emergency Patient Transport Services (NEPTS) in the following two categories which will be procured as two separate Lots within a single process. Lot 1 - Patient Discharge and (inter-hospital) Transfers (PD&T) Lot 2 - Planned NEPTS Journeys (Planned) The County of Somerset covers a large geographical area. It has a dispersed, mainly rural population of approximately 564,000 people, with only one in four of its residents living in one of its three largest towns; Taunton; Yeovil; and Bridgwater. Somerset has two Acute Trusts within its borders (Somerset NHS Foundation Trust and Yeovil District Hospital NHS Foundation Trust) as well as 12 community hospitals (run by Somerset NHS Foundation Trust), 64 GP practices, a hospice and several care homes. The Somerset population also relies on access to services in a few neighbouring out of county Trusts. These journeys, referred to as Zone B journeys, are generally for services which are more specialist in nature. Full details for each of the Lots including specification, activity, scope and requirements can be found within the procurement documentation. For the avoidance of doubt this procurement does not cover car services. Bidders should be aware that NHS Somerset has appointed Somerset NHS Foundation Trust as the Lead Provider Trust for Lot 1 - Patient Discharge and (inter-hospital Transfers) with the responsibility of managing this activity. The successful provider will contract directly with Somerset NHS Foundation Trust via the NHS Standard Contract - subcontracted for this element. For the avoidance of doubt NHS Somerset will contract with the successful Lot 2 - Planned provider for this service. The duration of the agreement will be for 60 months, with an option to extend up to a further 24 months at the discretion of the Commissioner. NHS Somerset Integrated Care Board (ICB) is seeking to commission GP and Primary Care Nurses Careers Plus Services in Somerset. The GP and Primary Care Nurses Careers Plus Services is a support and training service that is made available to all GPs who are thinking of leaving or returning to general practice, and may include those considering early retirement, those struggling in the middle stages of their career, those wanting to return to work after a long-term absence, for instance in a parenting role or after maternity leave. The service sits specifically within the retention agenda focusing on those GPs and Nurses who are thinking of retiring or leaving General Practice having lost confidence or enthusiasm for their role. The objective of the service is to offer alternative work options where appropriate, retaining the skills of experienced health professionals and improving their job satisfaction. As part of successful delivery of this support programme, the service provider is required to deliver a minimum of two new intensive, facilitated support groups a year to be added to the existing scheme for GPs (covering a minimum of 16 - 20 new GPs in total per annum dependent on demand). Additionally, the service provider would be required to provide capacity, advice and support to the Integrated Care System (ICS) Nursing Legacy Mentoring Programme which is providing coaching, mentoring and pastoral support to registered nurses in Primary Care. The Legacy Mentoring programme is being managed by the ICS Talent Hub (hosted by Somerset NHS Foundation Trust). The Contract will be for an initial term of 12 months, with an option to extend for any period up to a further 12 months. NHS Somerset Integrated Care Board (ICB) is looking to procure a digitalised Continuing Healthcare (CHC) end-to-end person management digital system, where information seamlessly flows between processes by connecting the entire lifecycle into one platform. The system needs to facilitate the services to deliver enhanced transformation across all spectrums of the service. Somerset's CHC Service provides high quality, safe care to Somerset's most vulnerable Adults and Children, and currently has a combined active caseload of around 1,400 individuals. The ICB is seeking an efficient and effective digital system to manage the caseload and the inherent complexities associated with the services. One of the key requirements for a new system will be the need for it to be sufficiently flexible to support a wide range of services outside the traditional CHC / Funded Nursing Care (FNC) / Fast Track (FT) / Continuing Care (CC) agenda. Service wide transformation will be implemented across all areas including clinical, business, performance, finance and contracting and commissioning. Enabling better integration between systems across Health and Social Care is also critical. CHC is a unique service as it covers and links many elements of the health and social care system. Therefore, interoperability is of paramount importance, allowing for the CHC service and wider Health and Social Care teams to share and view records. This will support improved accessibility and visibility of data which in turn will enable more efficient and streamlined workflows for all teams and a better person experience. The Commissioner has a maximum financial envelope available of £200,000 per annum, including VAT. The total maximum contract value for the 3-year Contract is £600,000 and including VAT. At the end of the initial contract term the Commissioner will have an extension option for a further 1 year (1 x 12 month) period available which will be awarded at their discretion, giving a maximum possible contract value of £800,000 inclusive of VAT, over 4 years if the full contract term is fulfilled. The service will assess all patients in the community aged 18 years and over who are identified as being at high risk of an adverse outcome from Covid and have Covid symptoms and/or a positive lateral flow test. It is expected that these patients will have been provided with a letter by NHSE or have been identified as high risk by their specialist/consultant. NHS Somerset Integrated Care board (ICB) is seeking to secure a provider who has the capability and capacity to deliver a Support Service for Children, Young People and Families who have been affected by child sexual abuse (CSA). This will cover children and young people aged 0-18 years, who are living in Somerset or who have a registered Somerset GP. The service will focus on providing support for CYP aged 0-18 years who have experienced CSA, including those who may fall outside the threshold for admission to specialist mental health services and/or other specialist services. The service will work collaboratively with specialist services to ensure comprehensive support and