NHS Arden and Greater East Midlands Commissioning Support Unit (AGCSU) on behalf of Hertfordshire and West Essex (ICB) (The Commissioner) would like to notify the market of an opportunity to provide a Special Allocation Scheme Service in the following areas: East and North Hertfordshire West Essex South and West Essex The service commencement date is 1st April 2024. All interested providers are invited to participate in an early market assessment questionnaire. Please note the deadline for expressions of interest is 12:00PM on 30th August 2023. The service(s) will provide GP led primary medical care services to patients registered on the Special Allocation Scheme (SAS) who have been removed from general practice due to exhibiting violent or aggressive behaviour. The service(s) shall also provide support for rehabilitation of Registered Patients in order to address the underlying causes of violent or aggressive behaviour and encourage and support facilitation of return to general practice. The service(s) must be offered throughout Core Hours of 8am - 6.30pm Monday to Friday. Each area has around 12-15 patients and the estimated annual contract value for each area is £30,000. It is the responsibility of the Contractor to secure and fund appropriate and safe location(s) from which to deliver the service(s). The location must be; a) Suitable for the delivery of primary medical services; b) Appropriately registered with the Care Quality Commission (CQC); c) Safe for staff, Registered Patients and other patients; d) Accessible for Registered Patients within the Hertfordshire and West Essex ICB area e) A stable environment which actively reduces anxiety for Registered Patients. This is purely an early engagement exercise; any subsequent procurement will be advertised separately and all organisations wanting to participate will need to respond to the procurement advertisement as and when published. This is a market testing exercise and does not commit the Commissioners to carry out a procurement process. Current services will remain in place during this period in line with existing contracts and service level agreements. The procurement will be advertised in accordance with the requirements of the Public Contracts Regulations 2015 (as amended) (the "Regulations") to the extent that the Regulations relate to services within the scope of Schedule 3 thereto(Social and Other Specific Services). The Commissioner and AGCSU are, therefore, bound only by those parts of the Regulations applying to Schedule 3 services. If more than one compliant provider registers an interest for this opportunity, meets the requirements set out in this Notice and the questionnaire then the ICB may set out details for a competitive process. Hertsmere residents have access to routine and same-day GP appointments, extended access appointments, and out-of-hours services. However, Hertsmere lacks its own Accident & Emergency (A&E) facility, necessitating residents to rely on lengthy private or public transportation journeys to neighbouring areas to access urgent care - a considerable undertaking, considering Hertsmere is home to some of HWE most deprived communities. Low acuity A&E activity from Hertsmere could increase by approximately 27% over the next 2-3 years additionally same day primary care appointments account for between 45% and 56% of all Hertsmere’s primary care activity, with this activity set to increase over the next 5 years. To address this expected surge in demand and bring urgent care closer to Hertsmere residents, it is proposed to establish an Advanced Nurse Practitioner (ANP) led minor illness service within Hertsmere. The ANP minor illness service will operate from one Borehamwood based GP surgery and one surgery in Potters Bar PCN. The service will provide capacity to see a total of 60 appointments (44 appointments in Borehamwood location and 16 appointments in Potters Bar location) a day across both sites (excluding weekends, public and bank holidays) – totalling 15,120 appointments annually. The intention is to offer a 12 month contract with the option to extend by a further 12 months. The contract value split over the two sites is £400,00 in year one and £400,000 if the extension is mutually agreed.

Prior Information Notice (PIN) The purpose of this PIN is to inform the market that the Authority has made the decision that working with a collaboration of existing local providers, will be the best solution to provide an affordable, sustainable, and fully Integrated service model in Hertfordshire and West Essex. The Authority and its incumbents will work collaboratively to ensure delivery of the Integrated Urgent Assessment and Treatment Centre Service objectives, standards, and outcomes. Possible provider stakeholders could be, but are not limited to: • Princess Alexandra Hospital NHS Trust • Primary medical care – GP practices and Primary Care Networks, GP Provider Companies/Federations • Herts Urgent Care (HUC) • Essex Partnership University Foundation Trust (EPUT) • Hertfordshire Community Trust (HCT) • East of England Ambulance Service Trust (EEAST) • Essex County Council, Hertfordshire County Council, District Councils • Voluntary sector This arrangement will be contracted via either an Alliance Contract or a Lead Provider arrangement, and by working collectively the providers will achieve better patient outcomes for Hertfordshire and West Essex and will help to facilitate a more integrated, resilient healthcare system. The contracting model will be agreed as part of the due diligence process. The Contract to be entered into by the Authority and the partnership of providers will be the NHS Standard Contract 2023/24, and subsequent revisions thereof. In addition to the NHS Standard Service Contract the providers will be required to agree and sign a Collaborative Agreement with each other and the Authority. The Contract is intended to run for 5 + 2 years commencing on the 01.11.2023. In accordance with Regulation 72 (1) (a) of the Public Contracts Regulations 2015, the services covered by the arrangement may change and expand as the partnerships develop through a Collaborative Contract. To participate in discussions regarding fulfilling the role of Integrated Urgent Assessment and Treatment Centre Service provider, a provider must: (i) be willing to work with a group of providers as outlined above (PAH and primary care as a minimum) (ii) (ii) have previous experience of multi-disciplinary integrated care service delivery, (iii) (iii) a willingness to sign up to appropriate system wide outcomes, (iv) (iv) have a good understanding of the local system and operating environment, and (v) have the necessary financial capabilities to hold and manage the scale of risk of overspend for the contract and be willing to participate as a part of the wider Hertfordshire and West Essex Integrated Care Partnership (ICP) through taking a degree of income risk share and signing up to an Alliance Agreement with all other key provider stakeholders in the system. The