Humber and North Yorkshire Integrated Care Board (ICB) requires support to develop Action Learning Collaboratives for Maternal Safety; and Frailty and Falls. The Action Learning Collaboratives will be co-developed and co-delivered with York and Scarborough Teaching Hospitals NHS Foundation Trust, Hull University Teaching Hospitals NHS Trust, and North Lincolnshire and Goole NHS Foundation Trust.

The Humber and North Yorkshire Health and Care Partnership has an evolving network focused on all aspects of transformation across the delivery of efficient and effective diagnostics services for the local population. The Supplier will undertake the following activities to support the Humber and North Yorkshire Diagnostic Programme: • Undertake a robust data validation exercise with each of the acute providers to provide assurance that weekly activity returns for diagnostics are reflective of true performance and all national guidelines; • Share best practice insights from work undertaken in other areas, to offer guidance and support to the Humber and North Yorkshire Diagnostic Business Intelligence Team with developing a system wide waiting list performance dashboard; • Undertake a productivity and benchmarking review of endoscopy and imaging services across Humber and North Yorkshire

The service will support individuals to: • manage their money & debt management incl budget planning, • maximise benefit entitlements, • access training, education and/or gain meaningful employment, • maintain/develop social circles, relationships & friendships, • forge positive relationships with the local community, • develop independent living skills & confidence to manage their own needs, • access specialist advice, make referrals to other professionals, • adopt healthy and active lifestyles therefore preserving independence and quality of life, • achieve their desired outcomes. Personal Support The Personal Support service aims to enable the individual to use and develop their personal strengths to the purpose of developing, maintaining, and thriving in positive social activities that are meaningful to the individual, including volunteer and work opportunities, and will be delivered via a personal budget. Hospital Discharge Volunteers Support Service (North East Lincolnshire) - the service will work with older people who become deconditioned during short hospital stays to re-enable them to optimum levels prior to discharge, creating impact in hospital (better flow) and in the community (reduced care packages), this will include: • providing a co-ordinated approach to reduce deconditioning when in hospital; and • supporting employed staff to engage with individuals through a range of initiatives designed to address reducing deconditioning collaboratively led by nursing and therapy staff supporting the workforce through the introduction of volunteer support workers. The intervention will include a variety of initiatives which provide purpose for individuals to get up and dressed each day and offer them an activity to do, so they don't become bored and disengaged. Working with identified individuals to keep them positive and focused on their recovery. The service will operate 5 days a week Monday-Friday, providing core coverage from 09:00-17:00hrs, and additional support at weekends where volunteer availability permits. Integrated Neighbourhood Teams (INTs) Training Development Programme - development and delivery of a targeted support and development programme for those leading and developing INTs The development programme will: • Develop strategies and plans for INT working that are genuinely innovative, challenge deep seated assumptions and avoid the difficulties of the past. • Create a deep understanding of integration and why integration is so deeply challenging. • Forge collective ambition and understanding of INT working that goes beyond co-location, MDT meets and basic coordination of existing roles. • Create clarity of purpose that can mobilise and engage others. • Provide opportunities to learn from and develop plans with others based on evidence and practical application.

