NHS Devon Integrated Care Board (ICB) seeks to commission a Provision of Services for Out of Hours GP Support to Devon Community Hospitals - South and West Regions. The service will be provided to patients residing as in-patients in the Devon Community Hospitals - South and West Regions. Aim / Purpose of Service To provide urgent (as opposed to emergency or routine) primary care support to Devon's Community Hospitals during the out-of-hours period where it is required, and the patients' needs cannot be appropriately met in the in-hours period. Hospital staff will be able to seek the advice of a clinician, who will arrange for a GP to attend the hospital to undertake a face-to-face assessment if clinically indicated. This is likely to include (but not be limited to): • Patients who have had an acute deterioration and it is not clear or appropriate to directly admit to hospital. • Patients at or nearing end of life who require an urgent medical review A full specification and list of defined requirements and outcomes are available to view once registered on the Atamis e-procurement portal. Contract Terms The Initial contract duration will be for 6 months with an optional extension of up to 6 months with services due to commence on the 2nd of December 2023. Financial Envelope There is a set Finance Envelope of £190,000 per annum The Financial envelope is £95,000 for the initial 6 months and then up to a further £95,000 for the up to 6 months extension. The envelope is exclusive of VAT as the service is exempt. Contract. The Commissioner will be contracting for the services using the NHS Standard Clinical Contract (long form) 2023/24 version. 1.) The Commissioner would like to understand the level of provider interest in delivering these services. 2.) It may be that a single provider is unable to deliver all elements within the Commissioner's requirement and therefore partnership working may be required. A single NHS standard contract to deliver children and young people's emotional wellbeing and mental health services, in line with the specified requirements is being considered. The Commissioner is seeking to place a single contract, they welcome partnerships, consortiums and/or sub-contractual models, which is at Bidders' discretion. 3.) The geography covered is the NHS Devon footprint and aligns to the three Local Authority footprints of Plymouth, Devon and Torbay. 4.) The proposed model includes digital provision, delivery of evidence-based interventions and a youth work-based approach. 6.) In the contract lifetime a range of potential changes are foreseeable, including but not limited to: • The geographical coverage of the 'targeted' provision maybe adjusted or additional geographical areas added. • The inclusion criteria to access the provision maybe adjusted between 0-25 years. • The access requirements maybe increased, such that the provision is asked to expand its offer, in relation to specific components, or across the totality of the provision, to enable access to more children and young people. • The commissioners may change to include a range of other partners including but not limited to Local Authorities and schools. Any or all of the above changes would be associated with a need to vary the contract and maybe associated with additional investment. 7.) TUPE may apply. The Commissioner would like to understand the level of provider interest in delivering these services. There is a requirement for the development and ongoing maintenance of a bespoke website, which will serve as a comprehensive and reliable source of health information and signposting for maternity, children and young people (CYP), families, and their healthcare professionals for whom NHS Devon is the responsible Commissioner. It will need to have the ability to provide healthcare information to support self-management, signpost to appropriate services according to need, and provide appropriate advice through locally and nationally recognised sources. There will need to be information provided to be in line with nationally recognised on a traffic light system/health RAG rating for individuals to help identify the appropriate service for health care navigation from self-care through to 111/999 ambulance for common childhood illnesses, if there are worries or concerns. National information will need to be sourced through recognised websites along with evidenced based information and health service navigation in an easily accessible format, for example the traffic light system for feverish illness recommended by the National Institute for Health and Care Excellence (NICE). There will be a requirement to publish local clinical pathways and information that has already been approved/passed through ICB governance - ideally a level of usable website upfront ahead of development of local pathways. There will be a requirement for the provision of technical innovations and development to the website as required, in line with user feedback.
