As part of the NHS Long Term Plan Ambitions, Living Well Derbyshire are working with Community Mental Health Teams, Local Authorities and the Voluntary Sector to create 'Provider Collaboratives' in local areas to support the improvement of outcomes and reduce out of area placements. The focus is to expand on community-based support for people in their own locality.
The Collaborative is an inclusive forum where people come together to develop a shared understanding of the current system, listen to local voices, shape the offer and respond to local mental health needs.
The purpose of the Leadership Co-ordinator Role is to steer the monthly collaborative meetings for each individual location and to support and promote the involvement of provider networks which sit in each local area.
