This is an Intended Approach Notice to award a contract using the Most Suitable Provider Process for the Provision of Recovery Colleges (the Service) for NHS Mid and South Essex Integrated Care Board (NHS MSE ICB) (herein “The Contracting Authority”) The Service has been historically commissioned under separate contractual arrangements held by the individual Clinical Commissioning Groups (CCGs). The ICB was established on 1st April 2022, replacing the CCGs as the Contracting Authority. As a result, these contracts will now be subsumed into a single contract commissioned by the Contracting Authority. The Services are currently delivered by three different providers through a multifaceted approach with differing offers across the system which is reflective of the previous CCG individual commissioning pathways, resulting in inequitable offers across the population of MSE, as well as no current offer in the Basildon and Brentwood locality. The ICB has proposed a single system-wide offer, to amalgamate all of the contracts for these services into one potential contract to support integration, reduce fragmentation of services and reduce the number of contracts that MSE ICB will need to manage going forward. This will enhance and improve outcomes to elicit patient safety and quality. The Contracting Authority has worked with the three incumbent providers and is of the view that this cohort of existing providers have the necessary experience, knowledge and skills to offer a collaborative provision via a lead provider model. Recognition that the services will need to do more with less funding and recognising the benefits and efficiencies of doing things once. Providers will also need to share risk and invest in the service provision through this collaborative partnership. The Contracting Authority intends to award a single contract to suitably, technically capable provider. In order to award a contract, the most suitable provider must have demonstrably satisfied the minimum Basic Selection Criteria, including financial and economical standing, satisfying the Key Criteria assessment and the commercial envelope. The awarded provider will be expected to share the risk, resources, capacity and to work collaboratively with the ICB. NHS Mid and South Essex Integrated Care Board ICB (M&SEICB), the contracting authority, has a responsibility to commission primary medical services on behalf of the local population. This duty is overseen by the Primary Care Commissioning Committee. Due to significant housing growth, a new primary medical care health center is due to open at Beaulieu Park, located in Chelmsford Essex in Autumn 2024 to meet the health needs associated with the housing growth. The ICB intends to operate this new site as an additional site of an existing local primary care provider - local practices (non- special allocation service) whose boundary covers the practices site; and/or local practices (non- special allocation service) with a clinic site within 7km of the Beaulieu Park site of Primary Medical Services under an GMS or APMS contract. The Beaulieu Park services are to be provided under the providers’ existing contract, with the Beaulieu Park site added as an additional service location. Services for the Beaulieu Park primary care services will be provided by way of a variation to the successful providers’ existing APMS or GMS contract. Any new patients registered, associated with the Beaulieu Park site, will be funded at GMS rates – if a provider is currently on an APMS contract new patients registered associated with the Beaulieu Park site will be funded at GMS rates. Providers will need to accept a patient list size of zero from day one of the contract with the Beaulieu Park new housing residents becoming registered and through patient choice any patients choosing to register from their current practice to the new Beaulieu Park provider.

