The commissioner seeks an experienced provider who will deliver an innovative and effective service that will promote and protect the mental health of survivors of rape and/or sexual abuse that occurred at any time in their lives through the provision of therapeutic interventions that responds to complex trauma and diverse needs. The contract term will be 6 years, with a maximum budget of £219,000 per annum and a total contract value of £1,314,000. Commissioners reserve the right to add funding to the contract over its duration to accommodate service development measures or initiatives, up to an equivalent value of the original contract. For the avoidance of doubt, this would mean an annual contract value of, for example, £440k per annum and a consequent increase in the overall contract value. NHS Bath and North East Somerset, Swindon and Wiltshire Integrated Care Board ("BSW ICB"), Somerset Integrated Care Board ("SICB"), Bath and North East Somerset ("BaNES") Council, Swindon Borough Council and Wiltshire Council (the "Local Authorities") are responsible for commissioning Health and Care Services including Public Health Services within their localities. References below to the "Commissioners" are to BSW ICB, SICB and the Local Authorities collectively. This procurement is driven by the need to re-procure Community Services throughout all life stages. In line with the BSW Care Model, the life stages being procured are defined within two main groupings: • Starting Well o To provide children, young people, and families with support and care in the community, physical and emotional wellbeing, Community health and targeted support. • Living/Aging Well o Supporting self-care and prevention, proactive care and support, responding to urgent needs, rehabilitation and recovery from ill health and/or crisis. The Dying Well element explored as part of the Market Engagement covering Palliative and end of life care in all settings and bereavement support is included in the two main groupings. The procurement seeks to appoint a sole provider or consortium of providers with a nominated lead provider to provide a service which is innovative, meets patients' expectations and fits the needs of the BSW population now and throughout the contract period. Contracts will be for an initial term of 7 years with a possible extension of any period up to a further 2 years at the discretion of the Commissioner. Services are planned to commence from 01/04/2025. The scope and value of the contract with the Provider will be based on the initial financial envelope. As a minimum, the Provider will be responsible for the delivery of the core services that are defined within the Memorandum of Information within a budget of c. £138m (including VAT) per annum. There is a potential that the scope and value may change through the procurement process and during the contract term. The Commissioners are seeking services to be delivered innovatively in a way that meets the needs of the population now and throughout the contract period. As such, the Commissioners will expect the Provider to continuously engage with stakeholders and, in agreement with Commissioners, evolve the services ensuring they are fit for purpose and meets patients' needs. The services needs to work in an integrated manner offering seamless community-based care, particularly from a patient's viewpoint. Due to the complexity of the re-commissioning, and the complex nature of the community services being commissioned, the Commissioners are using a phased approach to secure the transformational change required. To enable this change and further integration of services, the Commissioners reserve the right to include the services defined as 'reserved services' within the Memorandum of Information within any contract without recourse to a further notice or procurement process. The combined total value of these reserve services is estimated to be c. £17m (including VAT) per annum. Any further investment in addition to the c. £138m (including VAT) per annum available for core services and the c.£17m (including VAT) per annum available for reserve services will be determined during the procurement process or the contract term alongside opportunities to improve value for money. As such, Commissioners have taken a view that over the life of the contract further investment and inflationary increase may be required which has been included in the total estimated value of the contract. The service seeks to identify, engage, understand, and support people through a person-centered approach who repeatedly and often escalate a number of presentations to the emergency departments at Royal United Hospitals in Bath, Great Western Hospital in Swindon and Salisbury District Hospital in Wiltshire (and other acute and aligned resources) and are using healthcare more than, or differently than, expected for their specific needs. The service will proactively work with a rolling cohort of people, using a person-centered, 'what matters to me' conversation and "team around the person" approach to better deliver against the agreed needs of that individual. The key objectives are: • Identify those at greatest risk of ED attendance and non-elective admissions. • Proactively work with a rolling cohort of HIU people, really understanding what they need. • Provide a