continuity of care for these individuals. Service Objectives: • To deliver age-appropriate, high-quality and effective services for CYP, their families and carers addressing the psychological and emotional impact of CSA as early as possible. • To provide individualised support focused on the unique needs and contexts of each C/YP ensuring services are tailored to their presenting issues. • To empower CYP and their families to move forward positively in their lives by promoting resilience and recovery rather than focusing solely on the events of the abuse. • To minimise the long-term negative impact of CSA on the mental health, emotional wellbeing and overall quality of life for CYP and their families in Somerset. The service will: • Deliver a range of effective and evidence-based advice, information, talking, therapeutic and creative interventions. • Deliver a consistent and timely support service to the multi-agency workforce working to support children who have experienced sexual abuse, enabling staff from other services to safely and effectively 'identify, hold and respond'. • Provide appropriately trained staff who have an expertise in engaging and building relationships with CYP and their families and have the skills to assess and understand the barriers CYP may face, involving other agencies and professionals as appropriate. • Provide staff who are friendly, well-trained professionals, capable of offering advice, counselling and other appropriate interventions to support CYP. • Be offered in a safe, comfortable, confidential and non-judgemental environment. • Develop and maintain effective and efficient local and national networks to build a holistic, integrated and seamless package of support for CYP, their families and carers. TUPE is likely to be applicable, further details can be found in the procurement documentation. The contract term will be for 6 years with an optional extension of any period up to a further 2 years. The contract value is £380,436 per annum giving a contract value of £2,282,616 for the initial contract period and £3,043,488 if the full extension period is adopted. NHS Somerset Integrated Care Board are seeking to secure a prime provider with the capability and capacity to deliver an Alternative Provider Medical Service at Minehead Medical Centre, in line with specified requirements. The latest registered patient list size is 11,738 (NHS England January 2025). There is an opportunity for the provider of the new contract to potentially increase the registered list size and subsequent income through providing a good quality service. The practice is situated in Minehead, a town in West Somerset with a population of 11,757 in the 2021 census. Minehead is in a remote area of West Somerset with the nearest town of Taunton around a 45-minute drive away. There is an Urgent Treatment Centre in Minehead although the nearest main hospital, including the Accident and Emergency Department is in Taunton. The distance to travel to the main hospital site and the Accident and Emergency Department in Taunton needs to be appropriately considered and incorporated in the delivery of services to patients. The service will: • Provide Primary Care Medical Services in accordance with the Alternative Medical Services Provider (APMS) contract. • Offer registration to residents living within Minehead (see patient registration area within the tender documentation). • Open from 08:0-18:30 Monday to Friday, excluding Bank Holidays. The Contractor will: • Provide GP led primary medical care services to patients residing in the Practice Registration Area and/or patients registered with the practice as temporary patients according to National Guidance as amended from time to time. • Have an integrated and fully supported primary health care team to work in partnership with all other NHS and non-NHS healthcare Contractors and stakeholders (including, but not limited to, district nurses, social services, mental health services, acute trusts, voluntary and third sector services and Public Health England etc.) on the same basis as other GP practices in the area. This will include participating in and helping to integrate local collaborative models of working such as enhanced access arrangements, Primary Care Networks initiatives, NHS Somerset Integrated Care Board (ICB) Primary Care Strategy and other strategies that have elements that are relevant to primary care (e.g. mental health). In addition, the Contractor will engage and participate in future local collaborative models of working such as neighbourhood teams as and when they arise. • Participate in the QOF and in the Delivery of Directed Enhanced Services (DES). The current list of Directed Enhanced Services is in Annex 1 of the service specification. NHS Somerset Integrated Care Board (ICB) is seeking to commission GP and Primary Care Nurses Careers Plus Services in Somerset. The GP and Primary Care Nurses Careers Plus Services is a support and training service that is made available to all GPs who are thinking of leaving or returning to general practice, and may include those considering early retirement, those struggling in the middle stages of their career, those wanting to return to work after a long-term absence, for instance in a parenting role or after maternity leave. The service sits specifically within the retention agenda focusing on those GPs and Nurses who are thinking of retiring or leaving General Practice having lost confidence or enthusiasm for their role. The objective of the service is to offer alternative work options where appropriate, retaining the skills of experienced health professionals and improving their job satisfaction. As part of successful delivery of this support programme, the service provider is required to deliver a minimum of two new intensive, facilitated support groups a year to be added to the existing scheme for GPs (covering a minimum of 16 - 20 new GPs in total per annum dependent on demand). Additionally, the service provider would be required to provide capacity, advice and support to the Integrated Care System (ICS) Nursing Legacy Mentoring Programme which is providing coaching, mentoring and pastoral support to registered nurses in Primary Care. The Legacy Mentoring programme is being managed by the ICS Talent Hub (hosted by Somerset NHS Foundation Trust). The Contract will be for an initial term of 12 months, with an option to extend for any period up to a further 12 months. The total value of the contract is £158,000 for the initial 12 months contract term, with a total contract lifecycle value of £316,000 including the 12 months extension.
