The aim of the service is to provide evidence based minor injuries care that optimises health and wellbeing and reduces the impact of ill health, whilst reducing pressure on both emergency and Urgent Care services.
Minor Injury Services provide highly responsive, effective and personalised services outside of hospital for the patient with urgent but non-life-threatening needs. These services deliver care as close to people's homes as possible, minimising disruption and inconvenience for the patient and their families and relieving pressure on emergency care services.

This is a multi-agency approach to improving care in the community and reducing avoidable conveyances and admissions to secondary care, which involves primary care, EMAS and secondary care geriatricians.
This contract covers one element of the service, namely the Charnwood GP Network. This provides GPs to undertake home visits for patients referred by the UHL Consultant Geriatrician or Geriatric Emergency Doctor following contact from EMAS.

To provide a high quality 24/7 urgent care acute home visiting service for patients residing in LLR, which operates as part of an integrated system offering urgent care at home for housebound patients, urgent response for falls, a night nursing service and an urgent primary care visiting service for out of core hours.
The service is provided to patients who have urgent health needs and are at increased risk of admission into hospital. The service aims to reduce the number of avoidable emergency admissions and attendances at Emergency Departments (ED) and to maximise the use of other care pathways over a 24-hour cycle. The service receives referrals from the local Clinical Assessment Service, General practitioners' (GP) partners, healthcare professionals, and care home staff.