Authority is inviting interested providers who are willing to make the commitment to collaboration in an alliance way of working, and who believe they have the necessary capabilities and experience set out above to respond to the Request for Information (RFI). Market engagement for Primary Medical Services for Hertfordshire and West Essex Integrated Care Board - The Limes GP Services & Sollershot Surgery GP Services The purpose of this combined Prior Information Notice (PIN) is to invite interested providers in regards to two entirely separate contract opportunities listed below*. There will be a single market engagement and procurement training webinar and Request for Information (RFI) process to cover both opportunities. The deadline for the submission of the RFI is 12 Noon 30th March 2023. The primary care medical services stated below* will be selected, as appropriate, which may be through a full competitive procurement, as separate services and contracts. The separate services which are subject to this PIN, RFI and market engagement combined with procurement training are: *The Limes GP Services - This is an APMS contract which is due to end on 31st January 2024. *Sollershot Surgery GP Services - This is an APMS contract due to expire on 31st December 2023. Spring House: A PIN has already been issued for Spring House alongside relevant market engagement. Providers who are interested in bidding for GP Services at Spring House can attend the market engagement event associated with The Limes and Sollershott Surgery IF they require any procurement training. This procurement training will be delivered after the market engagement presentation for The Limes and Sollershott Surgery, in the same session. For the removal of doubt the market engagement element is associated with The Limes Surgery and Sollershott Surgery and not for Spring House GP Services and therefore no RFI’s are invited for Spring House GP services. The purpose of this PIN is to invite interested providers, who are willing to make the commitment to collaboration in an alliance way of working, and who believe they have the necessary skills and experience to deliver the requirements set out to respond to a Request for Information. Hertfordshire and West Essex ICB has a vision to deliver a truly integrated community gynaecology service working collaboratively across Acute, Community and Primary Care to provide specialist assessment and treatment for gynaecological conditions delivered in community settings closer to patients’ homes to improve the quality of care delivered for patients with gynaecological conditions in South and West Hertfordshire (SWH). The SWH vision is for the Enhanced Community Gynaecology Service (ECGS) to be the default preference for patients requiring specialist assessment and treatment for gynaecological conditions within the South and West Hertfordshire area. The service will provide an integrated model for gynaecology which will include out-patients, fast access to a range of diagnostic and out-patient procedure activity currently undertaken in existing community and secondary care Gynaecology services. In addition to specialist assessment, treatment and non-complex procedures the service will act as a mechanism to ensure appropriate onward referral into secondary care in-patient elective care. The provider will be expected to have in place close working relationships with the Acute Services to operate direct listing with the ICB’s main providers of Secondary Care. Further information on the service to be provided is in the attached draft Service Specification for the Enhanced Community Gynaecology Service which can be found within the Suppliers attachments area of the Bravo e-procurement portal. To this end, the ICB has been working in partnership with its key provider stakeholders (the incumbent providers) to bring the three pathways across Acute, Community and Primary Care together into an integrated multi-disciplinary model which meets that vision and improves the quality of care for the patient population of South and West Hertfordshire. Through these discussions with current incumbent stakeholders, HWE ICB believes it has identified the providers most capable and uniquely placed of leading and delivering the Enhanced Community Gynaecology Service. Hertfordshire and West Essex ICB has an ambitious strategy to redesign adult respiratory services. The new service will provide a truly integrated respiratory team capable of delivering "joined up care at the right time in the optimal setting" as set out in the NHS Long-Term plan. The aim is to develop an integrated care model for respiratory which is focused on delivering integrated pathways and transitioning the system away from individual organisational models of care. The new service will incorporate a multidisciplinary approach (including Primary, Community and Secondary Care) to enhance the care of patients with respiratory disease. The delivery model will focus on prevention, identification and early intervention and on-going management of respiratory conditions, , which for the South and West Hertfordshire population will aim to: • Reduce their risk of developing respiratory disease • Ensure equitable access to early and accurate diagnosis of respiratory diseases, with early intervention to improve quality of life and slow disease progression. • Provide high quality, consistent and integrated care for those living with respiratory diseases, with treatments and support provided in the most appropriate setting. The service will also deliver a service for patients who have been diagnosed with post-Covid syndrome. The Adult Integrated Respiratory Service will be required to fully integrate with existing commissioned services e.g. Rapid Response, Respiratory Virtual Ward, to ensure that patients are managed on the most appropriate treatment pathway and that they experience a seamless transition of care.

Market Information - Non-Emergency Patient Transport Services (NEPTS) for Hertfordshire and West Essex Integrated Care Board formerly NHS Herts Valleys Clinical Commissioning Group, NHS East and North Hertfordshire Clinical Commissioning Group and NHS West Essex Clinical Commissioning Group. The key objectives of the Procurement are to establish a contract with a patient transport Provider who can provide a high quality, cost effective service, efficient, and reliable transport service for patients. It is also the expectation that the service is comprehensive and resilient, and the provider will work as a partner within the local health system, adapt to the evolving landscape, and be able to respond to the system challenges. The scope of NEPTS will include patients travelling to and from renal dialysis, patient journeys within the ICB, high dependency unit, and 'out of area' journeys from the ICB to other regions out of area including London acute trusts. The Commissioners intend to procure the NEPTS under a single contract to cover the requirements of the entire region of Hertfordshire and West Essex.
