Adult Community Specialist Palliative Care Services
NHS North West London Integrated Care Board (the “ICB) is intending to commission Adult Community Specialist Palliative Care Services (CSPC), for the provision of Community Specialist Palliative and End Life Care Services for adults (18+) living with progressive life limiting/ terminal conditions with or without co-morbidities for a proposed contract term of between 12 to 36 months (the “Proposed Contract”). <br/><br/>Adult CSPC services are part of a larger group of services that support people at the end-of-life. They operate to support people away from hospital and are geared toward people who have complex and specialist needs that cannot be met by general services alone, such as general practice and community nursing. Instead, people with complex needs are cared for by a network of services which are staffed by clinician’s and teams who have specialist skills and training in palliative care.<br/><br/>These services are currently being delivered by Harlington Hospice, St John’s Hospice, St Luke’s Hospice, Royal Trinity Hospice, Marie Curie London Nursing Services, Meadow House Hospice (provided by London North West University Healthcare NHS Trust ), Central and North West London NHS Foundation Trust and Central London Community Healthcare NHS Trust. <br/><br/>The ICB is of the view that the services described are capable of being provided only by the providers mentioned in this notice, and are not aware of other providers who can deliver the required level or care and services at the current tariffs and who have estates in the areas where services need to be delivered. <br/><br/>The described charitable hospice services are partially funded by the service providers including charitable contributions. The funding envelope available is based on continuation of charitable funding to support delivery of specialist palliative care services, in line with the stated aims of the charitable hospice sector. <br/><br/>For a provider to be able to apply, they need to be able to demonstrate that they have the appropriate estates (i.e. hospice inpatient unit facilities), are specialist palliative care providers with the necessary specialist workforce in line with the description of the procurement. The model of care outlines the common provision of CPSC service in all eight boroughs in North West London: <br/><br/>1. NHS NW London ICB currently has an arrangement in place with PrescQIPP CIC to deliver the above services. PrescQIPP CIC is an NHS funded not-for-profit organisation that supports quality, optimised prescribing for patients. With subscriber input and collaboration across NHS organisation, the platform provides system wide support across the ICB and it's ICS members to benefit from insightful resource provision.
The purpose of this notice is to be transparent about NHS NW London's intention to explore options to procure this service and to undertake a further competition, if possible. NHS NW London ICB is currently of the view that this is a niche service and PrescQIPP CIC is the only capable provider that is best placed to successfully deliver this service. As such, should PrescQIPP CIC be the only capable provider, it is NHS NW London ICB's intention to award a further contract (NHS Standard contract-short form) to PrescQIPP CIC from 1st October 2023 for up to 2 years and 6 months (estimated value, £65,500.00 per annum).

• Continue to build an accurate picture of health and wellbeing needs of this cohort.
• To reduce acute systems pressures in the system by the demand of the hotels utilising acute settings for routine primary care challenges.
Key service requirements include:
• To provide a 30 minute face to face health screening assessment with a GP within 15 working days of arrival; assessing communicable disease risk and status, wider health and wellbeing, mental health, medications.
• To register the patient with a local practice.
• To be able to offer patients ongoing GMS care including but not limited to:
- Immunisation catch up (for all ages).
- Onward referral to wider services.
- To ensure that asylum seekers are accessing all services under PCN DES and wider LES provision.
• To provide outreach clinics where possible on site in the hotels for screening.
This would be to service approximately 7500 service users residing in at least 45 premises located across all eight (8) NW London boroughs, with the current highest concentrations in Hounslow and Hillingdon.
NHS North West London currently has contract arrangements in place with Primary Care Networks (PCNs) where services are delivered at scale through NHS Standard Contracts. The contract term expires on 31st March 2028.
The purpose of this PIN is to be transparent about NHS North West London ICB's intentions to include the services set out above within current PCN arrangements from September 2023 onwards (the estimated value in year 1 is £817,400).
This would be a network-based contract, in order to map against and integrate with the work that is already happening at neighbourhood level to tackle health inequalities; this could mean there could be multiple providers even within one health borough location.

Aims and objectives of the service:
-Support Hillingdon GPs to put in place a local search to keep track of SMI physical health check progress and to share that information with our local SL providers, so we can identify people who need support encouragement to have an SMI health check, specifically those from black and minority ethnic groups who need more intensive support for them to engage.
-Work with practices and support the use of GP receptionist staff to systematically call SMI patients and invite them in - targeting 75% of the practice list to come in for their physical health checks.
-Educate all local Supported Living and Residential providers about SMI health checks and the importance of supporting service users to attend GP practices for the checks.
-Work with practices and PCN's to develop a model of SMI check that is person centred and delivered from community settings that supports people with SMI Greater access to and take up of other services.
Key Service Outcomes:
-Provide educational briefings to supported housing staff and any relevant supporting staff about the importance of SMI checks and how to reassure Patients who have concerns.
-Provide performance measurement reports on numbers of SMI's being completed.
