NHS Arden & Greater East Midlands Commissioning Support Unit (AGEM CSU) on behalf of NHS Coventry and Warwickshire Integrated Care Board (ICB) (referred to as the Commissioner) wish to inform the market of an opportunity to provide Intermediate Minor Oral Surgery (IMOS) including treatment under Conscious Sedation across the Coventry and Warwickshire ICB area for the following lots;<br/><br/>Lot 1: Coventry<br/>Lot 2: Nuneaton or Bedworth<br/>Lot 3: Rugby<br/>Lot 4: Warwick or Leamington Spa with a spoke site in Stratford -upon-Avon<br/><br/>The specialty of Oral Surgery (OS) deals with the diagnosis and management of pathology of the mouth and jaws that requires surgical intervention. Oral Surgery involves the treatment of children, (as set out in the IMOS specification found in document 3 (published with the competitive process documentation) in respect of procedures 3 and 4 only), adolescents and adults, and the management of dentally anxious and/or medically complex patients.<br/><br/>Oral Surgery care is provided by Oral Surgeons and by Oral & Maxillofacial Surgeons as the clinical competencies of these two specialties overlap. <br/><br/>Lot 1 to Lot 4, contract duration is for 7 years (total value £10,126,171) with the option to extend for a further 3 years (total contract duration 10 years, total value £14,465,960). A separate Personal Dental Services Agreement will be established with the preferred provider for each Lot. NHS Coventry & Warwickshire ICB in conjunction with NHS Herefordshire & Worcestershire ICB are seeking providers who have the relevant experience and skills to provide an Ophthalmology Co-ordination Service Hub.
The Ophthalmology Coordination Service Hub (OCS) will support the system-wide approach to the management for all non-emergency (routine and urgent) ophthalmology referrals from community Optometry practices across Coventry and Warwickshire. The OCS will provide a single point of access, administering all incoming referrals from community Optometrists, offering patients a choice of providers in accordance with patient choice regulations, and submitting onward referrals to secondary care (accredited independent sector or NHS provision).
The OCS supports the streamlining of ophthalmology referrals across the ICS, reducing delays to patients being referred from the community to secondary care and freeing up primary care capacity. For every routine and urgent referral, the service will provide patients with choice of access based on the referring condition, supporting patients being seen by the right person, in the right place, the first time.
The service will not undertake clinical triage of incoming referrals.

Aims:
• To reduce the number of inappropriate hospital admissions for people with a learning disability or autistic people who are in mental health crisis and may have complex needs, by providing intervention and crisis management.
• To prevent crisis situations from escalating by providing timely and appropriate interventions.
• To provide a safe and supportive environment for people with a learning disability or autism at risk of inappropriate admission to a specialist learning disability, mental health or acute hospital setting.
• To promote independence and improve quality of life by supporting individuals to manage their mental health and develop coping strategies.
• To provide short term support enabling a return to home for those referred to the service from inpatient settings including acute settings.
Objectives:
• To provide a short term, 24/7-person centred community service for people with a learning disability or autistic people who are at risk of inappropriate admission to a specialist learning disability or mental health hospital.
• To provide a holistic model of care within the community.
• To support individuals who are at the point of crisis through implementing a personalised, flexible and rapid service designed to meet the needs of all referred individuals. This includes robust personalised planning and communication embedded into the referral and placement process.
• To support individuals to maintain positive relationships with family and friends and other support networks, engaging with these networks and other professionals to enable collaborative working to support the individual.
• To work collaboratively with other health and social care professionals to provide coordinated and person-centred care, including working jointly with existing or subsequent care providers to ensure continuity of support and effective discharge.
• To provide person-centred support with time limited episodes of planned individualised care, which should inform the focus of approach based on each service user’s needs. For example - dedicated staff teams will work with specific service users during their access to the service, to promote consistency in approach, enhanced knowledge of the service user and from this meaningful, purposeful and appropriately risk managed interventions and onward movement.
• Where possible, the service will work with commissioners to pro-actively identify individuals who may benefit from earlier intervention and provide support to prevent crisis.