NHS Humber and North Yorkshire Integrated Care Board (hereafter the Contracting Authority) is seeking to commission a Voluntary, Community and Social Enterprise (VCSE) Infrastructure Support (North East Lincolnshire) through an Open procurement process. The preferred Bidder will be required to demonstrate they are able to provide a coordinated, coherent and sector-focussed VCSE infrastructure support which has a demonstrable positive impact on maintaining a thriving voluntary community sector (VCS) in North East Lincolnshire. The Provider will collaborate closely with partners across North East Lincolnshire to ensure local organisations benefit from new developments and opportunities in the Borough. By working together, they will help maximise the positive impact of Devolution and the substantial government investment, with a focus on supporting job growth and enhancing the quality of life for the residents of North East Lincolnshire. The service will be required to: • Support to the VCSE organisations and groups to access sources of funding, including social investment. • Enable a voice and influence within the sector through facilitating and developing engagement and communication channels. This includes the promotion and awareness raising of the sector with relevant stakeholders. • Facilitate improved collaboration across the VCS and support for the enhanced role of the VCS within respective place partnerships (economic, social, health, environment and cultural). • Support the establishment of new, and the capacity of existing, VCSE organisations, to flourish in North East Lincolnshire, including those which reflect diversity within our communities. Health and care system partners in Yorkshire and Humber have identified that there is a need to develop a collaborative workforce training programme. The aims of the training programme will be to: • enable the adaptation of service models to priority groups; • develop tailored interventions to the individual needs of people who smoke particularly those in priority groups; • Very Brief Advice (VBA) that is relevant to non-clinical settings; and • provide effective first point of contact and triaging by administrative staff. It is proposed that by delivering advanced training to all health and care system partners in Yorkshire and Humber it will provide parity to communities, and offer equitable and quality interventions from each of the individually commissioned services across the Yorkshire and Humber area. The training programme will include: • Development of a suite of advanced training materials which will support the delivery of training to commissioners, managers, practitioners and administrators of stop smoking services in the Yorkshire and Humber area; • Very Brief Advice+ training for non-clinical staff; • Service adaption and intervention tailoring training courses; • Annual webinars

Assess the impact of the HNY Centre for Excellence in Tobacco Control Programme through research into tobacco, alcohol and eating habits, undertaking public perception surveys and evaluating the effectiveness of marketing and communications campaigns.

The service will provide effective management, co-ordination and signposting of High Intensity Users of the local Emergency Department (ED) at Scunthorpe General Hospital (North Lincolnshire). The service will reduce the activity High Intensity Users have on GP Practices and will establish, utilise and coordinate multi-agency and existing professional services to negotiate a reduction in 999 calls. The service aims to demonstrate a reduced workload on unscheduled care services and the wider health economy resulting from reduced 999 calls which otherwise would have attended ED and result in an admission. The service will safely manage and co-ordinate the chaotic and demanding nature of the patient group through the use of multi-agency support and the volunteer sector.

Development a local integration of the population Health web-based mapping business intelligence software platform, SHAPE Local, for the purpose of profiling and service planning for Places (East Riding of Yorkshire; Hull; North-East Lincolnshire; North Lincolnshire; North Yorkshire; and York), Primary Care Networks (PCNs) and Integrated Neighbourhood Teams (INTs). Segmentation visualisations - creation of visual indicators; and integration with the NHS Humber and Yorkshire ICB data warehouse - the use of SHAPE to help identify neighbourhoods and local area profiles, including drawing from the Humber and North Yorkshire segmentation data and utilising the tools available within SHAPE such as population targets The service provision will include analyst and development time and training sessions of the use of the software.

The Illicit Tobacco Survey service provision will design and implement a survey to provide a robust assessment of the illicit tobacco market in Humber and North Yorkshire, sampling in all 6 'Place' areas. The Supplier will: • Undertake fieldwork sample, primarily with young adults (16+), and adults (18+), both smokers and non-smokers • Undertake fieldwork sample of young people aged 14 and 15. • The methods to be used in the study will be sound, scientifically robust and able to provide data that will meet the ICB objectives, providing meaningful benchmark data, including but not limited to: o Volume of legal and illegal tobacco consumed; o Illicit share of the tobacco market in Humber and North Yorkshire; o Sources of illicit tobacco purchases; o Type of illegal tobacco bought; o Proportion of smokers buying illicit tobacco; o Segmentation of the illicit tobacco market by gender, key socio-economic groups, age and demographics and any meaningful shifts in the market; o Attitude determinants to trying and buying illicit tobacco and willingness to report sales among non-buyers; o Comfort levels about the illegal tobacco trade; o Comparisons with data from other areas in the UK where available; o Identification of any other emerging trends on how the market has changed over the last decade; - Gather intelligence on tobacco and related products topics such as public support for specific tobacco regulations and knowledge of tobacco health harms as well as the impact of increases in the cost of living. - Provide statistically reliable population-level data across a geographical footprint to be determined. • Reporting survey findings to Commissioners. The contract contains an option to extended the contract term by a further three (3) months

Evaluation of the impact of a mass media campaign for stop smoking, including pre- and post- campaign evaluation and production of an evaluation report.
