The Community Urgent Response Team (CURT) GP (previously known as Community Response Team (CRT) GP) role was established in response to the COVID-19 pandemic to provide senior clinical decision making capacity to the existing Community Response Team provided by North Lincolnshire and Goole (NLaG) Community & Therapies Division, which provides an appropriate community response to patient need as determined by clinical triage.
The CURT team provide assessment, treatment and support to patients in their usual place of residence, where they meet the inclusion criteria for the service, who are experiencing a health or social care crisis and who might otherwise be admitted to hospital. Care is provided by a multi-disciplinary team, including the CURT GP role, who undertake a holistic assessment of the person's needs and determine and provide an appropriate response.
Since the establishment of the CURT GP role, local service provision and pathways have evolved and transformed in line with national guidance and also recovery from the pandemic. This includes the delivery of the 2hour Urgent Community Response requirements and a greater national focus on the role of community services in delivering urgent care in the community where it is safe and appropriate to do so. This will reduce demand on ambulance and acute hospital services, support people to live independently for longer and improve patient outcomes through delivering urgent care needs at home.

This service is for people discharged from Hospital on the NHS discharge to assess pathway 2 which relates to discharges to a community bed-based setting which has dedicated recovery support offering short-term input to help the person recover, enabling people to either live more independently. People discharged from hospital on pathway 2 require rehabilitation and reablement, some require specialist input at Sir John Mason House Intermediate Care Centre.
The service is to be delivered though an integrated approach with health and care partners across North Lincolnshire health and care system in line with the Choice Policy, building on the principles of the national community rehabilitation and reablement model.

The Specialist Assessment of the Frail Elderly (SAFE) services delivers proactive care for people at risk of, or living with, frailty in North Lincolnshire. The GP-led specialist service undertakes holistic Frailty Assessments for a proactively identified patient cohort and develops person-centred care and support plans, including advanced care planning where appropriate, for those individuals.
The service is delivered in the patient's own home or in a clinic setting, based on individual's need. The service provides both 'routine' and 'rapid access' appointments. Rapid access appointments are offered for those patients where there is a high risk of hospital admission within the next 2 weeks, or where a patient has been discharged from Same Day Emergency Care (SDEC) or acute Frailty service and is at risk of readmission.
