The service will provide First Contact Practitioner(s) in Musculoskeletal Care in order to reduce the burden of the GP workforce, improve quality of care to the patient.

As part of the GP 5 Year Forward View (GP5YFV), GP practices are investing in associate clinicians who will reduce pressure and support General Practice and support to provide a better service to their population. The service will provide First Contact Practitioner(s) in Musculoskeletal Care in order to reduce the burden of the GP workforce, improve quality of care to the patient.

The service will provide open access, free and confidential counselling for young people aged 14 to 24 and forms part of the Thrive Hull Partnership of support and services for children and young peoples' emotional wellbeing and mental health. The service is targeted to support the city's vulnerable groups and helps reduce health inequalities. The service is core to other provision as part of the wider youth work opportunities delivered within the centre that supports good emotional health and provide better outcomes for young people.

The service is to deliver a community based, musculoskeletal service user education, clinical assessment and treatment service. The service will treat service users with a range of musculoskeletal conditions affecting their neck; spine and lower back; and upper and lower limbs; including their feet. The service will have Consultant/General Practitioner with Specialist Interest (GPSI) input with access to key diagnostic tests and offer a range of treatment interventions. Service users will be able to access advice to help self-management or be able to self-refer to get access to treatments. GPs, secondary care professionals and other health professionals will be able to refer into the service to get easy access to quality MSK services.

This service is for the provision of Primary Care Anticoagulation Warfarin Monitoring and Management Service in Primary Care for the following GP Practices • Beech Tree Surgery, Selby • Dalton Terrace Surgery, York • Escrick Surgery, Escrick • Sherburn Group Practice, Sherburn in Elmet • South Milford Surgery, Leeds • Tadcaster Medical Centre, Tadcaster • Unity Health, York • Scott Road Medical Practice, Selby • Posterngate Surgery, Selby

A palliative and end of life care service for patients aged over 18 years requiring domiciliary end of life care, complex symptom management and assessment of their changing condition and/or disease progression. The focus of the service is to improve the quality of care for patients at the end of their life with the aim of ensuring that patients have the optimum quality of life until death.

The service provides 24/7 video-enabled clinical healthcare support into care homes. The system allows nursing and residential care homes to connect to the Airedale Hub for clinical advice and guidance. The aim of the service is to avoid alternative health services being used where it is possible to care for residents in their usual place of residence and reduce pressure on local NHS services. The focus of the service is that residents in care homes are provided with a professional, responsive, and caring service that provides support tailored to their individual needs to enable them to access appropriate health care services and to live a full life.

The service provides direct access to non-obstetric ultrasound diagnostics following referrals from GP Practices. The aims of the service is to aid early diagnostics to be undertaken closer to home; improve access to diagnostic testing; and avoid the need for unnecessary referral to secondary care.

NHS Humber and North Yorkshire Integrated Care Board (hereafter the Contracting Authority) is seeking to commission Wheelchair and Postural Services - Hull and East Riding of Yorkshire, through an open procurement process. The preferred Bidder will be required to demonstrate how they will deliver a clinically safe, high quality, dignified, cost effective service that puts the individual at the centre of their own care which supports and maintains their on-going health and wellbeing needs. The service will be required to: • Provide a timely, comprehensive holistic assessment of an individual's posture and mobility needs, taking into account medical condition, functional ability, social situation and environment; • Provide posture, pressure relief and where appropriate, predictable changes in Wheelchair User needs; • Provide a service that meets the Wheelchair Users wider health and wellbeing needs; • Enhance and maintain the Wheelchairs Users quality of life; • Support personalisation, independence and offer a Personal Wheelchair Budget where eligible and appropriate; • Support an overall reduction in hospital admissions and/or length of stay; and • Demonstrate Wheelchair Users (and carers) are central to the decision making in their wheelchair assessment and provision - evidenced through patient centred outcome measures.

The preferred Bidder will be required to demonstrate that they can provide and deliver a quality focussed and resilient service model which operates in a sustainable, innovative and flexible way, ensuring service user satisfaction throughout. The service will be required to: • Ensure that any transfer and transition of services is effectively managed to ensure a continuation of the existing level of service provision and to prevent de-stabilising the delivery of corporate services; • Ensure that Services are supported on a 24/7 basis as appropriate to requirement; • Ensure that all systems, and associated infrastructure, are securely operated and maintained; • Ensure a proactive support service is available, supporting a clinically essential service, with robust incident management processes; • Propose innovative and cost-effective strategies to address service improvement and sustainability and /or create financial efficiencies; and • Fully understand all local and national requirements and work closely with the ICB to help them plan better; to discharge their statutory duties; and exploit technology and systems in support of the challenging NHS transformation agenda.

Individuals with a clinical frailty score (CFS) 7+ (post assessment) will be offered annual reviews, including proactive identification of those who are likely in the last year of life. Provision of an intermediate care (step up / step down) services located within a care home managed by the GP Practice - Pocklington Group Practice, which will: • provide ongoing assessment to determine future care and support needs. • provide support during a period of ill-health to maximise level of independence and/or return to previous level of independence where possible. • support develop confidence with mobility and all self-care tasks by having access to therapeutic interventions. • ensure any health conditions can be treated and improved through access to appropriate nursing intervention and medical care and review, which complements re-enablement and is appropriate to the hub environment. • provide care for people closer to home.

The service provides online counselling support service for children and young people within North East Lincolnshire aged 11 - 25 years that have emotional or mental health concerns. The service will aim to help children and young people manage their emotional wellbeing concerns at the earliest opportunity before these problems escalate, resulting in the possible need for more specialist interventions and to deliver measurable improvements in the emotional and mental health of children and young people and thus improve their outcomes.
