The expected value for the award is £260,000 for the 2-year contract period fully inclusive of any applicable VAT.<br/><br/>Additional funds may become available during the life of this contract, and should such funds become available the Commissioner reserves the right to extend the contract . At this stage we cannot confirm whether additional funds will become available or the value of such funding, however we anticipate that such funding could be up to a total of £900,000 over the maximum 4 years.

This notice is an intention to award notice under the Provider Selection Regime competitive process.<br/><br/>The purpose of this Service is to deliver care within a Residential Care Home with Nursing (referred to from this point as a Nursing Home) for those patients who have been assessed as eligible for NHS Fast Track funded care, have expressed a preference for care in a Nursing Home and whose needs can be safely and appropriately managed in a Nursing Home. The Service is commissioned to provide dignity and assurance to people and their families when they are entering their final weeks of life and is expected to be safe, responsive and efficient. This includes enabling patients to achieve the best possible quality of life whatever their disease or disability, gender, ethnic background, or sexual orientation.<br/><br/>This will be achieved through the application of a holistic person-centred approach to care which is integrated seamlessly with all other health services, especially GP primary medical services and specialist palliative care services.<br/><br/>The aims of this Service are:<br/>• To deliver a responsive, caring and person-centred service for patients and their loved ones in the last weeks of life.<br/>• To facilitate prompt commencement of care in a Nursing Home, for patients identified as in the end stages of life under the NHS Continuing Healthcare Fast Track pathway.<br/>• To commission end of life care which is delivered by staff with enhanced levels of training and competencies in relation to end of life care.<br/>• To prevent inappropriate or avoidable admissions to hospital for end-of-life care.<br/><br/>Please note that all Lots are restricted to a maximum of 2 beds per nursing home, except for Lot 2 (which is 3 beds). This service will form part of our Central Norfolk intermediate care bed model and be supported by Norfolk Community Health and Care NHS Trust (NCHC).<br/><br/>This pathway is for individuals registered to a Central Norfolk GP (ie; North Norfolk, South Norfolk and Norwich practices), who are 65 years and over with moderate cognitive decline and who are identified to require a short period of reablement and recovery in an appropriate bedded setting to enable discharge home/usual place of residence (UPOR) or suitable long term placement, following an Acute Hospital admission.<br/><br/>The pathway includes an in-reaching multi-disciplinary team (MDT) comprising; Physiotherapy, Occupational Therapy, Social Care and Primary Care with operational delivery through NCHC.
