The Hospital Discharge and Peer Mentoring service will work closely with key partners to reduce the likelihood of individuals being re-admitted into hospital within 30 days and offer support, both professionally and peer-led, to individuals who have been admitted to hospital with alcohol misuse related problems and to ensure that individuals with a treatment need enter structured pharmacological or talking therapy treatment. Sheffield City Council expects that the successful provider will: • Reduce the number of unplanned inpatient alcohol related admissions for those who have frequent admissions to Sheffield Teaching Hospitals (STH). • Reduce the number of incidences of relapse and readmission within 30 days of discharge for this cohort. • Offer transitional support (professional and peer) to individuals who have been admitted to hospital with alcohol misuse related problems. • Support this cohort to engage in alcohol treatment and recovery services to start and maintain their recovery by building resilience and recovery capital. • Raise awareness and change attitudes about alcohol use, influencing community norms about the use of alcohol. • Develop strong partnerships working with other relevant services offering alcohol support and treatment to support this cohort. • Actively champion a thriving recovery community within the city. • Engage with local communities in public spaces and at events, with the goal of conducting brief interventions to provide information on safe drinking limits and signpost and refer people to support/treatment services. • Provide opportunities for paid employment for those in recovery. • Provide a safe space for this cohort to present and receive support, taking into consideration the potential for certain spaces being intimidating for this cohort of people. • Utilise outreach in the community to engage with service users.
