The Council wishes to contract with a provider to deliver a responsive Pathway 1 service on a pilot basis whereby the individual who is discharged or stepped up (i.e. admission avoidance) home to the community at which point they will be assessed by a multi-disciplinary team within 24 hours. Reablement in the community or Discharge Home To Assess through Walsall's Intermediate Care Service (IC service) are in line with Home First principles which aim to enable people who are discharged from hospital to recover, reable or rehabilitate. The key elements of the service specification are:- o following a description of the presenting needs on the ward facilitate the prescription of care/support in the person's own home (rather than prior to discharge) within 24 hours of hospital discharge o enable the independent sector provider to be a key part of the MDT process and develop the local responsive service model in the spirit of co-production o provide infrequent 'step up' referrals to avoid hospital admission with a subsequent prescription of care being undertaken in the person's own home o provide infrequent 'end of life' care and support packages to enable an individual to die in their own home o assist in the development of a responsive service referral and assessment process o assist with the development of the local technology in care offer o utilise the functionality of electronic care management system to record goals/outcomes o evaluate the success of this pilot including the collection and analysis of customer feedback The block number of hours will grow incrementally from week one of the contract period:- o Week One: Maximum of 100 hours o Week Two: Maximum of 200 hours o Week Three: Maximum of 300 hours o Week Four: Maximum of 400 hours o Week Five onwards a maximum of 500 hours per week: The volume will be reviewed by the end of week 4 and will begin to reduce from week 21 onwards. The volume will not exceed 500 hours per week unless agreed with the Council and the volume will be monitored and reviewed on an ongoing basis. The Council is looking to appoint one supplier for this Contract. The procurement process will be subject to the Light Touch Regime.
