Hospital Discharge Volunteers Support Service (North East Lincolnshire) - the service will work with older people who become deconditioned during short hospital stays to re-enable them to optimum levels prior to discharge, creating impact in hospital (better flow) and in the community (reduced care packages), this will include:
• providing a co-ordinated approach to reduce deconditioning when in hospital; and
• supporting employed staff to engage with individuals through a range of initiatives designed to address reducing deconditioning collaboratively led by nursing and therapy staff supporting the workforce through the introduction of volunteer support workers.
The intervention will include a variety of initiatives which provide purpose for individuals to get up and dressed each day and offer them an activity to do, so they don't become bored and disengaged. Working with identified individuals to keep them positive and focused on their recovery.
The service will operate 5 days a week Monday-Friday, providing core coverage from 09:00-17:00hrs, and additional support at weekends where volunteer availability permits. The service will be delivered from Diana Princess of Wales Hospital, Grimsby

Population Health Web-Based Mapping Business Intelligence Software Platform - Humber and North Yorkshire
Development a local integration of the population Health web-based mapping business intelligence software platform, SHAPE Local, for the purpose of profiling and service planning for Places (East Riding of Yorkshire; Hull; North-East Lincolnshire; North Lincolnshire; North Yorkshire; and York), Primary Care Networks (PCNs) and Integrated Neighbourhood Teams (INTs).
Segmentation visualisations - creation of visual indicators; and integration with the NHS Humber and Yorkshire ICB data warehouse - the use of SHAPE to help identify neighbourhoods and local area profiles, including drawing from the Humber and North Yorkshire segmentation data and utilising the tools available within SHAPE such as population targets
The service provision will include analyst and development time and training sessions of the use of the software.
The Illicit Tobacco Survey service provision will design and implement a survey to provide a robust assessment of the illicit tobacco market in Humber and North Yorkshire, sampling in all 6 'Place' areas.
The Supplier will:
• Undertake fieldwork sample, primarily with young adults (16+), and adults (18+), both smokers and non-smokers
• Undertake fieldwork sample of young people aged 14 and 15.
• The methods to be used in the study will be sound, scientifically robust and able to provide data that will meet the ICB objectives, providing meaningful benchmark data, including but not limited to:
o Volume of legal and illegal tobacco consumed;
o Illicit share of the tobacco market in Humber and North Yorkshire;
o Sources of illicit tobacco purchases;
o Type of illegal tobacco bought;
o Proportion of smokers buying illicit tobacco;
o Segmentation of the illicit tobacco market by gender, key socio-economic groups, age and demographics and any meaningful shifts in the market;
o Attitude determinants to trying and buying illicit tobacco and willingness to report sales among non-buyers;
o Comfort levels about the illegal tobacco trade;
o Comparisons with data from other areas in the UK where available;
o Identification of any other emerging trends on how the market has changed over the last decade;
- Gather intelligence on tobacco and related products topics such as public support for specific tobacco regulations and knowledge of tobacco health harms as well as the impact of increases in the cost of living.
- Provide statistically reliable population-level data across a geographical footprint to be determined.
