MBC One You Kent - Maidstone (Triaging Referrals)

Description

Maidstone Borough Council (MBC) is looking to procure the Tier 2 Weight Management referral pathway under the One You Kent Service. One You Kent is an integrated adult healthy lifestyle service which has a holistic approach towards individual lifestyle behaviours such as weight loss, stopping smoking, getting more active or reducing alcohol intake. The service will be responsible for receiving and recording referrals via a client management system, liaising with clients through face to face and/or telephone consultations, offering access for weight-loss programmes both in-house and through an external provider and signposting to additional support services. We have a dedicated contractor in place to deliver a Tier 2 Weight Management programme with a physical activity element but require the chosen provider to run an additional provision to meet demand and allow residents to have a choice. For example, to run a Tier 2 Weight Management programme with limited to no exercise element. We would like to see a streamlined service for our residents to ensure they have the right support at the right time.
