NHS Devon Integrated Care Board (ICB) seeks to commission a Provision of Services for Out of Hours GP Support to Devon Community Hospitals - South and West Regions. The service will be provided to patients residing as in-patients in the Devon Community Hospitals - South and West Regions. Aim / Purpose of Service To provide urgent (as opposed to emergency or routine) primary care support to Devon's Community Hospitals during the out-of-hours period where it is required, and the patients' needs cannot be appropriately met in the in-hours period. Hospital staff will be able to seek the advice of a clinician, who will arrange for a GP to attend the hospital to undertake a face-to-face assessment if clinically indicated. This is likely to include (but not be limited to): • Patients who have had an acute deterioration and it is not clear or appropriate to directly admit to hospital. • Patients at or nearing end of life who require an urgent medical review A full specification and list of defined requirements and outcomes are available to view once registered on the Atamis e-procurement portal. Contract Terms The Initial contract duration will be for 6 months with an optional extension of up to 6 months with services due to commence on the 2nd of December 2023. This will range from babies of a few days old and their families to the elderly. The service includes diagnosis of, quantification of degree, and rehabilitation of hearing loss, tinnitus or balance impairment. It also includes provision of supporting services for patients who may be considering or have bone anchored hearing aids and cochlear implants in addition to providing audiological support to ENT inpatient and outpatient services, as commissioned by those service providers. NHS Devon Integrated Care Board (ICB) wishes to commission a single provider to support individuals who may require some additional assistance on discharge from hospital (or another care setting), from an unregulated provider. This will be a flexible service that will offer a range of support in a person centred way. This could be a few hours support to help someone on their first day at home, assisting with shopping or providing a few visits and/or telephone support over a limited number of weeks. The aim of the service is to provide support to patients at home once they become medically fit, so that they can regain independence at home and reduce the risk of deconditioning in hospital. Key principles of the service: • Work as a key member of the multi-disciplinary Care Transfer Hub to identify individuals who would be suitable for accessing the service. • Act as single point of access and triage for individuals identified as appropriate for this service. • Deliver a flexible, person centred, solution focussed service embedding the principals of a Discharge Personal Budget (the Discharge Person Budget enables a unique solution for each service user within the unit cost). • Ensure service users are settled in safely at home after a spell in hospital. • Support service users to become independent at home again. • Improve service users' outcomes and wellbeing post discharge. • Where support exists already, to refer/signpost and not duplicate. • Develop and maintain knowledge of other services and networks and ensure service users are linked to information and encouraged to access support which may reduce anxiety and enhance ability to continue living independently in their home e.g. benefits, community meals, befriending, day centres. • The support will seek to reduce the impact on POC delivered in the community by Domiciliary Care, in turn improving hospital flow and discharge (reduce reliance on statutory care). • Reduce risk of readmission • Reduce discharge delays. • Reduce average length of stay in community beds. • Improve 7 day working through supporting discharges at weekends. • Support early flow by participating in early discharge planning before the day of discharge. • Support discharge to usual place of residence in line with Better Care Fund (iBCF) Metrics. The contract value is £395,735 per annum with a total contract value of £1,187,205 for the 3 year period, with an optional extension period of 2 additional years at the commissioners discretion.

NHS Devon Integrated Care Boards (ICB) ("the Commissioner"), is seeking to secure a Prime Provider to deliver children and young people's emotional wellbeing and mental health services across the NHS Devon ICB footprint. 2.) It may be that a single provider is unable to deliver all elements within the Commissioner's requirement and therefore partnership working may be required but please note that The Commissioner is seeking to place a single contract. The Commissioner welcomes partnerships, consortiums and/or sub-contractual models at Bidders' discretion but wish to contract with a sole Prime or Lead Provider only. 3.) The geography covered is the NHS Devon footprint and aligns to the three Local Authority footprints of Plymouth, Devon and Torbay. 4.) Services are scheduled to commence on 1st July 2025. The Contract will be for an initial term of 3 (three) years and 9 (nine) months, with an option to extend for any period up to a further 3 (three) years, as defined at the sole discretion of the Commissioner. 5.) The proposed model includes digital provision, delivery of evidence-based interventions and a youth work-based approach. Further detail of these aspects and the broader requirement are captured in the Specification, which will be provided to registered bidders. 6.) In the contract lifetime a range of potential changes are foreseeable, including but not limited to: • The geographical coverage of the 'targeted' provision maybe adjusted or additional geographical areas added. • The inclusion criteria to access the provision maybe adjusted between 0-25 years. • The access requirements maybe increased, such that the provision is asked to expand its offer, in relation to specific components, or across the totality of the provision, to enable access to more children and young people. • The commissioners may change to include a range of other partners including but not limited to Local Authorities and schools. Any or all of the above changes would be associated with a need to vary the contract and maybe associated with additional investment. 7.) Orthodontics is the dental specialty concerned with facial growth, development of the dentition and occlusion, and the assessment, diagnosis, interception and treatment of malocclusions and facial irregularities. NHS Devon Integrated Care Board is seeking a provider who has the capability and capacity to deliver a digitalised Continuing Healthcare (CHC) end to end person management digital system, where information seamlessly flows between processes by connecting the entire lifecycle into one platform. The system needs to facilitate the services to deliver enhanced transformation across all spectrums of the service. Devons CHC Service provides high quality, safe care to Devon's most vulnerable Adults and Children, and currently has a combined active caseload of around 3,000 individuals. The ICB is seeking an efficient and effective digital system to manage the caseload and the inherent complexities associated with the services. Devon's caseload, plus combined legacy data is circa 3000 records. The new system is in the first instance to replace two existing systems. For Devon ICB this is a replacement for an off the shelf record system. For Torbay and South Devon, there is a locally developed bespoke system that requires replacement. It is intended the new replacement system will operate for both organisations. One of the key requirements for a new system will be the need for it to be sufficiently flexible to support a wide range of services outside the traditional CHC / Funded Nursing Care (FNC) / Fast Track (FT) / Continuing Care (CC) agenda. Service wide transformation will be implemented across all areas including clinical, business, performance, finance and contracting and commissioning, enabling better integration between systems across Health and Social Care is also critical. CHC is a unique service as it covers and links many elements of the health and social care system. Therefore, interoperability is of paramount importance, allowing for the CHC service and wider Health and Social Care teams to share and view records. This will support improved accessibility and visibility of data which in turn will enable more efficient and streamlined workflows for all teams and a better person experience. The Commissioner has a maximum financial envelope available of £200,000 per annum, inclusive of VAT. The total maximum contract value for the 4-year Contract is £800,000 and (inclusive VAT). At the end of the initial contract term the Commissioner will have extension options of up to 3 years in total, made of 3 x12 month periods, which will be awarded at the commissioner's discretion, giving a maximum possible contract value of £1,400,000 inclusive of VAT over 7 years if the full contract term is fulfilled. As laid out in the tender documentation, this is single lot competitive process under the Provider Selection Regime 2023 (PSR) regulations. The geography covered by this service would be the NHS Devon footprint which aligns to the three Local Authority footprints of Plymouth, Devon and Torbay, working in communities across Devon, Plymouth and Torbay, with community-based services and within the four acute hospital settings.