NHS Mid and South Essex Integrated Care Board (the Authority) intends to award three contracts to deliver Level 1 Sedation Services for children and adults in Mid and South Essex, following a Most Suitable Provider (MSP) process under the Health Care Services (Provider Selection Regime) Regulations 2023. Level 1 conscious sedation services provide basic, safe sedation, mainly inhalation sedation and single drug intravenous sedation, to help patients undergo routine dental treatment. These services follow nationally recognised IACSD Standards, which set requirements for trained staff, appropriate clinical environments, monitoring, and safe recovery. The Authority seeks to increase Level 1 dental sedation capacity in North Mid and South Essex, specifically from Chelmsford northwards, with services located along the A12/A120 corridor to ensure good patient access. Providers will only be deemed suitable therefore if they operate an existing dental practice within this defined catchment. In applying the Process, the Authority has assessed the five Key Criteria and updated its provider landscape knowledge through a key criteria assessment and data gathering process, to confirm provider capability and local connections. This Intended Approach Notice to follow the Most Suitable Provider Process, pursuant to The Health Care Services (Provider Selection Regime) Regulations 2023 (the Regulations). The Contracting Authority, NHS Mid and South Essex Integrated Care Board (the Authority) is using the most suitable provider process (the Process) as defined by Regulations 6(6) and 10) to award a Land Option (“the Contract”) via a mixed procurement (commencing 25th March 2025), for the development and delivery of a General Practitioner Medical Surgery and associated facilities and services under a General Medical Services (GMS) contract (the Requirement). The opportunity is deemed to comprise a mixture of in-scope health care services and out-of-scope non-healthcare services. The main subject-matter of the contract is deemed an in-scope health care service because the estimated lifetime value of the health care services being delivered under the MSPs existing GMS contract is higher than the value of the one-off sum associated with the Land Option and associated capital works (non-healthcare services). The Authority is of the view that separating out the process around selecting an appropriate GP Practice to award the Land Option to, from the selection of a GP Practice to deliver general medical services from the healthcare premises developed on the land would have a materially adverse impact on the Authority’s ability to act in accordance with the procurement principles. Further details of the Authorities assessment leading to the decision to undertake a mixed procurement under PSR can be found in detail at the attached document titled Supplementary Information for the PIN _ Ashingdon Road 04.03. For avoidance of doubt, this Process will not result in the Authority and the most suitable provider (MSP) entering into a new contract. The Authority is responsible for running the Process as the legal entity responsible for commissioning Primary Medical Services in Mid and South Essex. The Authority and existing GP Practices that have expressed an initial interest in being considered as the MSP are already within contractual arrangements via each Practice’s existing GMS. Instead, the Process intends to identify and award the opportunity to enter a Land Option with Bloor Homes and Rochford District Council. The Authority is expecting the council/developer to transfer the land directly to the MSP following the successful conclusion of this exercise. The MSP will be responsible for working with the council to develop the site for delivery under their existing GMS contract. Further work will be needed as the site develops to amend the MSP's existing contract to reflect any proposed plans (for example, whether it is an additional branch site or a relocation). The development has made a financial contribution of £262,300 available to the MSP that purchases the land, to support the improvement of healthcare services in Rochford, which is the value that has been communicate in this Notice as the estimated total value. The MSP will be required to source the necessary capital to implement the requirement through solutions such as partnering with a specialist healthcare facility developer or some form of borrowings. No further capital resource is available through the NHS to support build costs. The estimated lifetime value of the Land Option may be between £3.75m and £5.25m based on an NHS compliant build costing in the region of £5,000-7,000 per square metre. The planning permission shows the land could accommodate a healthcare building of 750m2 plus on-site parking. The estimated lifetime value of the existing GMS/PMS contract could be between £26m and £99m depending on which Practice is selected (because it is based on the global annual sum of their existing contract and a 45-year contract duration).

Historic commissioning arrangements of the Service have led to health inequalities in access and delivery of pathways across the Mid and South Essex (MSE) Integrated Care System (ICS). Due to new ways of working, increasing collaboration, and communication across primary, community and secondary care, implementing the Service will ensure there is equitable provision and pathways for all MSE residents. Working with stakeholders the Authority developed a new community pathway for people aged 16 years+, based on the East of England Musculoskeletal (MSK) Pathway Improvement Framework, the Best MSK high impact recommendations and adhering to the Getting It Right First Time (GIRFT) pathway. This new model of care will provide integration across the Best MSK pathway including Single Point of Access (SPoA), clinical triage, physiotherapy, orthopaedics, spinal, rheumatology, pain management, surgical interventions and therapies across MSE. It will assess and treat more patients outside of acute services and deliver improved outcomes, quality and patient experience of care. In applying the Process, the Authority has considered the five Key Criteria and has identified the Service and proposed contracting model will support the MSK core