To provide a Supported Living/Supported Housing service for people with mental health needs and/or a learning disability, and to (where possible and appropriate) act as a step towards independent living. Support will be delivered in a way which promotes individual’s independence; enabling and encouraging individuals to carry out tasks themselves. This service is a support service, not a personal care service. Personal care should not be required by individuals living at the scheme. Where personal care is required alternative care provision will be sourced. This service is primarily focused on supporting people with a mental health need and/or a learning disability which could include (but is not exclusive to) a diagnosis of Bi-polar disorder, schizophrenia or autism and could include some behaviours who require support and guidance in regaining independent living skills and managing their mental health. Decisions about appropriate placements will be made in conjunction with Monmouthshire Council Community Learning Disability and Mental Health Teams, and once the service is up and running will take in to account the needs and wishes of the residents living at the property at that time. Monmouthshire County Council are proposing the establishment of a collaborative multi‑lot framework agreement to deliver general building maintenance and repair works to the non-domestic property portfolios across 5 authorities. Monmouthshire County Council (MCC), wishes to establish a single provider contract for the provision of high quality and cost-effective maintenance services, and to benefit from the economy of scale of combining the two council networks. MCC is managing the procurement process on behalf of the participating authorities (Monmouthshire CC and Torfaen County Bourough Council), in accordance with the Procurement Act 2023. This is a services contract being procured under the Open procedure, and any contract award will be subject to the terms and conditions contained within the tender documentation. Upon Award, each authority will enter into its own contract with the successful supplier. On the above basis, MCC is inviting Tenders from suitably qualified and experienced contractors for the provision of, and maintenance of street lighting and illuminated traffic signs, along with associated facilities and services, as detailed within the contract documents. These term maintenance contracts are being tendered by MCC for itself, and Torfaen County Borough Council (TCBC). This will take the form of two separate NEC3 2006 (Option A, with bills of quantities) contracts with a shared Preliminary Items section. It is the requirement of the contract to provide all aspects of street lighting, illuminated traffic signs, maintenance and installation, within the geographical areas specified within the contract. The works will include equipment renewal, refurbishment, replacement and upgrade, inclusive of civil engineering and traffic management elements as appropriate. It should be noted that the contracts will also be utilised to install new equipment as and when required, and therefore the overall contract value may increase. Overview The contract requirement consists of the maintenance of street lighting, illuminated road traffic signs, bollards, subway lighting, pedestrian refuge beacons, school crossing flashing signs, remote monitoring, dimming systems, bus shelter electrical systems, vehicle activated signs, vehicle electrical charging points and feeder pillars. This is a collaborate procurement between Monmouthshire County Council (MCC) and Torfaen County Borough Council (TCBC). We are recommissioning a hospital prevention and discharge service designed to help individuals remain or resettle in their own homes after hospital admission. This is a preventative service that does not require CIW registration and will not provide formal care. It aims to support people over the age of 50 living in Monmouthshire who lack suitable family or community support. The service will operate five to seven days a week and provide short-term interventions to enable safe hospital discharge or prevent admission. It will work closely with social care teams, hospital discharge teams, and community healthcare professionals across Monmouthshire hospitals and other local hospitals, including Royal Gwent and The Grange. Referrals may also come from community healthcare teams managing individuals at risk of admission. Support will focus on practical assistance and signposting. This includes helping individuals organize online shopping, access transport, and connect with community resources. Once immediate needs are met, the service will encourage social engagement and financial stability by facilitating benefit checks, linking to third-sector organisations, and arranging services such as Meals on Wheels. Successful provider must have strong knowledge of local resources and voluntary sector support. Although primarily delivered face-to-face, the service will include follow-up phone calls and emails to monitor progress and make referrals where necessary.

