NHS North East London Clinical Commissioning Group (NEL CCG) is seeking to commission an all-age bereavement service to support Adults, Children and young people within the City of London & Hackney. You are invited to submit a bid for the Bereavement Service. This procurement is to offer dedicated bereavement counselling support to improve the mental wellbeing of bereaved family members, carers, relatives and friends. Please refer to the service specification document for further information on the service. In line with the Health and Care Bill it is envisaged that CCGs will be abolished and replaced by Integrated Care Boards (ICBs). Any reference to CCGs as Contracting Authorities in this procurement should be interpreted as referring to the ICBs that will take their place in due course. The estimated total Value of the 5 Year Contract is £1,004,415.00. The total value is based on an annual value of £200,883 per year. The Contract will be for a period of 3 years with the possibility of an extension for a further 2 years at the discretion of the CCG. The service is expected to commence in October 2022.

The Primary Care Network Facilitator service will cover all of North East London and will provide facilitators to work with primary care teams to support the early diagnosis of cancer. They will be responsible for supporting, educating and influencing primary care teams, health professionals and local health systems to improve cancer services in line with NEL Cancer Alliance’s programmes of work and strategy. The service will support the NHS Long Term Plan and the Primary Care Network Direct Enhanced Service (PCN DES) requirements. City & Hackney continues to demonstrate an on-going commitment to place-based integration. The C&H Place Based Partnership has made great progress in bringing services together so they are organised around eight Neighbourhoods. City and Hackney adopted more of an asset based approach that is focusing on what matters to residents; working more closely with local communities; and taking a more proactive approach to identifying and supporting residents who have complexity in their lives. The City & Hackney Neighbourhoods programme funds local health, social care and voluntary sector providers to deliver towards the above aims and to reorganise their services around a place-based model. The City & Hackney Place Based Partnership funded a Community Pharmacy provider to deliver the following non-clinical support to the Community Pharmacies in local the Neighbourhood: • Support the roll-out and neighbourhood engagement of GP referrals to the Community Pharmacist Consultation Service. This includes providing training for GP support staff, engaging with residents and community groups, promoting the use of the new Health First minor ailments scheme and monitoring number of GP CPCS referrals by neighbourhood. • Work collaboratively with neighbourhood teams to deliver a successful COVID-19 and Influenza vaccination programme, focussing on the vaccination of the residents in care homes and residential homes. • Work within the neighbourhoods to deliver greater population health through preventive care work with a focus on measuring/ monitoring blood pressure, healthy lifestyle weight management, exercise, alcohol & smoking issues, and promoting self-care advice and self-help. • Work with each primary care network to generate a specific neighbourhood action plan where community pharmacy can support system partners. • Ensure the workforce needs of Community Pharmacy are considered as part of the development of the Neighbourhoods organisational development plan. • Support the evolving pathways of the Neighbourhoods programme including anticipatory care, Multidisciplinary Meeting - including Mental Health Multidisciplinary Meeting and blended teams and Children and Young People pathways. Additional information: In order to deliver this service, the organisation interested must be able to represent all Community Pharmacies in the City & Hackney area and be able to demonstrate accountability to the Community Pharmacies in C&H by becoming members or shareholders of the organisation. The organisation must have good working relationships with Primary Care Network Clinical Directors and GP's across City and Hackney, and be able to evidence previous collaborative working with GPs. The ICB currently procures, and would expect the organisation providing the service to be a not-for-profit organisation that was established by the City & Hackney Local Pharmaceutical Committee, called Pharmacy Services Partnership (PSP), which was created by Community Pharmacists to represent them in Hackney and City of London. PSP is a registered company and has been the provider of this activity since the start of the implementation of the Neighbourhood Model because no other Community Pharmacy membership organisation exists in the C&H area. The ICB is now seeking to determine if there is any other organisation interested to provide the services across City and Hackney and that can represent all Community Pharmacists. The ICB is of the view that there is no alternative provider to the incumbent provider because of the unique way that Pharmacy Service Partnership Ltd was established by the local Community Pharmacists and LPC specifically to represent Community Pharmacists where there was a common interest. The Psychological Therapies and Wellbeing Alliance (PTWA) is an innovative collaborative contracting arrangement that aims to: 1. Co-ordinate the provision of Psychological Therapies and Wellbeing services across all City and Hackney providers. 2. To provide patients with an integrated point of access and to ensure that resources and patient flows are balanced between services to avoid the build-up of long waiting lists. 3. To ensure that the full range of needs are met, closing gaps in service provision. The PTWA board co-ordinates and integrates service provision between providers in the psychological therapies' pathway through a structure of shared performance and contractual framework, shared targets, aims and through sharing expertise, best practice, training and development. One of the providers is appointed as a fund holder who receives the funds from the CCG and pay the funds to the other providers. The Alliance itself is underpinned by an alliance agreement that is signed by all of the providers and the CCG. This enshrines the role of a PTWA board where the providers and the CCG are jointly and severally responsible for key decisions about the delivery of services and allocation of the available funds. The PTWA is open to the admission of new members. This PIN is to publicise to any organisations that is interested in joining the alliance; that they can apply to join the alliance by responding to this notice. Any applicant will be asked to prove they meet the essential criteria as a condition of admission as an alliance member. Once admitted as a new alliance member the organisation can take part in the discussions with the alliance board and be able to propose services that can improve the delivery of the PTWA in City and Hackney. Essential criteria from entry of new members in the PTWA: Organisations considering to be part of the PTWA must meet the following ESSENTIAL criteria at of their application: • Essential/pass criteria 1: The applicant must be an established provider of services (at least 18 months of locally provided services) which are integral to the psychological therapies' pathway, common mental health problems or severe mental illness. • Essential/pass criteria 2: All psychological therapists providing any Psychological Therapy service, are fully accredited with the relevant national professional body and outcomes and activity are reported on to the Alliance as relevant and appropriate • Essential/pass criteria 3: The applicant must receive a minimum of £100K of annual recurrent statutory funding annually (CCG or local authority funding) for the provision of services to City and Hackney residents. • Essential/pass criteria 4: The applicant must be able to pass due diligence check of necessary policies, procedures and accounts required before an NHS Standard Contract can be awarded. This includes CQC registration where relevant. Additional information: The evaluation of the applications against the criteria will be completed by the end of October 2022. Community based Hospices based in North East London for Adult service users. Providing high quality end of life and palliative care to all adult patients living in their last phase of life (usually patients with confirmed end stage conditions and terminal illness such as cancer).