de-medicalised, non-judgemental, shame and trauma informed human support to resolve any underlying social and emotional need which led to hospital attendance. • To coordinate wellbeing and connect with other services, enrolling them to help to get to the desired end. • Reduce ED attendances and avoidable non-elective admissions. • Impact positively on reducing the high intensity use of healthcare. • Reduce ambulance conveyances as a by-product of reducing ED attendances. • Improve people's quality of life. • Provide high quality and personalised interventions. • Promote self-care and health and well-being of people (health coaching) • Minimise inappropriate use of resources. • Enable people to have a positive experience of the service. • Support people to flourish through sustaining job opportunities, reconnecting with families, improving well-being etc. The initial contract term will be for 2 years, with an optional extension of any period up to 1 year. The maximum budget is £1,080,000 over the 3 year term. The annual budget for 24/25 is £360,000. The new service will commence on the 1st July 2024. Bath and North East Somerset, Swindon and Wiltshire ("BSW") Integrated Care Board ("the ICB") seeks to secure a provider who has the capacity and capability to deliver community eye care services for patients registered with a GP within the BSW footprint and in line with specified requirements. It is expected that the services will work collaboratively and have a harmonious and efficient relationship to provide a safe and co-ordinated eyecare pathway. The ICB seeks to work with providers who are committed to innovation and creativity in delivering good health outcomes. The scope of this tender covers 3 lots: Lot 1: Optometry Triage Referral Service Lot 2: Community Urgent Eyecare Triage Service Lot 3: Community Urgent Eyecare Clinical Management Service Lot 1: Optometry Triage Referral Service Working with the Referral Service, the Optometry Triage Referral Service will provide registered optometrists with relevant experience to enable the efficient assessment and triage of ophthalmological referrals to secondary care. Lot 2: Community Urgent Eyecare Triage Service The Triage Service will ensure people who have eye-related signs and/or symptoms that are perceived by those presenting to need prompt assistance with their concern. Via a single point of access the Triage service will provide remote/telephone triage directing patients with signs and symptoms of recent onset urgent ocular presentations to a telemedicine consultation or arrange for a face to face assessment and Clinical Management. Lot 3: Community Urgent Eyecare Clinical Management Service The Clinical Management Service is for those who have been referred via the Triage service. Patients will be assessed, agree a management plan which may include self-care, therapeutic recommendations, referrals and signposting. Through remote/telephone triage, telemedicine and a network of community-based providers/practitioners providing face to face appointments, patients will be able to gain prompt access to an eyecare consultation and, in most cases, a care plan for the patient to either self-manage their ocular condition (with access to appropriate OTC topical medications where appropriate from Pharmacy), with advice and guidance. Telemedicine will reduce the burden on patients visiting GP surgeries, pharmacies, high street opticians and secondary care facilities physically. The use of telemedicine will mean many people can receive an eyecare consultation from their home. When telemedicine is an inappropriate mode of service delivery, patients will be signposted to a face to face appointment with a community-based practitioner or signposted to hospital ophthalmology services or A&E in the first instance. TUPE is likely to be applicable, further details can be found in the procurement documentation. The contract term for both lots will be for 3 years with an optional extension of any period up to 2 years. The maximum contract values per lot are: Lot 1: Optometry Triage Referral Service - £85,000 per annum, giving a contract value of £255,000 for the initial 3 year period and £425,000 if the full extension period of 2 Year is adopted. Lot 2: Community Urgent Eyecare Triage Service - £195,000pa giving a contract value of £585,000 for the initial 3 year period and £975,000 if the full extension period of 2 Year is adopted. Lot 3: Community Urgent Eyecare Clinical Management Service - £176,000pa giving a contract value of £528,000 for the initial 3 year period and £880,000 if the full extension period of 2 Year is adopted. Bath and North East Somerset, Swindon and Wiltshire Integrated Care Board are seeking commissioner a digitalised end to end patient management system to support the referrals, assessment, approval, contracting, payment and case management of NHS funded care packages. Information is required to seamlessly flow between processes by connecting the entire lifecycle into one platform and through an enforced digital workflow. The system needs to facilitate the services to deliver enhanced transformation and is expected to become the primary source of record keeping for all aspects connected with individuals receiving NHS funding. The ICB manages an active caseload of around 3,000 individuals across AACC, FNC, ABI, Personal