Monmouthshire County Council is inviting Expressions of Interest (EOIs) from training providers, apprenticeship partners, FE institutions, and specialist skills organisations interested in delivering qualifications, vocational training, and apprenticeships as part of the development of a new Monmouthshire Multi Skills Centre/skills hubs. This notice forms part of early market engagement to assess the market’s appetite and capacity to support the creation of a high quality, industry aligned Skills Centre/hubs in Monmouthshire. Two potential locations to develop a centre have been identified: Innovation House Magor and Castlegate Business Park Caldicot; but we are open to suggestions for other locations. Monmouthshire County Council has recently developed a Strategy for Commissioned Domiciliary Care in Monmouthshire 2024-2034. This strategy sets out a plan for the redesign and recommissioning of care across Monmouthshire to meet the needs of current and future residents in the County. The strategy contains a high-level 2 phased implementation plan, phase one of the plan will focus on tendering for the provision of long-term domiciliary care and support in the South of the County from the autumn of 2024. Long term care and support will include the provision of personal care for older people, people with disabilities and mental health needs and people with dementia. We anticipate the tender will comprise of a series of block contracts, which will be the primary mechanism for the delivery of long-term domiciliary care and support in the South of the County. Monmouthshire County Council is seeking to commission an experienced organisation to provide semi-independent supported lodging support to Young People with complex support needs within Monmouthshire. The service will support a maximum of five (5) young people at any one time, who will be aged sixteen (16) to twenty-four (24) and who have varying support needs, some of which will be complex. The Council aims for each young person to step down to other suitable accommodation by the age of eighteen (18) when safe and appropriate to do so. The service will aim to help offer a stable, consistent environment, so that the young people can be encouraged to build their independent skills. This will enable them to reflect and make appropriate choices for their future, enabling them to live independently and successfully within the community. The support offering will be person centred, outcome focused, trauma informed and will aim to build the young people’s skills to support successful transitions into independent living. This will be provided via a dedicated supported accommodation pathway with wrap-around trauma informed support. Support will be designed and delivered to support the young people to build resilience, promote independence, develop or sustain their capacity to live in their own accommodation and achieve increasing levels of independence. The aim is not to just provide a young person with a home but to ensure they have a healthy transition into adulthood. This will be underpinned by a suitably trained and competent staff team who will deliver trauma informed support which will include structured diversionary and engagement activities.

Providing high-quality sensory impairment service to residents of Monmouthshire who have a sight, hearing, or dual sensory impairment. The service addresses the need to provide early diagnosis and intervention and ensures that where possible people are taught skills and strategies to minimize the impact of their sensory impairment and help them continue to maintain a healthy and independent lifestyle. Providing: -One and a half full time (1.5 WTE) sensory impairment specialists with an approximate case load of 18-20 people per month. -Advice, training, and development including providing sensory impairment training for professionals and carers throughout the year. -Preventative strategies that delay or mitigate the impact of potential sight, hearing, or dual sensory loss. -Aids and equipment; issuing key equipment to help maintain independence (this equipment is sometimes at a cost to the individual). Provides a hospital discharge service (HDS) in Monmouthshire and is available throughout the county to residents over the age of 50, including -Those over 50 who may have some degree of confusion but do not require specialist care. -Those assessed as likely to benefit from short term and/or intensive support within their own homes. -Users at the point of hospital discharge, and for up to 6 weeks. The HDS provides practical and emotional support and encouragement, easing the individuals transition from hospital to home and on to independence. The team are alert to individual’s problems in the post-discharge period and will refer or signpost to other services, or for readmission as necessary. Support can be provided in a variety of ways and is tailored to the individual’s needs, with a focus on confidence building. This may include support with shopping, regaining independence in the home, signposting to other services and information and advice around benefits. The service is usually offered for a maximum of 6-8 weeks but is tailored to an individual and may be shorter or on rare occasions longer dependent on circumstances. The team work closely with the hospital discharge team and can receive referrals directly from hospital, social workers or from individuals and their families. Networking and promoting is also essential across both health and social care to ensure the appropriate people are aware of this service.

The Local Links service enables older people over the age of 60, (including those with a mild/moderate mental health issue, including anxiety, depression, dementia) to improve or maintain their quality of life, it is particularly focused on those who are socially isolated and would benefit from the support of attending a local weekly group in their area around Monmouthshire. The service is predominantly focused on the delivery of clubs in people’s local areas, these may be run in community centres and areas with guest speakers and or based in local pubs or restaurants to ensure people are meeting others in their local community. Forging friendships and networks, volunteers then look to run the groups allowing the coordinator to oversee and move onto redeployee elsewhere into the development of new clubs or group. The staff and clubs also offer an informal way of supporting people’s health and wellbeing and offer signposting to other service areas and support.

The funding supports delivering of social activities for older people including those with a mild/moderate mental health issue such as depression, anxiety, dementia. The provision of this low-level social support service is a key component in supporting people to be engaged, preventing isolation and loneliness and complements the social care strategic aspirations of the Council. The scheme aims to reduce social isolation, provide people with links to their local community and the opportunity to build and develop friendships. Attendees also have the opportunity to purchase a reasonably priced lunchtime meal.