purposes of reducing unwarranted variation, sharing best practice, improving access, outcomes and experience of patients, enabling best lifelong MSK health within all communities, and improving quality and patient safety. This is an Intended Approach Notice to award a contract using the Most Suitable Provider Process for the Provision of Talking Therapies (TT, formally known as IAPT) and Psychological Therapies for Severe Mental Health Problems (PT SMHP) referred to within this notice as “Mental Health Core Services” (the Service) for NHS Mid and South Essex Integrated Care Board (herein “The Contracting Authority”) The Service has been historically commissioned under separate contractual arrangements held by the individual Clinical Commissioning Groups (CCGs). The ICB was established on 1st April 2022, replacing the CCGs as the Contracting Authority. As a result, these contracts will now be subsumed into a single contract commissioned by the Contracting Authority. The Services are currently delivered by four different providers through a multifaceted approach with differing offers across the system which is reflective of the previous CCG individual commissioning pathways, resulting in inequitable offers across the population of MSE. The Long-Term Plan and Joint Forward Plan (2023) states a commitment to increase access to psychological therapies with a focus on maintaining treatment times and recovery standards. The ICB proposes a single system-wide offer, to amalgamate all the contracts for these services into one contract to support integration, reduce fragmentation of services and reduce the number of contracts that MSE ICB will need to manage going forward. This will enhance and improve outcomes to elicit patient safety and quality. The Contracting Authority has considered which providers may be capable of providing the Service for the relevant patient population in Mid and South Essex. The Contracting Authority is of the view, considering providers that they understand are likely to have the ability to deliver services to the relevant population, and all relevant information available at this time that it is likely able to identify the most suitable provider. The Contracting Authority has worked with the incumbents and is of the view that this cohort of existing providers have the necessary experience, knowledge and skills to offer a collaborative provision via a lead provider model. Recognition that the services will need to do more with less funding and recognising the benefits and efficiencies of doing things once. Providers will also need to share risk and invest in the service provision through this collaborative partnership. The proposed contract duration will be for a 3-year period, with the option to extend for a further 24 months, from the 01st April 2025. The total aggregate value for the entire 5-year contract term for the Service is £84,844,761. The Contracting Authority intends to award a single contract to suitably, technically capable provider. In order to award a contract, the most suitable provider must have demonstrably satisfied the minimum Basic Selection Criteria, including financial and economical standing, satisfying the Key Criteria assessment and the commercial envelope. The awarded provider will be expected to share the risk, resources, capacity and to work collaboratively with the ICB. Any queries will be considered until the 28th June 2024 at 12:00 noon, in line with best practice, under Provider Selection Regime. NHS Mid and South Essex Integrated Care Board (the Authority) intends to award an Integrator contract to deliver the Special Educational Setting (SES) Sight Test Service for children and young people through local accredited optometry practices, following the Most Suitable Provider (MSP) process under the Health Care Services (Provider Selection Regime) Regulations 2023. Children under 16 (or up to 19 in full-time education) are entitled to free NHS eye tests, but pupils in special education schools face barriers to access. A successful NHS England pilot demonstrated benefits of delivering sight tests and advice in school settings. The NHS Long Term Plan committed to this service for residential settings, extended in 2023 to day schools. Funding is now recurrently allocated to ICBs from 2025/26. The ICB considers an integrator model most suitable, where a lead organisation coordinates and performance-manages local optometrists rather than the ICB holding multiple contracts. This approach supports Essex-specific partnerships, supply chain accountability, integration, measurable value, and social impact. In applying the Process, the Authority has assessed the five Key Criteria and reviewed NHSE pre-market engagement data to confirm provider capability and local connections. Based on this, the Authority believes it can identify the MSP without a competitive process. The scope of the service is to deliver sight tests for pupils attending special education schools across Essex (including activity associated with the Authority’s successor Essex ICB’s geographical footprint which covers North East Essex and West Essex also). There are an estimated 4,400 pupils in total across Essex within these schools and the service is based on an assumed uptake of 80%, equating to approximately 3520 sight tests per year. There are currently multiple pathways into different ‘Wellbeing’ mental health services due to historic commissioning arrangements made by five separate Clinical Commissioning Groups (CCGs). The Authority was established in July 2022, superseding the CCGs. The Authority aims to address the health inequalities across the system by implementing a new single service that will ensure an equitable service and support early diagnosis and intervention for patients. Currently, multiple VCSE providers deliver wellbeing-related services across the ICS footprint. These include: • Wellbeing Hubs • Happy Hubs • Positive Pathways for 18-25s • Adult Positive Pathways This fragmented model has resulted in duplicated services, variable quality and access, inconsistent outcomes, and inefficiencies in service delivery. The procurement process will be structured into two distinct lots to better meet the varying needs of the population and to streamline service delivery.