Health Budgets, Mental Health, and Learning Disabilities. The caseload includes both adults and children and fully funded and joint packages of care. It is clear with this caseload, and the inherent complexities associated with the services, that an efficient and effective digital system is essential. One of the key requirements for a new system will be the need for it to be sufficiently flexible to support a wide range of services outside the traditional CHC / Funded Nursing Care (FNC) / Fast Track (FT) / Continuing Care (CC) agenda. Service wide transformation will be implemented across all areas including clinical, business, performance, finance and contracting and commissioning, as well as other clinical pathways for example Acquired Brain Injury, Mental Health, and Specialist Placements. Enabling better integration between systems across Health and Social Care is also critical. These services cover and links many elements of the health and social care system. Therefore, interoperability is of paramount importance, allowing for the ICB and wider Health and Social Care teams to share and view records. This will support improved accessibility and visibility of data which in turn will enable more efficient and streamlined workflows for all teams and a better person experience. The contract term will be for 3 years with an optional extension of any period up to 2 years. The contract value is £250,000 per annum giving a maximum contract value of £750,000 for the initial contract period and £1,250,000 if the full extension period is adopted. NHS Bath and North East Somerset, Swindon and Wiltshire Integrated Care board (ICB) is seeking to secure a Prime Provider who has the capability and capacity to deliver Postvention Bereavement Services to residents or people who are registered with a GP, within the boundaries of Swindon and Wiltshire. A core feature of service provision will be the continuous involvement of people with lived experience to inform and influence ongoing transformation and development. This will be achieved through not only formal opportunities but also through comprehensive analysis of data and outcomes related to people seen within the services. Access Information will be gathered by the service provider and uploaded into the Mental Health Services Dataset (MHSDS). This will, as a minimum, allow number of contacts (at patient level) to be measured. Through joint work with partners and using outputs from the MHSDS, the provider will demonstrate how their service offer has provided timely access to the service and, where relevant, timely transfer to other partners. This will include: waiting time to first contact, waiting time to first intervention and waiting time for any onward intervention required. They will monitor this quarterly in conjunction with commissioners or more frequently if required. At first contact, staff will be expected to conduct a support conversation with the individual. This support conversation will establish individual goals and anticipated outcomes that the person will be supported to work towards. This will be recorded on the agreed patient record in order that the information follows the person and they only have to tell their story once. TUPE is likely to apply. The contract term will be for 3 years with an optional extension of any period up to 1 year. The contract value is £61,600.00 per annum giving a contract value of £184,800.00 for the initial contract period and £246,400.00 if the full extension period is adopted. The service will provide intensive, individual support aiming to help people with a Severe Mental illness (SMI) to gain and retain paid competitive employment with the implementation of the evidenced based IPS employment approach. The service will be for people aged 18+ with an SMI open to secondary mental health care and also Primary Care. The geography covered is Bath, North East Somerset, Swindon and Wiltshire. The contract will be for an initial 3 year term with 2 optional extensions of any period of 1 year per extension with service commencement on the 1st April 2026. The Commissioners confirm that whilst they are seeking a Prime Provider, they welcome partnerships, consortiums and/or sub-contractual models, which is at Bidders' discretion. As laid out in the tender documentation, this is a two-lot competitive process under the Provider Selection Regime 2023 (PSR) regulations. The service will deliver NHS Health Checks to eligible adults aged 40-74 years who have no previously diagnosed cardiovascular disease and who are registered with a GP and/or reside within BSW footprint. The outreach model will focus on improving uptake among individuals and communities who experience barriers to accessing Health and Care services, with particular focus on residents living in areas of higher deprivation (Core20) and locally identified PLUS groups. Delivery is expected to take place in accessible community settings and at flexible times, including evenings and weekends, to maximise engagement and reduce inequalities in cardiovascular risk detection. The contract term will be for 12 months with a contract value of £120,000 inc VAT. If appropriate to do and subject to ICB Governance, the Commissioners reserve the right to add funding to the contract over its duration to accommodate additional activity.
